What most people call tendonitis is actually tendinosis. This is kind of a big deal because many people who think they have tendonitis approach their injury like an acute inflammatory condition (ice, NSAIDs), when this isn't the proper approach for their degenerative condition.

If you’re new to my channel, my name is Dr. Drew Timmermans, ND, RMSK. I’m the founder of Regenerative Performance in Gilbert, AZ, specializing in interventional orthobiologics, which is just a fancy way of saying we help people reclaim their life from chronic pain without steroids or surgery. How I got here…
22 yrs old: Graduated with a Bachelor of Science in Kinesiology from the University of Western Ontario.
23 yrs old: Competed as a long sprinter, anchoring the 4x400m relay to a silver medal at the Canadian national championships, contributing to Western's first national title in men's track and field.
23 yrs old: Started working as a personal trainer for GoodLife Fitness in Canada, quickly becoming one of the Top 10 Personal Trainers nationwide.
23 yrs old: Herniated two discs in my lower back, ending my track and field career and sending me into a multi-year battle with chronic pain.
24 yrs old: Enrolled at the Southwest College of Naturopathic Medicine (now Sonoran University of Health Sciences) after discovering my passion for naturopathic medicine.
26 yrs old: Still battling chronic back pain, I finally became pain-free through a platelet-rich plasma injection.
28 yrs old: Graduated and become a licensed naturopathic doctor.
29 yrs old: Completed the first-ever residency in regenerative medicine and integrative pain management at the Neil Riordan Center for Regenerative Medicine.
29 yrs old: Co-founded Regenerative Performance to help people with chronic pain just as I had been helped.
Today: We help people who have chronic joint pain reclaim their life with cutting edge treatments like platelet rich plasma, and stem cell therapy.

Just wondering if one will heal from Tendinosis where they can feel normal again and not think about it. Feels like it'll never come right and I'll have this pain and niggliness forever...

Hey doc, I was diagnosed with peroneus Longus thickening parallel to the cuboid via MRI. My PT says thickening means tendinosis. Been battling for months as it was tendinitis and it’s now tendinosis. PT said PRP would be helpful given the low blood supply in the area. Does that make sense? Thanks!

My Achilles tendon has been flaring up as I had run 4-5 miles a day for three days.(yeah I’m dumb). I’ve taken turmeric for inflammation and have been hitting the sauna and working out my upper body a bit to get my system in recovery mode and it’s been feeling loads better. Good luck everyone. Hello sir, I'm Adel 22 years old , In my MRI i have diagnosed with mild subacromial subdeltoid bursitis, mild subacromial tendinosis, I'm doing physiotherapy from last 3 months.. all the stretching and exercise, eccentric exercise, still i have pain, fluctuating pain near my scapula , inner side and outer side of elbow, impeachment feeling underarm , sir i don't know what to do, i just don't want to take in injection or surgery , please help me out ?

Great video. What type of exercises do you think are good for tricep tendon tendinosis. Isometrics or eccentrics.

Thank you for sharing the knowledge! I’ve been dealing with a mild ecu tendinosis for the past 8 months and been doing OT for almost 3 months but still has not recovered and a long way to go from where I was pre injury. I’m debating if I should get the cortisone shot next because my therapist is recommending it. But after watching your video, I’m considering the PRP although my therapist and doctor never mentioned about this option…

If possible, please stay away from cortisone injections into tendons! It only makes things worse in the long run. Your therapist/doctor probably never mentioned PRP because they aren't trained in it because insurance doesn't pay for it.

@@DrewTimmermansND thanks for your feedback! Do you know if PRP is effective towards ECU tendinosis? Also, if I only get 1 cortisone injection, and do immobilization for 3 weeks after the injection then straight to physical therapy, is it still not a good option or is it possible my injury can recovery from it? Thank you!

