This young lady has a very interesting presentation After 2 shoulder dislocations she now presents with positive signs of a spinal accessory nerve injury. She has obvious neurologic injury and scapula-humeral dyskinesis.

Great video! Glad to hear her recovery is going better than expected. Not sure if your allowed to say so but was it the accessory nerve or long thoracic nerve that was damaged?

I have this also, did everything. From prolotherapy, chiropractic, training and even more… keeps weak and feel that nerve from neck to wrist.,.

I have definitely same lesion with her in my shoulder, upper trapezius. I got dislocated and then had surgery, both of bankart and slap repair. Doctor diagnosed i have some nerve lesion in accessory nerve but they doesnt have any idea to me get better. They said just wait for getting better of nerve. Can you let me know what did you do for her and what was effective?? Plz

i have this condition look for impingement of the spinal accessory nerve at the first rib and then C7 to T5. Avoid the surgery using stretching techniques through the mid trapezius muscles

I have the same condition, but with marked atrophy of the trapezius and cleidomastoidal muscle. Doctors here not figuring out how to deal with it. Saw one who told that is irreversible. Any help

Hey there I'm a PT and wanted to thank you for some general treatment ideas. I identified this on a patient the other day and was trying to find validation of some of my treatment ideas. Did you end up using NMES with her or was taping/strengthening rhomboids and serratus and functional exercise effective on it's own?

How often do you see or experience nerve injuries that after treatment(injections & decompression) still have a positive injury still present to the nerve ?

This is not a common injury. I saw an axillary nerve decompression and about 25% improvement only. Cubital tunnels and carpal tunnels do well.

Ortho EVAL Pal With Paul Marquis I actually have had 4 different injection ( for nerve issues into thumb and pointer/bicep) from Pain management where on the last set from the side (not the back side) experience and involuntary bolt into my right arm. What I expected was a nerve that was accidentally hit by the doctors needle. Fortunately this was the last set of injections for a total of 10 total injections- (4) neck (1) rotar cuff- partial tear (3) PRP for rotar cuff & (2) for lumbar Who would of thought you went to a specialist to get helped and actually get hurt more in the process. I noticed that I have trouble swallowing which I later learned that the nerve receiving treatment; actually controls that part of my throat area.

Sorry to hear this. You should probably start with neurodiagnostics (EMG, NCV) and identify th source.

I had a san injury for the last 6 months. Still not 100% but recovering. Trapezius atrophy and supraclavicular fossa are recovering. Winging has disapated. Shrugging and shoulder impingement recovered. Stabbing neck pain shut off 3 weeks ago. Off painkillers. Very painful injury. I suspect a few more months if not longer before all atrophy is recovered. Emg confirmed impairment of San and mri yielded negative for mass or lesion of the brachial plexus. Hit me up if anyone needs advice on their journey while coping with San palsy. Exercise is key! Hi, did you have problems lifting weights above your shoulder, and was your neck pain more intense when you turned your head to the contralateral side? I cant even tell where my pain ls located, its somewhere in the neck/shoulder, and its stinging like hell. Shrinkage of the trapezius is present, but almost no lateral winging of the scapula. I pray i can recover

@@BlackMesaTrainee , yes, I had bad shoulder impingement. Couldn't move my arm above the plain of my shoulders. Could only form a 90-degree angle with my body.

1 year in. I look symmetrical in the mirror, but I can feel that the muscles above my clavicle are still not as dense. Still dealing with residual pain in right trap that impairs quality of life. Sitting for long periods is uncomfortable. Still need to stretch my scms as they can get quite tight. Feels like my right arm is the dominant arm again. Can throw a baseball again. Lol. Still exercising!

@jesusmagallanes5835 shoulder shrugs, trap 3 raises, lateral raises, shoulder press, rear delt flys, cable rear delt fly, external rotation with a pully, face pulls. I started off with strictly mobility exercises to deal with my frozen shoulder and then switched to weight lifting.

The tests you did was way out of the title u gave to this video spinal accessory? Let her shrug shoulder. The scapular winging is obvious but again not spinal accessory nerve. The right on the spot test was u were able to emphasize SCM muscle wich would tell about spinal accessory. Shoulder dislocation could tear a lot of nerves along the way and if you say she had repeat dislocation its a probability. But again its misleading to title rhis spinal acceaaory n.

@@OrthoEvalPal how is the symptom of shoulder dislocation? Because my left shoulder had been get smashed by hand very hard 10 years ago. Now my left shoulder is more weak than right. And my left shoulder more bigger than right and have nodul when i move my left shoulder. If i palpate my left shoulder, it be pain in there (upper trapezius). I dont know why is it. My SCM was removed in 2015 and my SAN permanently damaged ( dead ) I am chronically miserable. Not sure if anyone can help me.

This recovered over time and with a full regime of strength training of the shoulder blade muscles and shoulder muscles.

Hi doctor, I am Looking to do eden lenge procedure since I have asn damage due to lymph node biopsy 10 years ago.. I tried physical therapy but it didnot improve that much since I didnt start the treatment early. Do you recommend the eden lenge procedure? Which cases should do the procedure?

@@michellebletcher1856 when i did the nerve test. It was normal but thr muscle is almost gone. It was baffling. So iam still not dignosed. However, the the 3t surgery is still an opition. Iam waiting for my genetic test then i will do it. I dont know about you but iam desperate since the injury fucked up my life.

I call BS. It is highly unlikely that she "fully" recovered with a "comprehensive strengthening" program. Those were severe neurological defecits. Define "fully" recovered. No way she has normal use of her trapezius or entire shoulder girdle.

We did a lot of shoulder stability exercises, rotator cuff strengthening and it eventually returned.

yeah but thats a chronic pain everyday therapy nobody has money for so the eden lange procedure surgery is best while shes young it will work in her favor without her going bankrupt
