Neuropathic (Nerve) Pain -- from Toe to Head
Dr. Ian Carroll discusses neuropathic pain, which involves damage to the nerve. The condition causes the nerves to fire incessantly, even if the initial source of the pain has been remedied. The brain responds by creating an ongoing message of pain.
Speaker: Ian Carroll, MD, Assistant Professor, Anesthesia, Stanford University Medical Center

Every single doctor I've seen for pain has tried prescribing me antidepressants to "help" with the pain. It angers me beyond belief that intense chronic pain is looked at as a mental condition or a drug seeker. Thanks for this video. I'm glad someone is still trying to actually help people by educating those that are interested to learn.

Understand that some low doses of antidepressants have been shown to reduce pain. If you look up "antidepressants and pain" you will see the evidence.

@@yournamehere6939 I was a passenger in a vehicle that was hit by a semi. I still have small fragments in my spinal cord. One doctor stood in my face and told me it was all in my head, long term pain isn't real. I'm glad I ignored my first instinct to punch him and saw another several more doctors. At least one rxd tramadol that worked enough for me to work, then they cut me off because the DEA decided it should be classed a narcotic.

@@alexgorron6470 that's horrible. So sorry for you Alex. I get no understanding from my doctors about my pain either. Lots of autoimmune suppressive drugs though. Now going into pancreatitis from the meds. I'm trying bile salts to see if it helps me. Pancreatic pain AWFUL. Liver area pain AWFUL. 4 years now. Life has been very difficult and depressing. We must do alot of searching on our own unfortunately. My meds only make me feel worse than I already feel. Blessings to you Alex. I hope you find something to help. @Ronald Krikorian Its a mouthful - "Adult onset tethered cord with giant intrasacral meningocele, " which has caused permanent damage to the nerves associated with the cauda equina, plus my spinal cord having been under tension for 30+ years, which also lead to lumbar arthritis and protruding discs and all that. I also have a connective tissue disorder that causes instability of the spine (all joints, really) so I go to the chiropractor multiple times a month to get everything put back where it belongs.

I too have never heard neuropathy explained in this way. I’m 54, and have had three back surgeries, discectomies from T-1 through L-5, and lamiectomies, with spinal fusion, at all levels. I’ve just never been free of nerve pain, even after the surgery. I’m a nurse, and continue to work full time, battling nerve pain every day. People just have no idea how bad nerve pain really is. Along with the nerve pain, I have numbness and tingling from my lower back to my toes, and some days it feels like my legs are on fire. Thank God I’m able to get up and work most days! Remember, it could always be worse…..God Bless.

Intentional discrepancies of facts by physicians and am now permanently dependent on a wheelchair at 53. I am now on a helpful regiment of medications but the breakthroughs pains of late are upwards of 8 to 9 on the Richter scale. I applaud EVERYONE who will speak up against the drug companies and thank God in heaven there are Dr's like this gem that are eilling to share this information! Are you in Canada? I was 51, presented with a true and severe nerve compression injury and I was utterly gaslighted. All ten doctors I saw at home misrepresented the truth, wrote fraudulent statements in their consult notes and I was purposely misdiagnosed! They were covering up that metal had migrated in my body and wanted to avoid a lawsuit. I’m lucky to be alive. I had to hire a registered nurse consultant to get the life saving and sanity sparing surgery I needed! Doctors can no longer be trusted. They have lost true compassion and caring.

They treat me like I am crazy. It keeps getting more painful. I can't do this much longer. I have had 2 back surgeries and the last one helped for about a year and a half. Thankyou Dr. Ian. As a chronic pain sufferer for 50 years. ( FYI Adhesions and fibromyalgia etc) Diet changes and fasting etc have given me degrees of improvements...this talk is the next gift from the Lord in my journey of improvement. Like many commenters I have been through the ropes of specialists , medications and mistreatment. Being married to a wonderful doctor for over 40 years ( and knowing many dedicated doctors) I do not resent them and realise they do their best for which I am grateful. They do not know everything as my husband says ...and we are all learning always. We are also responsible for our own health as well. Having said that , I've had my share of frustration etc. Let's move on from now, receive any help such as this and share our experience with others and doctors. By the way , I'm a Christian so I pray for my doctors not just for myself. My love and prayers go out to all who have and are suffering like myself.

