Does a Mirena IUD work similarly to BC tabs in perimenopause? I am guessing not because it releases only progesterone and not estrogen, right?

I am in peri menopause, and have had back-to-back IUD’s (Mirena) since 2014. I haven’t noticed any improvements. In fact, I went from no periods, now periods every 2 weeks. 😭

It’s refreshing to know I’m not alone and not crazy and that it isn’t in my head because it happened all of a sudden and although it’s the early peri menopause I cant imagine going through this for 10 years 😫gulp

It hit out of nowhere. One day I was fine and then all of a sudden I’m having all the same symptoms you just described. I went from being able to sleep well and feeling calm to anxiety panic attacks and insomnia. I didn’t even think it was possible for it to be a rapid change like that. Actually I don’t know if that’s possible

I just turned 44 .My periods were always on spot ,but from this April suddenly I started having hot flashes, night sweats and disturbed sleep. All together was very overwhelming and emotional for me .I had always taken care of my health and mom had her perimenopause at 48 and hit menopause at 53.This unexpected drama has hit me hard. I did my blood test and it's shows I am in premenstrual phase. But my periods is delayed by 5 days ,going through cramping . Feels like I have been hit by a wrecking ball

Thank you so much for helping women in menopause! I hit the menopause symptom jackpot and have every symptom! I’m 48 and started perimenopause around 44 it’s been rough! Bc did not work for me it made all symptoms worse. I had the iud put in (Merena) and it’s been 3 months and is so much better. It has not been helping my complete loss of libido. I’m sharing my experience on my channel to bring awareness to this highly neglected transition women go through! I was fine then boom overnight I feel like a different person. I’ve never had PMS issues before 40. I have anxiety & just started with the insomnia. Constantly worrying and overthinking. I just don’t feel like myself 😢

Thanks for talking about birth control being a good option. I’ve been on a continuous pill for many years and at 49, have had very few symptoms. My doctor encouraged me to stay on it for as long as I can and I’m so glad she did.

Thank you so much for talking about hormone therapy during late Peri. I have been told I am not ready for any HRT twice because my period comes every 25 days or so (even though now, at age 48, that it's very light and has tapered down to 24 hours vs 5 days of bleeding 6 years ago, which had akways been like that my whole life). I have theorized the docs are afraid to treat me due to very low risk antiphospholipid antibodies in which my rheumatologist said I can absolutely take HRT at the lowest dose needed to control my symptoms. I have not had a clot and do not have the dx of antiphospholipid antibody syndrome and yet they put that down in my EMR as a dx. I also have MSH6 Lynch (high risk for endometrial CA esp. High after age 50 and up) and have not had a hysterectomy yet because I have horrible sensitivity to hormone changes and due to their reluctance to treat me with hormones I am terrified that they would severely under treat me after a prophylactic hysterectomy and oopherectomy. As of now, my endometrial lining is never thicker than 5-8mm. I have intense brain fog, joint pains, back pains, fatigue, anxiety, night sweats, mild hot flashes, and decreased strength of orgasm and decreased lubrication and worsening muscle weakness as well as irritated, itchy vulva ..all of this started Sept 2021. I had always had bad PMS but it has now started happenning all month long except the week before ovulation (though many of my cycles are anovulatory as far as basal body Temps tracking goes). Everything I read about HRT according to NICE guidelines says I should get some sort of HRT and this is why I theorized that the docs are petrified to treat me. I forgot to add that at age 46 I was diagnosed with Osteopenia. I feel estrogen would greatly help symptoms of mine and help reduce my risks of osteoporosis and colon cancer that I have.

Wish you were my doctor! Learned so much! I’m 44 and started having bad peri symptoms right before period! I’m miserable!! I have an appointment with Gyno on Monday, and I’m hoping she listens and helps me!! I want to use the patch. Anyone know good doctors in Minnesota if my doctor ends up not helping. I just get scared that doctors will gaslight and not help.

Hello, just had my yearly with my fairly new gynecologist. I am 46 an in early perimenopause. After genetic testing, I have the ATM gene that my provider is concerned may cause breast cancer. We are discussing a Morena IUD. I did track my symptoms and discussed them with my provider.

Has anyone gotten horrible symptoms like 10-14 days before their period. Fatigue, cold sensitivity, and just feeling awful. If so, what do you do to help your symptoms besides BC?How old were you when you started with these symptoms? How do you know your definitely in perimenopause?

