Great podcast Mark, well explained and detailed. I had radical prostatectomy in 2007 at age 59 yrs with a very good surgeon. 7 on gleason with cancer breaking out from prostate gland on one side. So nerve sparing but I lost one nerve. Today i am sexually active and enjoy it with my wife. Use 5mg Cialis 90% of the time. Have spontaneous erections but not frequently or good as before surgery. Mine took longer to recover but it did and enjoy sex today 17 yrs later. Continence good initially has worsened - a bit of stress continence now with harder exercise., eg running,twisting - its manageable. I’m lucky no return of cancer. Keep podcasting.

Mark, I had prostrate cancer diagnosis 2.5 years ago. Had it removed 2 years ago. I was very lucky to find your videos before my surgery. Your videos have been so helpful, almost everything you showed and said in your videos I experienced, included the sex stuff. Thanks to you I have known what to expect all the way. SORRY about your recurrence, best of luck!

Never had prostate cancer or any other problem with my prostate, but listened to this awesome gentleman's honest/candid presentation to learn what if .... I feel better knowing that prostate cancer doesn't necessarily mean sex is over!

Had radical prostate surgery 2008. Still no change in PSA. Wife and I have adjusted to our change. I wish you well on journey.

Do you feel that the prostate surgery was a failure. Most of the people I talked to say it was a miserable failure. Only to find out there are other successful prostate cancer treatments. You have to do your homework.

After prostatectomy sex was challenging. I used injections first and after minutes hard as a rock. Then cialis, which worked nicely. After a short while, I didn't need assistance. I did keep a good amount of Cialis because it was paid for by insurance, and it works beautifully. I would like to add, make sure you check for prostate cancer at least once a year. Prostate cancer found early can be treated, finding it late could be fatal.

Thanks for sharing Mark. I am 8 months post surgery. I came across your videos a few weeks before that. They have been very helpful. My surgery went well. I do orgasm with the help of my wife or using toys. So far, no ability to inflate. My doctor says it is still very early. I am scheduled to learn how to use the shots in a few weeks. I am a little nervous about giving them, but I am told the needle is very small. I am looking forward to having penetrative sex. Also, I am not quite dry yet but making good progress. I am not discouraged as I was warned that full recovery would take three years. I am impressed by how you have been able to heal, and very happy you created this support community. I encourage people to hang in there. I am not yet where I want to be, but like you I am at peace with the road.

I wish my surgeon had suggested searching RU-vid for men's post-op experiences on RU-vid. We talked but I was shy and we did not communicate very well, post-op., especially putting a needle in. This is the very best podcast on post-prostrate surgery I have seen in six years. Most men I know that have prostrate surgery get divorced in a year. I am lucky. My wife knows that marriage is more than sex. -- I wish my surgeon had sent me to RU-vid, especially your podcast, which is viable and has positive results after all these years. One of the post op problems is (I'm not going to post that. Just do lots of Keagle Exercises....) Normally, I would not respond to this type of podcast (privacy) but your podcast can benifit so many men, wives and surgeons that I decided to just do it. Thank you. Reluctant poster. God bless!

Good advice for the doctors out there. Either they don’t know how to talk to their patients or don’t have the skill. When I first got diagnosed, I didn’t have any clue about prostate cancer. I thought my life was over. This guy, so called doctor, gave 29:21 29:21 me the news abruptly, for a second, I went blink. Maybe the nurse saw that, he apologized for him for giving such a bad news on my birthday.

This is a great video and very much my experience as well. I had my prostate removed almost 3 years ago and my physician was able to save most of my nerves. I was able to have sex 6 weeks post surgery but it was definitely not the same as my previous experiences. My physician did tell me that this was a good sign and that you have to keep trying in order to recover fully.

Again, I could never express the amount of gratitude I feel toward you for openimg up on this part of the subject. At just past 1 yr I had my first erection, not a great one , but something and I remain encouraged.