Way back in 1976 when I was in the USN I sneezed really hard and bent over and ruptured a disc in my back. I had fallen several times before with no major issues. I think this may have contributed to what happened. I was in severe pain and I was subjected to procedures that were supposed to help but only made the pain worse. I was finally placed in Bethesda naval hospital! They seemed to believe that I was "malingering" which is a term used to describe someone who is faking injury or pain to try to get out of duty. That was far from the case! I then needed to go through many tests which ended up being painful due to what was wrong with me. They finally decided that I had a ruptured disc and I needed surgery. The next day after surgery I was urged to get up and walk and lo and behold the pain was gone! About five years ago I started having issues with falling and to make a long story short I have severe diabetic polyneuropathy. I was misdiagnosed for 5 months and in extreme pain. It took another couple of months to convince somebody that I needed to see a neurologist. Meanwhile I was placed into a rehab where they kept trying to get me to walk at cetera without doing anything to help my excruciating pain. Even after I got my diagnosis from the neurologist they would not prescribe anything strong enough because the doctors are all worried about the opioid crisis. Finally the neurologist referred me to a pain center and they prescribed Tramadol which is the only thing that has helped me! Lyrical made me gain 100 pounds and become bedridden! After going off Lyrica and using Tramadol I lodt 94 lbs and am able to get around again... but I'm still considered disabled. Don't give up no matter how many times people try to tell you it's all in your head or it's some other thing that's causing your problem. You know your body they don't!
@nancydroll6523
@nancydroll6523 2 года назад
What it is difficult to explain to them, not just other people but Drs as well, is that things like Vicodin do not work on Neuropathic pain. Tramadol is a good starter, Percocet works, but the best thing I have found for my ruptured discs and degenerative disc disease is Suboxone. I will NEVER be pain free, but this makes it tolerable.

Not only is this dude brilliant but he also knows how to approach and explain things that most people who are not M.D.s can in fact come to appreciate and from which to gain knowledge. I am so impressed by this 1.5-hour lecture. My brother has had 4 major back surgeries and suffers without break from nerve pain; I am convinced that his surgeons injured his nerves (plural !) to cause his pain. Unbelievably, since he visits doctors because of this pain (doctors who are incapable of helping him at all), some are convinced that he needs a "serious mental health evaluation," like patient "DS" in Dr. Carroll's talk.

@JediJan

@Callie And Her Sweethearts I have had back / leg nerve issues for over two decades. Have taken many different drugs (codeine, anti-inflammatories, Vioxx, Lyrica and now Gabapentin) for nerve pains over the years. I now have CKD (Chronic kidney disease) as a result. 3 operations later I am much improved but find I can not take less than 3x100mg Gabapentin as nerve pains return. I actually suspect possibly fibromyalgia and other weird leg sensations like restless legs is a precursor to nerve pain. I had restless legs syndrome before experiencing nerve pain. I still get the weird leg sensations now, but not as severe as before, because I don't wish to take more gabapentin. I found doxylamine succinate helps calm restless legs syndrome. I wouldn't be surprised if that could possibly help fibromyalgia sufferers too. I understand that is not addictive, or hasn't been for me. Worst thing about nerve pain medications is I found weight gain issues.

I will tell you how irritating and insulting especially for a woman any age to go to doctor with pain and be told it’s all in your head, get your hair done, etc stupid stuff. Do doctors really think we have nothing to do but waste our time and Money to go in and complain of pain. Pain that is visibly ruining our quality of life. Studies prove men with same complaints get help without begging. Hope this has stopped. Back in 1980 it was prevelant when spikes of fibromyalgia reared it’s head across country . I facilitated pain support groups for 23 years at local hospitals. Was heart breaking to listen to patients mostly women who could not be heard by their doctors and properly treated.. great talk. Thanks for great info.