I do. I did the terrible non eating diet life in my teenage to just before 30s so I have a lot of issues started happening in my already 32 ish and got worse in 40s. I am 44 now and boy...I have so much worse everything...almost always having pms symptoms for good 2 weeks then probably feeling somewhat normal for about one week.. When I didn't know anything better what the hell happened to me almost feel so sudden around my mid 30s, countless doctor visits and tests started no joke..Well I am aware now that I have been suffering from perimenopose since my 32ish. It's on and off of terrible situations but over all you have not having any same body situation that you had in 20s. I just do natural way like take supplements for it, more efforts for good diet but there is still no miracle tho but I don't want anything chemical meds so try to change and adjust lots of things in your life like you became something else different seriously lol..

Thank you for such a GREAT explanation of this. I will be watching more. I have a rare neurological disorder called Small Fiber Neuropathy. I am on Lo Lo Estrin and it stops my menses because I can't handle to inflammation. My MD has told me that I can stay on that until 50 and then will transfer to another med. If you want a crazy disorder to look into, look into mine. I am curious how menopause will be affecting my body as my disorder progresses with very dysfunctional small nerve fibers.

Every video out there is about low progesterone! What if you very recently had Mirena IUD to control adenomyosis symptoms, and now you just feel tired and sleepy like a teenager, what would that be? 1) Body adapting to Progestin dose 24/7? 2)Or progesterone still on the low side? 3)or tipping on the slightly high side? 🤔 Can you make a video focusing on symptoms of higher than normal progesterone?

I'm so glad I came across your video! I am on my second period in a month and it won't stop. I am 45. My doctor put me on progesterone to slow down the bleeding but it isn't working. She keeps telling me to get a hysterectomy due to prolapse and irregular bleeding. We have been trying for a baby but I think it's time to give it up and treat my perimenopause like you said with birth control. I don't know if I am ready in my mind to do the hysterectomy.

I have undergone the same thing 4yrs ago (I'm turning 49 next month), super strong periods/bleeding. I just managed the anemic side effect of bleeding by eating green and leafy vegetables and taking liquid supplement which helps in blood health improvement. The important thing is you keep your hemoglobin up. After this, more on hot flashes and brain fog. I was having a hard time at work and stress triggered anxiety episodes, palpitations, talkabout whirlwind of things happening within your body. My OBGYN suggested scraping of my uterus wall due to blood (due to wall thickening) whenever I have urinalysis (with all else checked) but I declined. I will try the birth pills and see what happens.

I'm only 39 and already peri-menopausal, having random periods that have no steady rhythm. (I had the perfect 28 day cycle and knew when it was coming to the day my whole life till I hit my late 30s.) I already get hot flashes and night sweats and the other unpleasant signs of doom but I can't take hormonal supplements because I have high blood pressure. What are my other options when hormones are off the table?

I'm 52 and suffering in perimenopause. My worst symptoms are moods, anxiety, palpitations, insomnia. I have irregular periods and doctors I've seen in my area won't prescribe estrogen because I'm still having periods and don't have hot flashes. All they give me is progesterone which causes a ton of water retention and hunger. What can I do?

I’m 44 and started the Nuvaring. I plan to skip the week that I remove it to have my period. My headaches and fatigue are awful right before and during my period. I tried birth control pills and the Mirena but my headaches were dreadful the entire time. My periods are regular but my PMS and flow are worse than ever.

Any thoughts on using Estroven plant based pills? My MD wouldn't give me anything else due to my chronic hormonal migraines because she said any extra estrogen could up my stroke-risk. For the most part, the pills seem to have really helped with the hot flashes and most of the mood swings but my migraines have been a challenge. I also got prescription strength migraine meds I take occasionally.

Hello Dr. Hirsch, Thank you for your educational video. I hope you're able to address my questions. Would any nutritional supportive supplements (vitamins, minerals, NAC, ALA, RLA), naturopathic hormonal balancing supplements, and/or any Rx interventions delay the onset of menopause? Second, Is there any guarantee that a partial hysterectomy, would provide my body a lifelong, adequate supply of estrogen/other applical hormones from the remaining ovaries or ovarian tissue, that I need if I'm already in the perimenopause stage? Thank you

@@heatherhirschMD It is. I have been supportive. I have loved her no matter what. I have faith that eventually it’ll get better. My wife has been on HRT treatment with our naturopathic doctor (progesterone, bio-identical testosterone…with regular bloodwork). She also has a thyroid problems, autoimmune disorder, as well as PTSD from childhood trauma. I was just venting and trying to seek answers. In a moment of frustration I decided to Just comment on your video after hours of watching videos on perimenopause. I love my wife very much, but sometimes I take a beating and it can feel like I’m alone. Thank you for responding.