I was diagnosed with the cancer in October 2020 and decided to watch and wait, however my PSA level rose and it was suggested I make a decision. I chose radiation and my treatments (28) were from November 2022 through January 2023. I am 65 and single and have always enjoyed self satisfaction. After the radiation, I have been able to ejaculate even though I have not experienced an erection. The ejaculation and orgasm is nowhere close to what it was and the substance is almost like clear water. Kind of feels like my manhood was taken from me but I guess it beats the alternative 🤷🏼🤷🏼. It was nice to be able to vent here and hear your story as well…

Thanks for being so open and honest Mark. I am 7 months post a Radical Prostatectomy. As you touched on the number one objective when diagnosed with prostate cancer is to "get this cancer out of me"! The second objective is to regain bladder control which for the first few weeks is quite challenging but then this gets notably better to the point where other than some slight leakage I was able to do after 4 months. Regarding penile rehab, I have been using a pump and when adding Cialis with the pump and ring for sexual intercourse have had mixed results. I have also tried the Caverject injection (not the most pleasant experience in my case). I have also upped my exercise regime adding a 5 klm run which I do every Saturday morning and feel physically and psychologically better for this. It is early days regarding sexual function and feel positive about the future. Thanks again for sharing.

Joe, time seems to offer great improvements, but each of us will have a different experience. Half of the battle is putting our minds in check, and looking for both hope and acceptance when our side effects (incontinence, erectile function) are lingering longer than expected...or persist. My attitude in life is to always find the way to make the best of any situation, and when cancer curing surgery leaves us with these difficulties, find a way to make them a part of your happy life.

It’s so happy to find this video. I cannot have any satisfying answers from my physicians about sex. I had radiation therapy and , after recurrence from prostate cancer, they put me on hormone therapy. I’m so depressed about this, I’m seriously contemplating balling out of life. I withdraw from society. Thank for talking about something that I find out no one wants to talk about.

Thank you Mark for your thorough and detailed account of your experiences - I’m 75 and I think I’m in reasonable condition. Ihad my prostatectomy 7 months ago - I think it went very well - my incontinence has been improving and although I am able to get post-op orgasms my erectile function is not what it was - I did my Kegels exercises religiously for months but I’ve balked at the idea of the pump, injection and viagra options - I think I might have to step-up my recovery program again now - having listened to your humble but detailed account of your experience - with keen interest- thank you 🙏

Thanks , Mark Great video but more importantly great subject.. So I had prostate removed in 2021 at 51 to be honest I felt that life was over. I found your prostate journey and I started following you and knowing that you had been through and sharing your story really made a difference for me. 3 yrs later no erection and still leaking like flood. I broke it off with my girlfriend 6 months after surgery. Like you my number started increasing also.. I pray you can get that turned round.

Thanks for the great video. In my case, i have been on hormone therapy, Xtandi and Orgovyx for the past year and no inflation down there. Have you ever had that experience while you were on hormones or have you ever been put on one? btw- i have had my prostatectomy in Feb 2021

Mark, thank you for a VERY informative podcast on sexual function post-surgery. You covered a lot of topics that, as you mentioned in the beginning of your video, are not covered by doctors. I’m a little over 2 years post-radical prostatectomy and I’m able to achieve full erections but still prefer to use a low dose of viagra to help. Also, as you mentioned, I find that my penis becomes fully erect and rock-hard, however it is a challenge to maintain the erection, especially with my partner. Another thing I want to mention that my doctor did not tell me about prior to my surgery, is that I saw a significant decrease in the length of my penis. I asked my surgeon about this and he said this is normal and the length would start to come back in the weeks/months following surgery. I would say my penis went back to 95% of its normal length (both flaccid and erect), but there is still a bit of a loss. Not to toot my own horn or sound like I’m bragging, but fortunately I was very well-endowed prior to surgery, so the slight loss in penis length has not been a big issue for me. Also, the circumference of my erect shaft is the same as before and there was no noticeable loss of thickness after surgery. Once again, thank you for your continued informative podcasts/videos and discussing your personal experiences with your viewers. You’re certainly making a big difference. Gleason was 3+4 , robotic prostatectomy , not much incontinence so far , im back training but ED is still an issue , hoping for the best , fingers crossed. Had robotic prostatectomy in October 2021 at 55 in Chicago. Had full nerve sparing and everything is honestly 100% back to normal as before 2.5 years later except for of course dry orgasims. Now don't get me wrong, I am greatfull to be cancer free and have normal erections and even the orgasims feel pertty much the same however I greatly struggled with the loss of expelling seamen as a man. Yes I was aware of this before surgery but underestimated the loss of ejaculation. I now realize the component that turned me on most with sex was the feeling of being able to impregnate a woman. Still greatfull to be where I am at today.

