The westside story one and the kung fu fighting ones were definitely 2 of my favorites (betrayal five - gotta love it) but they left out the day dream J.D. had when eliot was getting made fun of for giving a lady a orgasm during a pelvic exam.
I love how in JD's fantasy Turk and Carla's actual baby was white. Personally I think my all-time favourite scrubs fantasy is the one in "My Butterfly" in which initially JD pacifies Kelso by playing the banjo, and in the other version Kelso and Cox entertain each other by playing the banjo and washboard. Died laughing when I saw that.

I think the most underrated JD daydream is from Season 1, when he's fantasizing about having this guy's perm. Easily overlooked since it was like 3 seconds long, but it was hilarious. If you're just going seasons 1-6, I'd also include "JD's funeral" from Season 6. Also like the Chorus Line homage from Season 4, where that ER chick is nailing guys in the groin. Cheap laughs, but, as we see in the Season 4 Bruce Lee homage, total commitment takes the Cheap Laugh to another level...
What about the entire episode "My Occurrence"? The whole episode was a daydream of JD's to help him cope with giving a cancer diagnosis to Dr. Cox's brother. People forget just how this show could touch on the feels.
moment 5 made me realize that oliver stone stole from star wars for platoon. sgt barnes = darth vader, and sgt elias = obi wan. they were battling for taylors soul. of course that has probably been reused in literature throughout history again and again but those r the specifics that came to mind