So informative! Not everyone who suffers pain needs a psych exam. Nor are a drug seeker. Those deflective assumptions occur far, far too often. Especially when the patient is a female.
@lilolmecj
@lilolmecj 2 года назад
In fairness, many medications and treatments had been tried. I think two things are at work, one is the persistent view that people are not in “real pain”, and that everyone likes the way narcotics make them feel, and therefore literally everyone is a drug seeker at some level. I would go as far as saying that most chronic pain sufferers would benefit from psychological and spiritual support. It wears a person down, and depression can easily complicate the picture. Further, sleep disturbances due to pain can prevent adequate REM sleep, leading to real psychological issues.

Side effects may appear if you raise the level too fast in any drug and no drug should be stopped all in a sudden. It has nothing to do with addiction but body adaptation! Great explanation of what chronic pain does to one!! A problem I experienced was that although my nerve pain was controlled with Gabapentin and Morphine…. I found myself ostracized by my Drs when the opioid crisis blamed me and my fellow CP suffers for the epidemic…. While the 20 years of GABA and Morphine was not perfect… I could at least get out of bed and now after years of trying to find relief from others my medical records are polluted with psychiatric innuendo and knee jerk diagnoses. I am sick from hearing Drs explain their views on the damage opiates cause….. I used them over 20 years…. never once asking for more and now on paper I look like a screaming junkie…. feeling shame every time I ask for pain relief

After two years of trying to diagnose the reasons for my pain, I was given the diagnosis of Fibromyalgia. This was not before med students were paraded past me and I was told I needed to seek help from a psychiatrist. It was the most horrible time of my life. When you know you are in pain and you know when and where it first started but you have no answers as to why it has become large and complicated and the medical community has no clear cut answers either you begin to think you really are crazy. some people in chronic pain commit suicide! Living, daily, in unrelenting pain is inhuman torture and having no one understand why is even worse. I wish I had a doctor like you! I should add, all this happened to me in 1990!

I got two and a half minutes into this video, where the subject patient was referred to a psychiatrist, and I started tearing up. I had to beg my primary care doctor for a referral to a neurologist - he just did not believe there was anything wrong. When I finally saw a neurologist, he diagnosed me with "severe and advanced" axonal neuropathy consistent with Charcot-Marie-Tooth. I was 40. Why I had to fight so hard to see the neurologist I will never understand. Over time, conduction studies also indicated demyelination. At 50 I went on disability, and I've had four foot surgeries and spent a year in a CROW boot. I am exhausted just doing nothing, and everything I do is against this awful wall of pain and fatigue. I take 2,400 mg gabapentin daily, but there are weeks at a time when the buzzing and shocks in my feet and aching in my arms and hands keep be from sleeping. It's bad enough having this, but it is doubly bad that so few understand.

I’ve suffered with nerve pain for 55 years. Pain management consists of controlled pain meds and cortisone shots. But the worst pain I’ve had is much more recent. I was diagnosed with West Nile Virus which was detected by a lumbar puncture. The lumbar puncture was performed by a doctor who obviously wasn’t great at it. As a result, on top of wNV, encephalitis, and meningitis, I was dealing with the most excruciating pain I’ve ever felt. It was so bad, I went to a very dark place in my mind. None of my doctors could help me. The only thing that helped was time. However, this nerve damage left me with a right leg that is only at about 50% capacity. I’m