Yeah, this is one of the greatest drawbacks of the surgery. We've been dealt this hand, and every time I think about some of what has been lost (ejaculation), I remind myself of why this had to be done. It certainly turns a new page in your life, and as much as this change is one of disappointment, we now have removed the cancer that could have ended our lives early. That's the focus I try to remind people of.

The irony is I was diagnosed with Gleason 6 low volume so active surveillance was the first choice but the prevailing "you have cancer and you're going to just wait until it gets worse before treatment" was a dominant force both externally and internal. On one hand, I am happy not to have to be doing repeat needle biopsies and imaging but can't help but wonder if I would have been one of the few where I never needed further treatment. In hindsight, I may have chosen active surveillance to keep normal ejaculation however by knowing my personality, I'd also be worrying about spread outside the prostate. It's the traditional "what if" game, of course. Before active surveillance was even an option, Gleason 6 was treated the same as higher grades with surgery or radiation. As we know, the extent of the disease is not fully gauged until the prostate is out and in Pathology and my Pathology report showed 1% adnocasinma of the entire prostate which is really low so that also plays into my thoughts.

My biopsy came out as low grade 1 but was spread around the prostate so was told it was either removal or 30 days of radiation to destroy the prostate with the cancer. The pathology report was that all the cancer was the low grade 1 and told by the urologist that he was 95% confident of cure. But worst part is there will never be anymore ejaculating which was the most exciting thing in sex life.

Mark thanks for sharing. I had robotic prostatectomy in 2018 at JHU. Thank God cancer free ever since. Surgeon was world class Dr Allaf. Urinary continence fine from the get go. Nerves spared successfully Erectile function returned starting around 3 months with help of viagra. I experience occasional soft erections that do not allow penetration. Seems to run in spurts. I find that penis ring helps. Silicone ring placed with help of wife at base of penis. Prevents blood from escaping to allow 8:31 maintain erection for full penetration.

Great post Mark. I'm almost three weeks post surgery and doing literally the same as you. I honestly believe its a mind over body thing and you have to retrain your brain. I've been able to get to so far about 70% of normal erections without using anything, drugs or pump. And every day I get a little bit further, so it may take a few more weeks, or months but I do feel 100% erection will be back sooner than later..

I had mine frozen when I was 52 my Dr said you will be impotent I thought that's great encouragement doc good confidence booster I still have some sort of function but not like before shooting blanks

Hi Mark, i am on hormone therapy, the ability is still there but interest is zero, strange thing is I feel so much calmer and relaxed to the point I even wonder how my life would have been different if my mother had had me castrated at birth, maybe easier for me as I live alone and no partner to worry about but maybe the thing most men worry about the most is not such a big deal, happy to be alive and getting on with life Phil

Is the hormone therapy killing the cancer ?

Yes the hormone therapy does shrink and kill cancer cells, but unfortunately maybe not all the cells get eliminated so at some point the hormone therapy stops working and the cancer starts to grow again , as I understand it at that point I would move on to second generation hormone therapy , see how long that lasts

Great video thanks mark I am 2 years post removal same experience I am pushing 70 so I know it may take longer to heal .viagra works fast but it does not last long. my partner is trying and we work together and hope for best I take a beet powder drink every night which has improved nocturnal erections PSA testing every 6 months. looks good for now

Thanks again Mark for doing this podcast and I hope you are sucesfull in your endevours. Here is my experience three weeks post op. My Dr told me shots that he would give me. hes not talking about Cialis or Viagra at all. Im basicaly not being told much yet. I dont know if im getting bad care from this Dr. As I dont know the sucess rate of my dr with other patients. My Dr is a bit hard to talk to about most everything , he basicaly wants to tell me and wants to get me off the phone ., Any advice

Hi Mark - thx for these podcasts. Have you ever considered to get an implant or do you have an opinion on that. I’m 18 months post surgery and only a pump works for me but it doesn’t feel natural.

