If you have a hearing loss in only one ear, a traditional hearing aid may not be your best option. Depending on the severity of that hearing loss, you may require a different treatment all together. If you can no longer understand speech in your bad ear, amplifying it with a hearng aid will not help.
When you have no usable hearing in your bad ear, and good hearing in your better ear, you need a CROS device. This stands for Contralateral Routing Of Signal. These devices look like small hearing aids and transmit sound from your bad side to your good side. This helps you overcome the Head Shadow effect and hear speech from your bad side, on your good side.
If you are deaf in your bad ear, but also have some level of hearing loss on your better side, your best option may be a BiCROS, or Bilateral Contralateral Routing of Signal device. This is where the hearing aid on your better side receives sound from the transmitter on your bad side, but also amplifies it for the better ear that also has a hearing loss.
You may also have a surgical option that is called a BAHA or Bone Anchored Hearing Aid. This requires he implantation of a metal post or magnet into your skull by a surgeon. You then attach a Sound Processor that picks up sound from your poorer side and vibrates your skull so your better hearing ear can hear it. Sounds like magic, but any vibration of the bones in your head can be heard by your Cochlea (inner ear hearing organ). This is called bone conduction. There are even headphones that use this type of technology!
At the end of the day, just because you can't hear in one ear, doesn't mean that you don't have treatment options. You just have to find a hearing care professional that is comfortable with this type of hearing loss and knows what they are doing.
Ultimately, any time you are diagnosed with a hearing loss in only one ear, or an asymmetrical hearing loss, you should be referred to an Otolaryngologist (ENT) to make sure it isn't something serious like a tumor. If it is, you will need to get that treated immediately.

I'm quite thankful to see inner ear technology has improved. There didn't seem much my doc could do about 30 years ago, or at least, didn't seem an option for me if it was around. I'm confident to get checked out again. Thank you for that.

I was also born deaf in the left ear..but I didn't realise I was born with it but I used to face many difficulties in noicy areas and sound environments like head shadow effect,dizziness etc...and I realised this unique challenge in my 13 year old time..I took a test of pure tone audiometry..and realise that I was born with SSD and the range of hearing loss is profound hearing loss..can you please tell me to over come this.. without baha..

Thank you for this video, I have SSD in my left ear, severe hearing loss, and just above the line of severe on my right ear. In two days I have an appointment to have a hearing aid from my hospital (Canadian) for three months trial, before I am give a perception by my ENT to go purchase. I'm excited to be able to hear again, but nervous that I won't be able to get the same treatment you recommend.

I'm deaf in my right ear but the past few years, a few times, I've heard a high pitch ring. Like some one turned on an old TV. It doesn't makes sense cuz I've had surgery and they told me there was something missing and it wasn't possible.

Was born with SSD (right ear). Use to tell my friends that locating sound is superhuman in my world 🙂 also, lots of noise further lowers my hearing capacity (cocktails, receptions, etc), people that know automatically walk on my left side (such as wife), arriving early at meetings is important so i can strategically sit for discussion purposes, etc etc Now planning to get hearing aid - we’ll see how it goes 👍

Thanks you for this video. The removal of a cancerous tumor on the eardrum caused complete hearing loss in my left ear at the age of 5. I am now 53 and never did get any hearing assistance for this. Now looking into the CROS hearing aid.

@@DrCliffAuD I have hearing loss in both ears due to bilateral Menieres which stated when I was about 58. I am now 67 and it seems to be stable in both ears for a year or so. I do have about 30% hearing lost in right ear (only way I know to describe it) and about 70% hearing loss in the left ear. Hearing recognition in right ear pretty good. Left ear pretty bad but still useable. I have Oticon OpN2 aids. My problem it seems I can’t get them set as good as I think they are capable of. I can hear pretty good but work recognition is hard sometimes. It seems just a small tweak would do it but they never can seem to get it there. Not sure if someone else would be a better option but my aids still have a year warranty with my audiologist who actually is on your list of “real hearing measurements” . Do these have all the equipment to do that and do all know who to use them as you do? Any aid is only good as the person who is programming it. Not sure where to go from here. I’m in church environments a lot and have played music my whole life. Even retired I depend a lot on my hearing. I don’t want to go through a 2 hour surgery to implant something on the side of my head or do ai think I am there yet or will be.