Hi there, i love your video. I am on day 3 of taking progesterone cream, not an order by the doctor, but just me trying out different things, because all my gyno could recommend was take the pill, this pill was ultra low dose, but even then, it gave be such high blood pressure and spotting everydaym i felt dirty, and had PMS everyday on that pill. Reason is, since i gave birth to my 3rd chid at age 35, i started to have night sweats every night. i thought i had cancer. My periods then became so heavy i couldnt leave the house. i would soak through pads and tampons every hour. Now that i am 39, i still suffer heavy bleeding, and clots, the clots are getting bigger unfortunetly, almost the same size as the palm of my hand!!! i have had to dea with really bad anemia for years now, and its wearing me out. SO i though i would try to find a treatment by myself, since pill doesnt work. DOnt know if this will work. I have had PMDD, i found that taking multi vitamins and minerals helped my moods a bit better. I also have zero sex drive.

This is literally like I wrote this. I had my son at 33 and have been going through exactly the same thing. I became severely anemic this past January and was bed ridden for 2 weeks from not being able to breath and fainting spells. I’m 39 and for the last 2 years have been asking what can be done. I had an ultra sound and test, no polyps, can’t do Ablation because you uterus is too small apparently also both my PCM and OBGYN said my only option is Full hysterectomy add to go on HLT which I don’t want to do. My dr just put me on lo loestin FE to see how that works but I haven’t been on birth control in years so I’m really nervous (I start tomorrow) about the side effects. It’s been a nightmare. How are you doing now? Have you found anything to help?

Just found you through Angie (hot&Flashy). I am 49 and perimenopausal for nearing 3 years. I still get a period each month which is normal for a couple of days but lingers lightly for 10+ days lately. A year ago my doctor put me on progesterone (Started at 100 but moved to 200 nightly) which has helped a lot with the symptoms of sleep, anxiety, headaches, and brain fog. He also has me taking supplements vitamin d/k2, magnesium to name a few. I have a decent diet and exercise 5x a week. Lately I feel the symptoms of anxiety, brain fog, lower libido sneaking back in. I have a doctors appointment in May. Should I ask about estrogen? What conversation would you recommend I have with my doctor? I haven’t found the best doctor for me yet (If that exists in my area) but he’s not bad.

Love your videos! I was to by physician I could not take BC pills with progesterone and estrogen because my mother had a stroke before age 65. I was given no other explanation 😢 May I ask why she would think I’m high risk? I don’t smoke at all. I don’t have high risk of cancer, no other diseases or high blood pressure. I just don’t understand why I cannot have this sort of BC🤷🏻‍♀️ I was on the Depo provera shot in 1997 which was the last time I had it but previously on the Pill for 10yrs. Please help me understand because this peri menopause is no joke and I feel sad for other women who might go untreated for lack on information

I was treated for perimenopause, or at least what they thought was perimenopause for five years until finally, they realized I was suffering from primary hyperparathyroidism. I say this, so that patience dig further than just settling for popping pills during perimenopause to mask symptoms.

Hi, I was on Femodene for 20 years. I stopped using it on my 50th bday in 2022. Since then no period. I do have some hot flushes, but nothing as bad as ladies described it. What I do have is terrible joint and body aches. I do take Vit D, some Calcium, Vit B 6&12, magnesium. I am a vegetarian and do not eat any meat. I exercise and do intermitted fasting 16/8. Drink my 2 liters of water. I think I am quite healthy, but the aches are very very bad.

I've been a healthy, fit and active vegan for over 20 years. That didn't help me at all with the peri symptoms :( I'm having an awful time and being healthy all this time was not the fountain of youth I was hoping for.

I’m 48, my periods are still pretty regular (schedule wise) I’ve had a couple times where I skipped a month and since I had always been pretty regular and now they’re super heavy. I knew something was going on. I also was having palpitations , anxiety attacks . Light night sweats, hot flashes. Not terrible but annoying. I do have anxiety, some light depression. I told these things to a doctor, a woman doctor also. She totally dismissed me . She was really young, so maybe that’s why. 🤷🏻‍♀️

Hi Dr I want to ask you I went thru early ovarian failure at age 36 I’m now 42 and I was on birth control pills (viorele ) for 3 years and my dr switched me to estradiol 1mg and medroxyprogesterone 10 mg for ten days a month ,after a couple of months my hair start to fall out and everyday I felt my hair was thinner she increased the doses to 2 mg estradiol I continued taking it for 14 months I honestly didn’t know if discontinuing bc made me loss hair or the medroxyprogesterone was making me loss hair however I’m back on the bc now after 14 months of HRT I don’t know if I made a good decision but because that happen to me before now I’m really scared if that same condition comes back to me after a couple of months of switching back to bc please tell me what u think thank you.