Summer 2020, robot assisted prostatectomy. Wish I'd tried radiation. I'd like to see stats on surgery vs. radiation re: post op sexual function as well as 5 year reoccurrence...4 years out PSAs are clean. 🎉... Ive had no ability to have penetrative sex, no independent wood ANYtime. Injections made oral sex possible (with a patient partner). But you can't use that stuff indefinitely...Then there's the urinary control. I'd rate myself mildly incontinent. I use washable padded underwear. They amount to a light pad per day typically... I'm mad at myself for not being able to ask better questions AND GET the answers from my surgeon...if my situation is typical - theres room for alot of improvement via providor - patient information transfer...but then, i guess it's unavoidable... I mean what hospital or surgeon is gonna tell you the statistical truth about their outcomes?

My understanding of radiation over surgery is, with radiation eventually you will experience the same side effects of surgery. You don't need a prostate, yes you will experience loss, but is being alive worth the (maybe) temporary loss?

I had Dr. Jack Steele do just that... he provided me a chart that showed "19" yrs success at the time...very impressive stats...re radiation treatment...both photon beam and seed implants... might have felt a bit lethargic during the treatments...but other wise just fine...had sex within a couple of wks after treatment... Was told sexual function...would be pretty much the same the way I went in...was going to be the way it would be at the end...of treatment... pretty much the case... Considered Robotic surgery...but saw the drawbacks...recovery...incontinence etc...plus in 2008'...it was still fairly new...with little historical data... I'm very comfortable with my radiation cancer treatment... and everything is still the same... I was "58" yrs old ...in 2008... btw...my Gleason # was said to be mildly agreessive form of cancer... and cancer was there on both sides of my family history... Hope it helps...

@@mygrrrlz What radiation does...is that it kills the cells...including the cancer cells...surgey removes the bugger... but does not kill the cancer... Correct...we do not need our prostrate...as a matter of fact...initially wanted surgery...ASAP...but when I settled down and realized it was more of a reaction to the cancer...and considered all other scenarios...including it matasticizing to other organs...bladder...colon etc... I opted for radiation... I'm glad your surgery went well...and has continued to do so... btw... miss my prostate...lol...

Can dry orgasm , post cryo prostate surgery, be reversed for the path of the seamen. It wasn't removed it was frozen.

I'm having severe Urological issues not related to the prostate per say but I have many issues that guys go through after prostate cancer and I started with all these issues at 36 and was truly imbarrassed and would cry going so much to the urologists and all the surgeries I had and have and having to self cath since I was 36 and now 44. But you are helping me so much by relating your experiences of which I'm going through since then and now sexually and urination and erection issues and all these things that us guys dont like speaking openly in general and to your doctor and I'm still imbarrassed and it's so hard for me. Its hard being checked and dealing with all the issues being young and a guy. So thank you

My prostate was removed in Sweden in 2010 it was great success sex was normal until the doctors told the found a microscopic tumar..I go for a check up every year ..till today I still go for a check...so,I started radiation in 2020.......2021 my results were great out of danger...but it's now it's alread 3 years and 4 months I've got ED problem no erection ever tablets don't work... what to do now???

Hey, Mark! TOP content, as always! ⭐ Thanks for sharing, following your videos since my diagnosis last year. Fast-forward: 45yo, R0 now after robotic surgery and biopsy coming clean (no increase in Gleason score, 3+4). I'm about 12 after surgery and catheter removed at 5th day. A few drips of incontinence per day and thanks God getting natural morning erections but still hurts a bit. My question: do you think it's still worth taking little doses of cialis? I forgot to ask that to my Dr but I already consider myself is a luxurious position, thanks God and the doctor experience! 🙏

@@suggarface1 Well, remember that at the end of the surgery, your urethra was connected and "fused" to your bladder. You want this to heal well, and not exert it. Another effect of this surgery is that your bladder is pulled down a bit to cover the space previously occupied by your prostate. This is a tight connection, and during full erections you may feel something "pulling" taught inside. It would be wisest to ask your doctor for his opinion on when to resume sex with penetration. I waited a full year, but just being over cautious.

