Alex and Dr. Scholz compare and contrast high dose-rate temporary seeds (HDR) and low dose-rate permanent seeds (LDR) for the treatment of prostate cancer. While there are no head-to-head trials, both forms of seeds seem to work about the same. Dr. Scholz suggests that HDR seeds may be better for more serious cases of prostate cancer since they are better able to go outside and around the gland, but that LDR seeds are more convenient.

In high-risk situations, temporary seeds might be preferable because they can treat around the prostate better. Permanent seeds, though, are more convenient.

Seeing a Radiology Oncologist next week after getting a Gleason 8 from a biopsy two weeks ago. Bone scan and CT scheduled beforehand and will be interesting to learn what the R/O Doctor will consider and present during this consultation appointment. Your videos offer such valuable information and available avenues to deal with this diagnosis and is greatly appreciated. Always look forward to watching and learning from them. Thank you👍🏽

You folks are great, thanks so much. I recently had a biopsy and afterwards I bled profusely, blood poring down my legs periodically for 2 days, all over the bathroom floor, it made me sick and I threw up and almost passed out. I have Crohns with a rectal fistula. Afterwards I thought that maybe a better procedure should have been done to avoid so much bleeding, maybe a targeted MRI that wouldn't be so invasive and cause so much bleeding My question, will the same happen with having low dose seeds implanted into my prostate??? I don't want to go through all this bleeding again. My doctor is pressuring me to have surgery, removal of the prostate but that won't happen. My PSA is a 10 and my Gleason score is 4 plus 3. I never smoked, don't drink alcohol or never drank coffee and have low blood pressure, my weight is normal for my height. . Thank you

Wow , um , I’ve had to look at a number of vids to determine what is going to happen . You tube doesn’t sort , they just pile them on . So that help confuses people . Um , I’ve been looking at this series , exclusively . So I think you might have answered my questions . At least I hope so . Um , but a second apinion might be in order , but thank you .

i will have a biopsy at the end of the month. I am learning about all the modalities of treatment should I need my prostate treated for cancer. The Dr. above states that with any type of radiation or seed implant there's a risk of erectile dysfunction and urinary issues but those blow over.pretty quickly. Is the Dr saying that these side effects are short lived like only lasting a few weeks or months? thank you, Erin

I just checked the video and it is a little confusing, so I will clarify. He is only referring to 'urinary bother' when he says, "it usually blows over quickly." We have a few other videos discussing the side effects of radiation--and other prostate cancer treatments too--that you can find by searching "PCRI radiation side effects" (or whatever you want to know more about) and they will go into more detail about the side effects. Unfortunately, all radical local therapies for prostate cancer have a risk of causing erectile function that does not recover on its own, and that risk varies greatly depending on a person's age, pre-existing function, and overall health. There are differences in the side effect rates and severity between different treatments (primarily between surgery or radiation, but there are other niche modalities too) and depending on the skill of the physicians and the rest of the team responsible for delivering therapy. Hay dos tipos de implantes de semillas radiactivas: 1. Las semillas permanentes permanecen en la próstata indefinidamente 2. Las semillas temporales se eliminan al final del procedimiento. Por lo general, ambas opciones son tan buenas como la otra, aunque suele haber preferencia por semillas temporales en los casos en que el cáncer puede tener una invasión local fuera de la glándula. Generalmente, la calidad del oncólogo radioterapeuta es más importante que el tipo de semillas.

Why are doctors no longer providing brachytherapy? My husband cannot find any physician that does this procedure.

Because there is not enough money in it. Everybody is pushing surgery.

I don't know where you live, but I had my brachytherapy done 10 days ago in Florida. It's available everywhere here. The procedure was simple and I'm feeling great with no apparent side effects.

@@DAGenao im Africa America in the state of Ga, i was diagnosed with prostate cancer at age 48 ,im getting ready to do the seed radiation implants im scared, i don't know if im making the right decision anymore, one says do surgery one says do this 😢, i don't know what to do😪😢😭, how long were you out of work, and what was your side affect.

@@polofunk Hi. You don't have to be scared because doing nothing is the worst you can do. I had the seed implant 8 months ago and I'm doing great. The most important part of seed implant is finding a doctor who has a lot of experience doing the procedure. This said, there is no treatment that is risk free, and they all have their side-effects. At age 48, you're still young and it is safe to assume that your sexual functions are still intact. If that is the case, seed implants are probably the best option for you. I'm 64 and I have not noticed a decline in my sexual activity after my procedure. Another point is that the seed implant procedure is an outpatient procedure and much less invasive than surgery. With surgery, you also have the issue of a decrease of penis size because when the prostate is removed, the surgeon needs to pull in the urethra in order to attach it to the bladder. Many patients are never told this important detail. You may also have issues with urine leaking and slight incontinence. The most annoying part of the seed implant is frequent urination and a weak urine flow. I experienced it for the first 6 months, and it can be very annoying. I've been taking Flomax to improve the urine flow, but the frequency of urination is almost normal now. My doctor told me that after about a year, I'd be able to urinate normally and most likely won't need the medication anymore. There are nights when I don't even have to get up to go to the toilet. Other days, I have to get up once or twice, which is fairly normal. Let me know if you have other questions.
