This hymn is said to belong to the Sri Guru Gita of Skanda Purana, but although many words, some lines and ideas are identical, an exactly similar arrangement of the verses was not found in the online versions. The first shloka, not found in usual documents of Sri Guru Stotra is the invocation of the second chapter of Sri Guru Gita. As a voluminous Purana, there are bound to be regional versions of Skanda Purana, many of whom are unlikely to be found online. Any additional information about the source of Sri Guru Stotram is welcome.