Give it time to heal. Was 2 years for me after nerve sparing prostatectomy until I got back erections in the middle of the night. Generic Cialis is under $1 a pill. If you have had nerve sparing surgery, Dr. will recommend shots or low dose cialis or viagra to promote return of blood flow which is necessary for erections. As you heal, normal erections will return and then Dr. will recommend higher dose cialis or Viagra 30 to 60 min. prior to sex.

How lucky are you so glad that you can have an erection I am out of complete radical prostatectomy, and. no erection for me. It’s sad but they I had stage three cancer and they took everything out of me. Well that’s the death of my sorrows. Happy for you.🎉

Gerry, we each have to find ways to cope with our own situations. Don't confuse my enthusiastic attitude with there being lack of sadness and sorrow...we all have it in different degrees. The best approach anyone can take is to accept where we have ended up, but being grateful that there was a good reason. I would trade a life of incontinence and erectile function for a life stricken with horrific cancer.

I was diagnosed with prostate cancer in the summer of 2019. Around that time I'd just met a woman who was interested in me and my feelings were mutual. The very last time I ejaculated was in November with this lady after a week of radiotherapy as some of you might know radio lasts for 6 weeks. I contacted Macmillen about a year ago to find out what happens to semen when one ejaculates, they said it disperses around the body, (obviously internally) my question to you is , do you agree with what I was told? From my experience my sex life hasn't changed too much to be honest. On you tube there's a lot of discussion about non ejaculatory orgasms and tantra of which I find very interesting. By the way I'll be 73 in October and I feel more alive now more than ever. Thanks for your time. 😎

Robert, it appears that you were told correctly. When you have an orgasm, there is still the release of sperm, but I am not sure if it still combines with with fluids to make semen. Whatever is released by the tested is absorbed by the body.

Saying that surgery is the preferred way if you're young enough and radiation is more for older men who can't tolerate surgery goes against everything I have recently learned from some of the best doctors in the USA. Surgery may be the best option if it is high risk and has spread. Otherwise, it is a matter of choice based what you think is going to give you the best outcome and highest long term quality of life. Surgery certainly guarantees that you get it the first time, but once they cut something out of you, it is gone forever, not to mention you're shooting blanks for the rest of eternity. Surgery also has a lot of down time and then physical therapy for months to help you control the stream.

Vicos, you have the wrong perspective...and I am not sure I agree with your statements. Firstly, the leading prostate cancer experts in the country and around the world will ALWAYS recommend surgery as a first treatment plan, especially if the cancer is low grade. Radiation is generally the treatment used if there is a recurrence. If after radiation there is further recurrence the treatment is via medication. In more advanced cases, especially with Stage 4 or metastasis, chemotherapy may be used. You seem to place sexual function above combating cancer, and this is the single worst mistake that some men make. Your treatment decisions should be based on the cure of the cancer, not retaining sexual function or continence. If treatment is early enough and done properly, you should retain both. As for shooting blanks, most men get prostate cancer well beyond their child rearing years.

@@prostatecancerexperience Well, I recently spoke with three oncology doctors (medical, radiology, and surgical) at Georgetown Univ. Med. and not one recommended surgery as the first treatment plan. They presented each as an option with pros and cons and leave the decision to me. The only thing all 3 were adamant about was that active surveillance was not a wise choice for me. and yes, quality of life is of paramount importance. I guess we'll have to agree to disagree.

@@VicosGet a second opinion, and from a different medical group. You have one chance to do this right, and you don’t want to make a wrong choice. Radiation is possible after surgery if there is recurrence, but surgery after radiation is not recommended.

Hi Mark! Thanks that you share all this info with us. I am 30 days after my operation (20 after catheter removal). I feel that I want to "do something". I have sexual irritations and thoughts, no full erection of course (let say a 30-40% of the normal that lasts some minutes). The solution is masturbation at first. Did you feel any pain while masturbating or reaching orgasm? You also followed this solution after the 30th day, right? Any bad sensation or pain? Thanks again for sharing your experience.