I left this same question on another one of your videos, but this video addresses my SSD issue exactly. I have mild to moderate hearing loss in one ear and profound hearing loss in the other. I went to an ENT/Audiologist group and they recommended either just 1 hearing aid (to save money) or a bicross solution. I also went to Miracle Ear and they recommended traditional hearing aids, saying that since my speech recognition is just as good when testing both ears together as compared to testing just my better ear traditional hearing aids would work well for me. I'm confused now on which to go with.

I’ve been deaf from 1 ear since I was little I was Probobly born deaf lol it sucks since elementary through high school my classmates would try talking to me during class but I would not be able to hear them I was really shy because of my deafness so I would just nod my head even though I had no idea what they would be saying.. I’m 27 now and I feel like this deafness has really affected me big time :( I know I talk different I’m guessing because of my deafness I’m very shy and have lots of anxiety it sucks I wish I could hear normal I Probobly only have about 5% of hearing on my bad ear compared to my good ear.

I am 51 years old and i have partial hearing loss in my right ear due to bad ear infections at school. While growing up, i had been seen by audiologist from LAUSD. Within the past 6 years, i've gotten free hearing tests from Miracle Ear. the 2nd visit to them was back in August of this year. I was told i need to get a pair of hearing aids and NOT what you've talked about. Miracle Ear is a well-established firm. Howeer, who has $6-7k to spend as opposed to getting lower prices ones.

Thanks for the information Dr. Cliff! Just wondering if, when using an Oticon CROS device, is the sound transmitted by the CROS upgraded by the "deep neural network" in the More aid worn in the good ear, or is the receiving device simply streaming the sound received by the CROS? I'm wondering if there is benefit in going with the higher tech Oticon or Phonak aids for the hearing ear simply in regard to sound quality.

Dr. Olson, Thank you for your videos. They are truly informative. I’m looking for a SSD hearing aid and currently am in between the Phonak Audeo B-R and the Untiron Moxi Now which I have been informed are from the same company and have a relationship similar to Toyota and Lexus. From what I’ve seen in researching these and sticking to the car analogy, Phonak is more like the Lexus while Unitron is more like the Toyota. Both have similar “engines” but the Phonak has more features. I’m interested in hearing your opinion on each of these devices or maybe seeing a video on this *wishful thinking*

Hi eljefe. The car analogy doesn't really work in this sense. Unitron produces high quality products just like Phonak. The analogy would make more sense if you took a Lexus, peeled the logos off, and stuck Toyota logos on the Lexus vehicle. Anyway, if you have SSD, your best bet would be to go with Phonak. They will be introducing CROS B-R in a month or two. Unitron has a go around for CROS/BiCROS by using a phone program, but in my opinion, Phonak leds the industry for SSD.

This was hugely helpful. Thank you! I especially appreciate the comment that not all audiologists are used to handling this. Do you have any sense of if the BAHA bluetooth, or outside piece, is getting smaller as we gain in technology? And as things change, can the device we get today upgrade with that technology?

There are limited upgrades that could happen with a BAHA system. The device requires Mass to be able to vibrate at an intensity that can transfer that vibration to your cochlea. I'd be surprised if they can make these Bone Anchored devices much smaller.

I was born deaf in one ear and managed with a hearing aid on my good ear which was damaged while diving. I was a teenager who did not realize the danger. Hearing amplification was the answer.

Hello Dr Clifford and thanks for your nice video clip. Here I have a comment: let's start from the point that the auditory cortex is super-imposed by the visual cortex in the case of SSD. In addition, the frontal cortex may indicate excessive effort to understand speech. These issues may lead to auditory cortex deterioration and excessive efforts on the understanding centers of the brain in case of SSD. All in mind, the activation of the auditory cortex is recommended in cases of SSD. Based on all these information, it seems that cochlear implantation is superior to CROS/BiCROS model in cases of SSD.

Wow,. Would cochlear implants be reasonable for someone with L moderate hearing loss who has had ossicular chain reconstruction on L ear due to cochlear adherence to skull, and mild hearing loss in R ear. 70 yr old.

You always answer my questions and I thank your or that. I just had another appointment at the ENT and at the Audiologist. He said, with my profound SSD a CROS hearing aid would not work for me because it would just make my good ear have more overload and have more background noise wich he said he couldn’t alter. He said I should get a Cochlear. I don’t want this. Now I have nothing and am not going out at all now because I cant hear in crowds or a group of people. Do you have any suggestions?