I have neuropathy caused by bunched up nerves in my buttocks like the top of my buttocks they say. I am losing the feeling in my legs and the ability to walk. This is on top of being the first 500 to get the so-called cure for hepatitis c which caused me to be disabled at the age of 38. Excruciating pain and they over medicated us with oxycontin then it went completely the other way for all you get is beautiful nephron which has no effect on my pain you think that would mean something to us pain specialist but no it doesn't. They just said give you like that's all you get and it's I've seen people respond like they're on heroin when they take that stuff but with me I sat there with just a headache and taken several of the pills and it did nothing didn't make me sleepy didn't make me awake didn't make me talk funny this is asking other people did nothing for my pain and I am at my Wit's end on why can't they do something now I have this neuropathy all these things are coming to fruition cirrhosis of the liver non-alcoholic emphysema fibroid disease and all these these tests so far have been perfectly normal like it's all in your head. Now I'm getting this horrific rash in my groin and area and thought that you know between the thighs and the groin and under my breast and it's wet I've even put anti-perspirant on it and that stopped it just cuz that it's like wet constantly sweating and it's like my sweat is so acidic it's just burning and I imagine it has to do with the liver or something but I don't know what to do about this rash and the pain problems. But I've heard that beautiful nephrine and Suboxone were invented by the same place that oxycontin came from. What the heck is going on. I can't get Vicodin I can't get oxycodone or anything like that and it actually the two were great together because you can take one and then take Beaufort and it keeps the addiction response down I've tried it it worked but my doctors they just keep lowering everything and I was on way too much before oxycontin so much antidepressants Ritalin everything I wanted. And then I mentioned it to another doctor in my doctor got so mad that I ratted him off that he dumped me. And I had to go through treatment to get off to overcome the withdrawals from that stuff. It was a nightmare. But why am I blamed why can't I just have a little bit of pain medicine but now it's like nothing and to be honest I'm 65 I'm so sick of this constant pain now it's getting worse I'm getting less pain medicine it is just and and the socially I'm not validated I'm cruely pushed out of the family a holidays my children just you know think I'm worthless you know and it all came about because I went to treatment and did my 10 steps and their reaction was a whole you'll never get off drugs you know. And and just I'm I'm a faking it for attention and drugs up to a 5 years ago my family thinks this and why would I do that I'm from a college oriented family why would I want to be on welfare so poverty stricken for all my life I have no way of changing it I can't work I can't go to school they destroyed us yet we signed a waiver I mean no doctor even knows anything about it. So they say.

Dr Carrol, this is fantastic to hear you speak about this topic. I am a remedial therapist in Australia and I added on modalities of the nervous system, muscular skeletal therapy, the lymphatic system, reflexology, and the brain. You obviously have a huge understanding of this knowledge of nerve problems and the data charts of nerve referral connections to the area that the patient says the pain or stiffness stems from. It’s all the nervous systems working here with any pain. Which after time causes referral pain that just is never ending. As this pain encompasses the whole body, I have treated my Fibromyalgia patients knowing using these references. I think Fibromyalgia could be caused systemically by years of referred pain. And have found Duloxitine is a great nerve suppressor. It’s like an onion layer that needs to be slowly pulled apart. Knowing the sections of the body that main nerves from the spine refer to in functional response to Pain, can tell us so much. Especially if say the thecal sac was being pinched by a protruding disc or transverse joint that is out of its structural normal. This pain goes all around the body from tingling to aches in shoulders, scaps, neck, jaw, spine, toes, legs etc. The chemical imbalance that can arise from these body changed functions and alerts of pain often trigger chemical imbalance in the brain of Serotonin , epinephrin etc as you have said. The charts for referred nerve pain should be studied by all medical disciplines. As well as a much deeper understanding of how nerves work with the body. Duloxetine works for nerve pain very well and all associated chemical imbalances that can cause depression in this pain situation. As for your. audience guy that spoke of getting into bed and having feet aches, stinging, hot or so annoying that you have to shake your leg or feet.I have found this is due to really tight calf muscles putting pressure on the nerve going down the outside of the lower leg. Trigger point massage and releasing parts of the calf muscle will stop this aching pain straight away. The patient needs to push hard into there calf muscles and find the spots of pain and massage deeply to relax the muscle. Great great speech, your knowledge is so refreshing for all medical, natural and remedial healers out there.

Due to constant pain, frustration, medication side effects etc he’s like a bear with a sore paw and so hard to live with. He’s vicious and selfish due to pain. Reliving pain would help him and our whole family! We are in Adelaide. Can you please let me know where you are 🙏🏻

@@007Jacqui Hello Jaquie, in answer to your question I would say no. Unfortunately breakdown of myelin is usually associated with MS and by changing his diet may help. Inflammation associated with insulin imbalances can also be involved. There are many RU-vid sites that you can visit for help regarding his condition. Just type it in or see if doctors sites can recommend the best treatments. Just checking myself it looks as though going onto the Keto diet may help a lot. I do agree with the previous commenter have written - it's not straight forward than it sounds to find a natural remedy for neuropathy. Be cautious and take advice from other people. I searched round a few sites and discovered some great advice by searching google on websites such as Gabs Neuro Guide. Good luck and hope you succeed!

