I love teaching women about the safety and efficacy of the use of hormone therapy when needed at menopause. In this video I explain the "Timing Hypothesis" Which demonstrates that hormone therapy can be safe, especially when taken within ten years of menopause. I break down the Women's Health Initiative (or WHI for short) that initially scared a lot of women and physicians about hormone therapy, but has been re-examined over the years to prove this "timing hypothesis." I also talk about other menopause and hormone replacement studies that have been done in younger symptomatic women.

Great content. I’m 51 and not yet in menopause just peri menopause and have been researching hrt and the benefits of preventing the three diseases. I follow Dr. Taylor’s channel on Menopause and she echoes everything you just said. It’s a shame that most physicians don’t recommend hrt and are against prescribing it due to misinformation and complete ignorance, sorry to say. My Gynecologist and PCP are not in favor so that leaves me ne, when the time comes, to go seek hrt via telehealth or overseas!!! Really is a shame. “My body, my way…my menopause, my way!!! ❤️

The most important fact about the WHI study was that the HRT used in this study was that the HRT used in women over 60 was SYNTHETIC HRT is now body identical made of yams. Thank you for your great info Dr 😍

I am so glad to have recently found you! I am 59 and was having some crazy symptoms...anxiety, nervousness, mood swings etc. I found a OBGYN and booked a much needed appointment. She took some blood and found my Thyroid-TSH was 3.9 and obviously may hormones had tanked. She put me on a low dose of bio-identical hormones, estrogen/testosterone(troche) and progesterone (pill) saying that sometimes the hormones will fix the thyroid issue. I was a tad afraid to try them for the reasons you stated. Well let me tell you, I felt like a different person in just a few weeks. My knees and joints even improved. CRAZY!... HOWEVER, I had no idea I would have periods again. UGG!!! Had to stop HRT for my upcoming knee surgery, so I will decide after if I will get back on them and put up with the periods again. I'll be watching more of your videos. Thank you!

Thank you very much to delving into this topic! I am working my way through your Menopause course and already learned so much. I took my notebook with all of my great notes and information to my annual GYN visit today, and I felt totally confident and capable of discussing HT intelligently with my provider.

So informative. Thanks again. There are so many myths and so many of my friends are so anti HT. I am 58 and had a hysterectomy at age 50. I live in Australia and we seem to be so much more reluctant to embrace HT. I am taking Livial, have osteoarthritis in both wrists and don't want to slide any further down the tunnel. Should I see if there is something else I can take? My GP is wanting me off HT soonish and I really don't want to go there as everything I see encourages to keep HT in my life. Also side note my sister had a blood clot last Christmas after international travel (takes ages to get anywhere in the world from Aus), she is 50 and is being told not even to think about HT. Is there nothing she can investigate. I look at her and feel so sorry for her and what she is going through. Many thanks. Love love love your work.

Wow! We are so happy to have you here on the channel and I see my colleagues and I have some work to do down in Australia. My personal medical opinion is there is NO indication to come off your HT. the biggest risk for clot is in the first 6 months, so if you are far past that then you can continue it - no problem. As for your sister - it seems like a provoked blood clot, for which she could still take HT with an informed doctor prescribing. I would start with a trans-dermal patch perhaps and do work up. she has options!

I'm 57 and have been on bioidentical hormone therapy for 7 yrs. I chose to take hormone therapy to hopefully reduce my risk for Alzheimers. I have several family members who have died from this. I follow all the latest research on this and feel that I'm on the right path.

Terri Allen there is a lot of research being done looking at the use of estrogen shortly after menopause to reduce Alzheimer’s. Clinically I feel as though condition does improve and I’m excited to see this long term Research. I think you are on a good path!

@@heatherhirschMD What level of HRT is showing beneficial in the current research? I'm 55 and am currently using the Climara brand Estradiol 0.05mg/day patch, and a Mirena brand Progestin IUD.

When I check blood estradiol levels they are typically 30-70 when a patient is on estrogen therapy.