I spent 7,500 on my Widex bi cross hearing aids. I might have well flushed that money down the toilet. It had a uncanny ability to fail when I needed it most. Restaurant, driving in a car…you name it. It would fail. I’ve had mine in for service twelve times. No make that thirteen. They failed today. This technology is NOT there! Do not waste your money! Went in to get it rebooted today and they threw their hands up! Ask the doctor to please turn up the volume on my one working hearing aid and said she couldn’t! The doctor told me this! Don’t do it!

I sustained hearing loss when an enfant but didn't know until 3rd grade. I'd learned to read lips by then. Then a guy from Miracle Ear came to my home. Said I was more deaf in my right ear. I asked, "Then why is it when using the phone I can't hardly hear what's said using my left ear, but hear clearly using my right ear?" He said that this is not unusual. My question, is this true? My hearing loss is minimal, has never changed. So I don't need to wear hearing aids. However, if ever needed, I certainly would! I'm still wearing ear plugs due to my husband blaring the TV. If I don't, the sound actually cause pain in my ears. Thanks again for an educational and insightful video!

I never really understood why these options were never mentioned to me from my specialist. The last recommendation I had was 15 years ago and it was for a Ossicular chain reconstruction, which I declined. I am looking into BAHA/Bonebridge implant for my case. I hope to hear from the hospital that specializes in these surgeries to see if I could possibly be a candidate for them.

I had a bone anchor hearing aid on my left side. I was the first one to have it done. It started to get infected etc which resulted in removing the implant out about 8 years ago By the ENT Department. Ive tried hearing aid in my left side, it just flares up, swollen,red and painful. I can wear hearing aid on my right side no problem. I believe it's affecting my Balance etc. Do you recommend having the bone anchor hearing aid fits under the skin??? Ive been Diagnosed with vertigo/BPPV, Treacher Collins, I have very tiny Ears, constantly falling over, Depression, stress, tinnitus. X

Yes I was born with treasher Collins syndrome and I don't have any hole or an ear I really need whole metal headband hearing aid I don't have to see a doctor specialist to get one and I do have insurance too

Dear Doctor I had tinnitus on my left year 8 months ago and after injecting steroids I managed to improve left year around 30-45 decibels at 250 and 500 Hz. Hearing level for other frequencies above 500 Hz are above 75 decibels. I cannot hear anything if I go to a noisy area. Please suggest me a good hearing aid. My right year is normal except for frequencies above 4000 Hz (50-70 decibels) as I am closer to 70 years of age. Thank you

I wasn't deaf when I was born. But when I was around 12, I began to hear less from my right ear. At 18yr old, it is completely deaf. The cros option is not available in mauritius so I'm waiting for it. Im 30 now.

Dear Dr. Cliff, I am on the market for a new hearing aid. I have a total loss on my left and some hearing loss on the right, so the BiCROS you mentioned is perhaps for me? I also googled and saw various vendors provide different products, e.g., Signia has CROS/BiCROS, but both Phonak and Oticon only mentioned CROS on their web sites. I am wondering if the latter also offer BiCROS, or perhaps the Signia CROS/BiCROS is actually one product that can be used as either CROS or BiCROS? Finally, I am also considering the Costco Kirkland Signature 9.0 but not sure if this Costco hearing aid is capable to offer CROS/BiCROS?? Thanks in advance.

@@durairajan5190 very tough to judge direction of voices but I’ve learned that the only time that matters is when someone is at a longer distance from me. So if someone shouts my name from far away, I generally can’t tell where that voice is coming from. Doesn’t really affect me all that much.

I never quite understood the difference between cros and bicross; perhaps because I never heard it explained very well. I have no hearing at all in one ear. I've seen audiologists for years who, I have no idea why, discouraged me from a bicross. I did finally get one and the hearing is MUCH better if there is not background noise at all. If there is so much as an air conditioner - and any background noise at all, it is worse - even though these are supposed to be state of the art hearing devices. It's a major bummer, as the good ear is not that great.

Which Bicross hearing aids do you have? I have had SSD from birth and have learned to live with it. When I heard about cross devices many years later I enquired with my ENT in England and he also discouraged me from getting one as he felt I would not have enough benefit from it. I now live in Germany and went to see an ENT here as I felt I had lost some hearing in the good ear, it turns out I have a mild hearing loss there. The ENT here has said I should definitely get a bicross. I am due for a test run with Bicross hearing aids in two weeks and have no idea what to expect.