Appreciate video content! Sorry for the intrusion, I would love your thoughts. Have you heard the talk about - Dinanlinson Perfect Shape Approach (Have a quick look on google cant remember the place now)? Winner of a video, been searching for "what are first signs of rheumatoid arthritis?" for a while now, and I think this has helped. Have you heard people talk about - Peyandon Dumbstruck Primacy - (do a google search ) ? It is a good one of a kind guide for discovering how to eradicate joint pain using natural ingredients minus the hard work. Ive heard some pretty good things about it and my mate got amazing results with it.

I have lymphedema, chronic venous insufficiency, peripheral Neurapathy, carple tunnel ,migraines I walk with a cane due to balance and not feeling my legs and feet all the time. I have bulging disk and degenerative discs. I spend half my day taking care of my body which has cause caused anxiety and depression. I have a wonderful family and some friends who help me with dealing with all my issues. I'm so lucky to have them. I have lost so much but have gained so much along the way. I have 7 beautiful and healthy grandkids and I have learned to enjoy the small things in life.

I’ve had neurological pain for over 35 years. I also go to a pain center. They have helped me turn the pain level down. It’s not gone, but I don’t feel anything can stop nerve pain. I take one medication for pain, it is a narcotic medication, one of several types taken over those 35 years. If a patient takes the exact dosage of medication it becomes easier to get control over the pain. Start taking more then prescribed will make pain control Impossible to maintain. Be responsible with narcotic pain medication.

I’ve been struggling with excruciating pain in my legs..resulting in loss of mobility...and pelvic pain out of this world...this has been going on for almost 8 years...I’ve been trying to find a doctor that didn’t treat me like I was out of my mind...or that I was just a drug seeking fool...I’m currently Lyrica...Quetiapine...Kadian...with no relief period...was on Cybalta but did nothing...couldn’t take Gabapenin...can’t sleep and the pain in my pelvis feels like the worst UTI in history...along with a an embarrassing feeling of sexual excitement but painful....I’m tired of being treated like I’m nuts..or hysterical...I can’t live like this...I stay in my bedroom most of the time...stress from the pain and inability to live even a semblance of a semi normal life...starting to think there’s only one way out...I’m a female in my mid sixties...the pain in my legs have me walking with a cane because of the electric shock like torturous pain...walking is almost out of the question now...it’s getting worse...at the end of my rope....

Have you had an MRI on your lumbar spine or any kind of X-rays? You also would want to cut out sugar, processed foods, and vegetable omega 6 oils (canola, corn, sunflower margarine) and lose any extra weight. As your health improves you will have less pain. But I can see you do have some nerve damage somewhere so keep trying to get a diagnoses. Lyrica without an opiate is obviously not working for you . You are going to have to request it and keep demanding help. I also suggest doing a spiritual practice with breathing exercises. This will help you with your depression. Not getting help with pain from your doctor is barbaric and they need to be held accountable.

@Christinnne, this sounds like small fiber neuropathy for sure. See if you can find a neuromuscular neurologist, because even most neurologists don’t know about this. It does not show up on standard neuro exams, or on EMGs and nerve conduction studies. Diagnosis requires a skin biopsy over the shin.

So it turns out, Doc, that this video made a huge difference in my life. I had my GP switch me to Imipramine and Lyrica, about a year ago, after watching this. And I can genuinely say that I have at least 70% less pain. I've gone from bed to wheelchair, then cane, and now walking unassisted. Thank you!! Fibromyalgia is my curse, for anyone reading this and wondering. Also Diabetes 2.

Nice presentation. Good info. But, Duloxetine can and does cause severe GI imbalance, causing regular episodes of severe nausea, vomiting and symptoms of ibs. This has occurred in patients utilizing Duloxetine as a regular, long term therapy. It has occurred in patients who have been properly titrated to the higher dosages. It makes sense since the gut is loaded w serotonin and therefore anything that effects the overall serotonin availability in the gut can affect GI function.