What about the benefit of HRT in increasing bone density?! I heard it can really help. Along with taking D3, vitamin K2, bone builders formula from Jarrow. Stay away from calcium, except from your leafy greens. And if you do take calcium, make sure it's the calcium hydroxy appetit form. Magnesium ratio to calcium should be 3:1. We all need magnesium from food and from supplements. Our bodies need it for so many reasons. Also, can you comment on why most women need a very small amount of testerone also. And why DHEA sublingual, or liquid, pharmaceutical grade, is also needed by most women. 5 to 10 mg daily. Yes, please talk about BIO-identical hormones for women, in detail!!! Thanks. Jean.

What is your professional opinion on a book written by a doctor, called Estrogen Matters? It's supposed to be a very good reference book and has good explanation. I need to buy it and sooon!!! Jean.

James Cardellini estrogen matters is a very thoughtful book and I do recommend it. Some women still chose not to take estrogen replacement and I respect and ho or everyone’s individual choices!

Hi, I am 46 years old. I had partially hysterectomy surgery since 2018. For few months I have been experiencing stiffness in my both hands and pain in wrists, pain in both knees and right heel, skin dryness, hair falling, no RA factor, no physio issue. Doctor advised me to take HRT. Kindly advice me regarding this therapy . Will it be beneficial or not for me ? Thanks.

What if you are considered high or higher risk? My sister died at 42 from breast cancer and my mom was diagnosed at 82 (she had a lumpectomy and is fine now). I am 53, cannot sleep, have incapacitating brain fog and bouts of rage, my personality has disappeared and am a crabby old bat with a petrified V. Nobody will touch me with a ten foot pole to prescribe HT. The Breast Center where I go “doesn’t do that” the actual Menopause Clinic in town said “we can’t help you”. So what should I do now? So I live out the rest of my life exhausted and sad?

It’s better than getting cancer! I was on Premarin and got breast cancer and a meningioma brain tumor. Hormone positive breast cancer treatments include taking aromatase inhibitors which greatly lower your estrogen levels and make your bones crippled yet the doctors push it because they get kick backs! Don’t trust any of them pushing their pills and creams! Bet she drives a Benz or 3!!!

I have been on testosterone pellets for 1 1/2 years and I started to feel super bad, I'm depress, gain a lot of weight I do exercise every day and follow a healthy diet, but the last time I got my pellets my Doctor elevated the dose of testosterone, I feel like I'm dying, I make the decision to stop the pellets but I'm afraid of the will draw. Need some help

Thanks so much for all the information. I have a couple of questions about possible side effects of taking HT. Can they help with weight gain? and Can HT contribute or help hair loss? I know only cosmetic concerns, but still worrisome for me. Thanks so much.

It can definitely help hair, skin, and nails, thought is not FDA approved for that very reason. Check out my video I did on hair here on my channel if you have not already!

Thank you very much for your great videos! I have a question regarding taking progesterone. I had a uterus oblation 4 years ago and started HRT in January. Do I need to take progesterone or only estrogen? Thank you.

Great question! Yes any women who still has a uterus needs progesterone even after an ablation!

Can you discuss the difference between the three types of estrogen? Estradiol, estriol, and ? I forget the third one. There is a good estrogen, and not so good estrogen. Also, I think progesterone topical cream is fantabulous. I need to get my hormones tested again, either thru saliva or urine. I usually get blood tests also. I'm new to your videos. Jean.

Hello Dr. Hirsch, I have been told by my GYN that having a patent foramen ovale is a contraindication for HRT (47 y/o, do have history of ocular migraines but no history of stroke, never a smoker, healthy weight & BP). Wondering if I should give up on HRT as an option.

Hi doctor! I am 54 and I have been on HRT 11 years. I have very fibrous breasts, recently they have gotten worse...heavy, more fibrous, and painful. My doc thinks it’s my body’s way of saying time to ween off HRT. What are your thoughts?

By the way, I was on Estratest and progesterone for 10 years. I am on prempro for the last year. I had to switch because I am now on VA health and Estratest is not covered. 😫

look on the North American Menopause Society website (NAMS). It is no longer that you need to wean off at a certain age. Make sure your doctor has read the 2017 NAMS position statement on that.