PRP can be very effective for ecu tendinosis yes. In general, cortisone injections degrade the tissue it's injected into. That's the risk you take with each and every cortisone injection. You and your doctor need to decide what risks you want to accept for those injections. It's possible you could be pain free after one cortisone injection yes, but you also may suffer more damage long term because of it

Thanks for this great video. Can athletes continue to train with Tendinosis, while doing PT? Recently diagnosed with mild quad tendinosis after an MRI, and was told it was okay to continue training despite pain. I have done about 3 months of PT and am continuing with stretching and rolling.

There are no live stem cells in any of those products. Those products can sometimes be beneficial for the constituents in them, but personally we don't use them anymore. But no matter what anyone tells you, those products don't contain living stem cells

How do PRP injections compare to stem cell injections? What are the pros and cons of both?

In general, cellular injections (bone marrow concentrate and adipose tissue) are stronger treatments than PRP. Adipose provides a great scaffold, which can be important if there are large defects. Bone marrow and adipose are more invasive procedures

Really informative video! I have had constant inflammation, swelling And loss of mobility in the inside my right knee for 7 years. I had physio for 6 years but that never solved the problem. My knee would lock and be incredibly painful to pop open, and this resulted in swelling and my tendon becoming really sore. This happened multiple times a year. Any suggestion as what is going on? Any information would be a huge help!

I'm 2.5 months post prp for later epicondylitis (elbow) and the pain has been worse than before the surgery. My Md says this is "normal". To make matters worse, I'm not allowed to take anti-inflammatory meds as part of the rehab process. I wish I had never gone through the prp procedure.

Your MD is not educated properly. It's not normal to be worse 2.5 months after prp injections. Fire that doctor and find a better one

I use my right hand too much, the tendon hurts sometimes, I think it's tendinitis , it's goes away after a few weeks and I'm 50 already, I'm trying to keep my hands stronger and health cos I need them,thanks!

20 year old here. I have recently been diagnosed with tendinosis on both of my wrists. I was told to see a specialist, but I have to wait like 2 weeks. Is there a possibility that you can make a video of any stretches or how you would approach such situation. Thank you :)

Sorry I unfortunately don't have the capacity to make videos for indivual requests like this. But I do have several videos on tendinosis already if you search my youtube channel. I specialize in regenerative injections, I leave the stretching instructions up to the physical and occupational therapists that I refer out to. Best of luck to you

I've had the ultrasound and says I have tendinosis and tenosynovitis on my left wrist. Got this when I tried downward dog pose in yoga for 15.mins..Have been feeling inflamed for 2 1/2 years. I had 24 sessions of PT and after that 1 shot.of steroid and also been using wrist protect gear everytime I work on the computer. Also tried hot and cold packs treatment. But the cyle.of inflammation goes on. Some days it seems norma when I dont use my left hand much especially when I just woke up in the morning but as soon as the days passed when I do my job in the computer, or holding things on my left hand, or even some.movements that flexes my left hand, the problem worsens and sometimes inflammation last for days. I'm 35 year old woman, do I still have a chance to get healed? What should I do or should not do to bring back to normal?

Im terribly sorry to hear that. I would need to do a full workup on you to give you personalized advice, but some different things to look into - oral vitamin c, alternating hot/cold therapy over the wrist, regenerative injections such as PRP, peptides like BPC-157 or thymosin beta 4

@@DrewTimmermansND Thank you Doc for the concern, I just wish I could personally come to you for check up but I live in Philippines. I've been to PT doctor, ortho surgeon but none of them helped. Should I wait for my tendon to heal by itself or ask these doctors again? I feel like the more meds they inject, I fear it might get worse. Cause when I do hot and cold compress or hot compress it becomes more sensitive and the more I use splint, other parts of my hand gets painful too. I'm trying to avoid things or movements that aggreviates it but I just can't avoid them forever and I need to work as well.

@@DrewTimmermansND Thank you Doc for the reminder. Thats why I'm hesitant to come back to the ortho doctor as he might do another injection, even if he said there is no side effect and only localized and before he did the injection he told me that its really hard to know where the exact location of the problem to inject. But the steroid only made it stronger for a few days then came back to its normal inflamatory state. Glad I only did one steroid injection.