Thanks for this video Dr Cliff, 3 months ago, I got flu and loss my hearing on one side (right ear). My ent doctor diagnosed that I had sudden deafness. I got 3 time steroid injection and hyperbaric treatment, but it didnt work well. And he gave me advice to do 'cochlear implant', but i'm not ready yet. Is there other solution? Could i use cros or bicros device? Because my left ear works well and i still can hear from my right ear, although it isnt clear.

Yes, CROS is an option for you if you have normal hearing thresholds in your Left ear. No need to get a CI until you are ready.

Get them to do the steroid shots in your ear during or after the use of prednisone. I suffered SSHL about a month ago and started on prednisone. It brought some hearing back but my ENT began shots in my hear that have helped tremendously. I had my last shot today and will have a hearing test to confirm everything in 2 weeks. I hope for you to find much success!

I have a glomus tumor in my middle ear that was treated with gamma knife rather than surgery. My cochlea is functional in that ear. If I got a BAHA wouldn’t it just send the vibrations to the cochlea on that side rather than the contra lateral side?

I have CROS from phonak and I'm facing an issue: When my surrounding is loud (like sitting in a plane, in a subway or in a restaurant with loud background noise like many ppl talking) my hearing aid is not possible to filter those background noises. Which means I hear background noise as loud as my talking partner next to me which is bad because I can hardly hear them bcoz of loud background noise. Isn't there any kind of noise cancelling like in headphones? best regards Geek000

I’m having the exact same issue with my bicross hearing aids. My transmitter picks up way too much background and when it sends it to my good ear it makes it very hard for my to understand the person who is next to me.

Thank you for your series. Which option is your recommendation for children? My 6 year old has unilateral grade 1 microtia and conductive hearing loss in that ear. He has normal hearing in the other ear. His hearing test results show he is a candidate for either. Corrective surgery to open ear canal is not a desirable option for us.

With Microtia, it makes more sense to go with a Bone Anchored Hearing Aid.

Hi Cliff, I am 23 years old SSD in my right ear and left ear normal. I have not used any hearing device till date. I am very careful for my right ear hearing. I want know can I live without hearing devices throughout my lifetime? I see many celebrities like rob Lowe, jane Lynch and Stephen Colbert living fine with one ear in their 60's . What's your view on this ? ?

I am 60 and have had SSD since birth. It was harder when I was young but as I grew older it became easier to tell everyone about my deafness. So friends and family members walk on my good side so I can hear the,

Sir can you pls tell me a solution for the hearing loss problem iam facing well am from India am a musician , composser and a sound engineer I lost my left ear at my age of 20yers no am at my 40s but my main problem is it’s becoming very difficult for me to go for any live shows though it’s very hard for me to hear clearly on stage and in recording also the situation is the very same due to which I was forced to move a bit backwards from music works can you pls get me help to clear it out what type of treatment should I go to make my ear once again come to life

im 100% deaf in my left ear and i just had the surgery for the BAHA i get my hearing aid next week and i had the surgery 2 months ago since i was 6 i was deaf in my left and we got told there was nothing to help me i had the cross sytem but that didnt help at all but i hope this one will work i cant wait Please Read :)

If you truly have single-sided deafness, CROS would have worked. It was probably just fit/programmed incorrectly. A BAHA provides similar results to CROS. Hopefully they program it correctly this time.

I m 26 now i was born with one side deaf still i didn't find good solution to afford...but will thank god for they have given at least one side to hear.. still no one know i m one sided but i hope i will manage without know anyone through my life ....and also i have ear infections too i mean pus too....this issue help me @sleeping only...lol....on the other hand i get insulting and ashaming only...

I have hearing loss in one of my ears since elementary it never bothered me 🤨 I always failed the hearing test beep thing lol now in my 20s I have notice it when I put earbuds on or when I cover my good ear and try to listen from distance

Dr. Olson, using BAHA, how does the patient perceive the origin of the sound? For example, suppose SSD occured on the right ear and install BAHA there. When the sound comes from the right side, does the patient's brain interpret it as the sound is actually coming from the right side? or from the left side with the good ear? If I want to be able to hear the sound from the both ears and live as if I never had the hearing problem in 1 year time frame, would Cochlear Implant be a better option? or BAHA accomplishes that as well as C. Implant? THANK YOU!

With BAHA you will not get sound perception back in that ear. You have a shot with a Cochlear Implant if you can get approved.

What about the system I have seen, a CI in one side and a hearing Aide on the other side. I don't remember what its called but I have seen this.

FX-322 injections are probably the closest thing we've got and that still needs to go through some extensive clinical trials.