Xaris, at 20 days post-catheter it is ok to try to masturbate, but cautiously. You will be healing for many, many months, and you don't want to compromise your surgical areas. Your urethra has been reattached to your bladder, so you want to avoid anything that can tug in your pelvic area. If you can masturbate to orgasm with no erection, please keep doing so. That sends a message to your brain that sexual function needs to be restored, and the brain will get working on trying to restore erectile function. This can take many months, but if you nerves were spared you should slowly start to see more of an erection, even just a small one at first. There is possibly some pain with orgasm, as the orgasm is a muscle convulsion. Having pain is always something to take note of, but expect it will diminish over time.

Thanks for your videos. What if you had surgery and your lymph nodes were removed, and your entire groin was swollen? Like the testicles and sack are the size of a grapefruit, and your penis barely shows outside of this swelling? Of course it has gone down as time goes by, but at what point is masturbation possible? How many weeks out?

A relative of mine had prostate surgery. The doctors removed his prostate, he has numerous serious side effects that the doctors never explained. He has constant urine leakage , with the removal of the prostate the doctors severed several nerves and now he has no erections. The biggest side effect that the doctor never explained was with the removal of the prostate the doctor had to remove part of the urethra. Well , the penis is attached to the urethra and with the partial removal of the urethra and full removal of the prostate my relative states a large portion of the penis is gone forever. He is is livid and on the verge of a nervous breakdown. My god there is seed implantation with radiation treatments and NO SURGERY. The doctors that are doing this prostate surgery are nothing but BUCTHERS . My relative also said the Doctors are making him take PSA tests . The doctors told him he will have the surgery to remove the cancer . What the hell is he is taking more PSA tests . Has the cancer returned? This prostate surgery is not a good procedure. Men out there do your research there are better methods to handle prostate cancer . Everything Mark is testifying is not what my relative experiencesd . I am very suspicious of his videos.

David, I have a lot of compassion for anyone who has to go through this process. The anger should be directed at the medical care received, and not at the proven medical science. This is why I tell everyone who is about to get treatment for prostate cancer to do their research and speak to multiple doctors for a consensus.

Radical prostactectomy on urologist recommendation in 1999. Biggest mistake of my life. No erections since then despite trying everything so wife divorced me in 2001. Wife demanded divorce and started sex with other men behind my back. Divorce wrecked our daughters. 14 year old became a heroin addict and moved in with heroin dealer and 16 year old daughter stopped talking to me. No ability to have relations with women since. Alone forever. Don't take Lupron either.

So sorry. It appears that you was given female hormones. My husband was put on a trial and given female hormones but he felt irritible, his breasts grew a bit and nipples became sore just like a pre menstrual woman. He gave up during the trial because the symptoms were unbearable. The symptoms disappeared when he stopped.

If the tumour remains insitu there is 100 % cure. My husband had a prostatectomy but no chemo. My husband was diagnosed in 1997. There were two tumours in my husbands prostate, one of which had started to break through the prostate. As a result clear margins were removed so nerves were damaged. I think started to break through is close to the edge of the prostate not outside. He survived prostate cancer. 11years later he was diagnosed with melonoma which had gone to the brain. The medics say the melonoma was a separate cancer . I read recently that melonoma could be as a result of prostate cancer. People believe if a person has had cancer they are more susceptible to a different cancer.

Mary, every man has a different outcome. Yes, for some, the after effects have an impact on continence and sexual function. But, it comes down to one's outlook on life, and what's truly important. Left unchecked, prostate cancer can lead to horrible death (bone cancer is very prominent). So, come what may, eradicating the cancer should always be primary, and we each then have to cope with what comes after.

Good podcast but saying surgery is the gold standard is a bit misleading. The option between surgery and radiation is more complicated depending on age, extent and a number of other factors. You run the risk of viewers thinking that a prostatectomy is the best choice when it may not be for a number of people especially now with many new radiation techniques available.