Yea you definitely want to stay away from those long term. Go check out The Ready State and try their mobility programming for 30 days. Might just change your world

I listened to all of the video which is great. I have recently had an MR Arthrogram diagnose my Subscapularis tendon with mild tendinosis. Would physical therapy alone including exercises get this tendon back to full health? Also how would PRP impact this tendon as opposed to not getting PRP?

Hard for me to say without working you up fully as a patient. More mild cases can do very well with PT alone, but thats not always guaranteed. PRP injections may not be necessary, but they may help expedite the healing process when combined with PT and supplements to help support connective tissue healing. If you try PT alone and it doesn't resolve your issue in 6 months you could explore prp injections.

Another question I'm wondering is if someone does PT, PRP, supplements etc, is it possible to get that tendon to full health comparatively to someone who has a normal non degenerative tendon? E.g it'll look similar to a person's healthy subscapularis tendon under MRI after PT, PRP, supplements etc? Thanks for answering my questions, newly subscribed.

Appreciate the sub! Once something has been damaged and even if it's been healed with prp or stem cells, it will never be perfect. An MRI may look normal, but I don't believe we can ever go back to the perfect old state (but I also don't think that we need it to go back to that state in order to have full healing meaning no pain and full function)

One last question, once the tendon has healed (no pain, full function) is it possible if kept healthy and looked after that you may not ever experience issues again with that tendon? Even at older age? Also, what are your thoughts on type 1 and 3 collagen supplements with hyaluronic acid and vitamin C to aid with tendon healing and repair? In this case, a tendon with tendinosis? I will be doing eccentric exercises to aid with strength. You've been a great help Drew. That's all I have.

Yes it's possible for you to never have issues with it if you continue to take care of it. We use a lot of essential amino acids instead of collagen products, but we use them occasionally

Can Tendonosis in the hip that goes undiagnosed for ten years cause nerve pain down in the foot? I've just been diagnosed with Tendonosis in left hip and I've complained of hip pain for ten years. So if I understand my ND correctly I had tendonitis first which never got taken care of, but I had pain and kept telling Drs. I've seen dozens of Drs all this time trying to get help. They've talked me into an epidural nerve block (That Dr used steroids), a sympathetic nerve block (no steroids), prolotherapy injections into arch of foot (3 times), ozone injections into lower back/SI joint/ankle, foot (15 times over two years), I've been to numerous pain clinics, kept asking for 2nd and 3rd opinions, they've told me I have mental problems, (well it is depressing), I've seen numerous chiropractors, NDs, diagnosed with chronic Lyme (which probably caused the tendonitis in the first place.) They just kept looking at my lower back and SI joints--imaging has been done over and over again. My treatments have been expensive, painful, exhausting and maddening. One Dr wanted to do more imaging of my neck. And yet all of them never looked at my hip joint. I am just baffled at how my left hip joint was ignored all this time. It's been like medical abuse.

I'm terribly sorry to hear you have been through all of that. Your story is sadly not unique as many others have been in similar situations. Tendinosis usually does not cause nerve pain, but sometimes tendinosis can cause other changes that result in other nerves becoming irritated.

@@DrewTimmermansND Thank you so much for replying. I think I'm seeing a top notch ND now who is semi-retired. And he does regenerative treatments. PRP will be done for the tendon over the hip joint and he said he may also have to do prolotherapy for the nerve on the front pelvis area. Forgot the name of that nerve. He wasn't sure either if the nerve pain in foot was connected but we won't know for sure until the hip area is taken care of. I'm relieved that finally there is a plan that seems promising. A middle-aged lady with hip problems and nobody looked at the hip! I'm just baffled and exhausted over this.

I saw a doctor who said I shouldn’t have so much pain from Severe tendinosis of left gluteus minimus tendon at the attachment…that’s how it reads in the MRI. Would you agree? I tore this about 5 years ago and it still bothers me greatly. Suggestions? Please help!