@@DrCliffAuD At 45 i can not wait longer though, the exhaustion is getting so bad not because i dont hear ppl, i hear too much on my good side (right) and it sometimes makes me not understand them even though i know they spoke. (10 to -5 DB , yes high tones are more sensitive on good ear, also my tinnitus in that ear is around 10k hz or so... nice tone :( ) First appointment with audiologists ended with them incorrectly charting bonetransfered sounds to my left DEAF ear and then mentioning they can not do Anything for me at all since one ear is so good. The deaf one she said she could measure a hearing aid on since it showed 65-80 DB hearing loss (all bone transferred i only heard sound to my right none on the left). I talked to my insurance and they said they are willing to 'look at it' but... i have to do the whole story again (as i have many of times as a kid since i was born deaf on my left). Now that i DO have to go through it all i wonder will a CI be something to help me regain some sanity, it should give me some directional hearing again and speach understanding on my left right with time and training if i understood the videos i sawcorrectly. (IF the nerve is still good, which since i dont know what is wrong other then the cochlea doesnt work) Otherwise i am trying to find some video etc on what a bone transfer device sounds like. Does it work good enough to be an alternative if my nerve doesnt work. The Cros also has some advantages, one it being also in my good ear and maybe be able to suppress the level of some frequencies. I Really wish that audiologist would have taken a bit more time before sending me off in the tinnitus and take SSD more serious. Managed to cope for 45 yrs, i dont go there for 'fun' or attention..... besides venting my rant, the main question is this: Do you think CI is , if possible, a good use to treat SSD if the other ear is good to perfect ? (except for high tone tinnitus and me hearing my bloodflow as a pumping sound, took me yrs to identify that sound!!)

Sir my left ear is totally normal but right ear has 75db of severe sensorinueral hearing loss...now which hearing should i go cros or traditional hearing aid?? I m confused as one audiologist advised to go for cros while oher has advised for traditional hearing aid....which would be better for me...plzz advice

Last week I woke up with a hearing loss (right side) of -60dB. It scared me like hell. No ear wax. I have a history of middle ear infection. My otolaryngologist suspects neuritis vestibulatis (nerve infection), for which he prescribes Medrol (prednisolone), a cortisone. Six days later on there is no improvement, this Medrol has no effect. I suspect there is something else going on. I think it could be a disruption of the ossicular chain. The sound simply does not make it to the inner ear. Very loud sounds I can hear but they are heavily distorted, like rattling and rumbling. I also experience tinnitus, this is a constant background noise along with short funny "plinking" sounds like from a tone generator at different frequencies. Unfortunately, I have to wait for 6 weeks to get a MRI scan that may bring some answers. I hope my hearing can be restored, as it is driving me crazy and it makes me depressed.

Generally, would this technology help someone with Meniere's disease? I've had it since 2004, and it's only the right ear. The hearing loss is periodic and is usually a 35 dB loss according to test I've had at the VA.

Meniere's is a totally different disease. Please do check out ENT specialist for treatment.

Hi Dr. Cliff, If you have a SSD with some hearing on the bad ear, would BICROS give you extra brain stimulation (to prevent dementia) over CROS or traditional devices?

Technically, if you have some hearing in the bad ear you may want to try AmpCROS instead of CROS or BiCROS. Otherwise, you provide no stimulation to the bad side.

My daughter was examined by the doctor in ear, nose, throat and found out that the left ear cannot hear. What to do? She is 3 years old and she can't speak as of now. My son can’t hear out of his left ear. It was discovered at about 3 years old too. I hope it’s reassuring to know that he is now 30 and his speech is absolutely normal. But his speech was progressing normally as an infant and toddler.

I was born deaf in my left ear and have no canals to my eardrum it's deformed I had many operations but they say I heal up too fast before they can put anything in there so i gave up now it just drains once a year mainly in Winter then closes up and I don't think i will ever hear in it in my life because of it so i just live with it and at times it can be very challenging but please tell me if there is any hope for me I would love to know please and thank you

Doctor please help me, I have a disseminated parasite infection which has spread to my ears too. I feel them moving and crawling in my ears, they’ve caused so many perforations and infections. Ent keep prescribing drops and told me he doesn’t know what to do. The ears have been perforated over 2 years, the only thing he can do now is refer me for ear surgery. No one can help me. I’m now having to use a hearing aid due to major hearing loss in the left ear, I’m tired of this. Why can’t anyone help me?