Thank you for an informative and interesting video. I am 46 years old and am perimenopausal and was just prescribed HRT today for the very first time by my fantastic and trusty woman's health doctor/GP. I am about to start on Estrogel (2 pumps nightly, preferably) and Prometrium 100mg tabets ( taken for 12 days per month on 6th to 17th starting in Feb). Im nervous to start but excited to hopefully see my terrible symptoms subside and/or improve. I was told to use two pumps for both upper arms or inner thighs - I think I'll do 1 pump per arm/thigh for a more even distribution. Is this correct? Can I stick to just the arms or do I need to alternate between arms and thighs? Thank you :)

I had a uterine ablation done 5 yrs ago due to heavy bleeding and no longer have periods my age is 52. I think I am in premenopause. Can you do a video on uterine ablation? Does having an ablation increase your risk of uterine cancer? Does having an ablation cause you to go into menopause sooner? Any info on this would be great. Thank you.

Would depo shot be an option for bc for pre menopause. I have fibromyalgia and CFS. My periods made me so ill before. The depo shot was the only one I had no side effects and no period. Just this month I’m having some break through and I noticed crazy 😜 insomnia and all the other fun symptoms out of no where.

I’m on continuous BC pills because of migraines I experienced during the placebo phase. I’m suffering thru peri currently (44), and dr gave me doxapin too help me sleep because I’m having horrible formication episodes. I was just having them in the evening, now it’s anytime of the day that I’m trying to sit down and relax. I can’t take the doxapin during the day cause it makes me dizzy and tired. I take benedyl but it isn’t working anymore. How do I tell my dr I can’t live with this side effect of peri. She said she was averse to putting me on hrt because it requires contact monitoring.,,🤷🏻‍♀️

I am 39 and last year i started having horrible heat flashes, night sweats, insomnia and anxiety. I have been put on the pill and it's been 9 months since i got these symptoms. I was avoiding the pill because my mom had breast cancer but i had to take these to remain functional (i was on the verge of quitting my job). I still live in constant fear of getting the big c. There's another doctor I've been watching as well. She talks about the breast cancer risk of birth control and it isn't as high as we have been made to believe. Her channel is called: menopause Taylor. She helped me less afraid of birth control, hrt, etc.

I found out the term PMDD when I was 38 and diagnosed myself with it, not suspecting it might be Perimenopause. And I stopped having irregular periods when I was 35 - always had 35-40 days cycles. Well ouch 😳

I am 41. I have so many symptoms of perimenopause it's not even funny. Is this possible? The vaginal thinning/dryness/cracking, the depression, anxiety, feeling like I'm walking in a daze, dry skin, dry lips (on my face), the fatigue, the fewer and less days of periods, the tingling in my feet (not all the time), the introversion, night sweats, insomnia, waking up drenched in sweat, joint pain, muscle tension, brain fog. Just to name a few Anyone have any suggestions? Can this be perimenopause at 41??? Please any suggestions would be greatly appreciated. This is no way to live!!!!

If you take birth control skipping the placebo pills, will your period stop or will you still have a period? If your period stops how to tell if you are in menopause? If you have a period is there say 28 day intervals? Thanks for your help, Brenda

Supposed to have no period if you don’t take placebo pills. Dr usually have you on it till 55 then switch to HRT. You won’t be in menopause if you take bc pill till 55.

This perimenopause state is extremely confusing in terms of management options. Dr told me my only options are 1. Low dose birth control pills which I don’t want to take due to stroke, blood clots risks. I had mini stroke during pregnancy 19 yrs ago and it still scares me. 2. Nuvaring, vaginal ring. Once again I prefer not to use synthetic hormones. She said no I can’t just double the dose of pharmaceuticals bioidentical estrogen gel and progesterone pill (promethium). It’s still too low of dose. I’m at a loss what my options are. I read comments that perimenopause women are taking bioidentical hormones. So I assume they’re seeing functional medicine people and getting compounded hormones?? This functional medicine route seems pricey since you have to get all these labs /tests done and insurance doesn’t cover visits. How much do you spend? I heard issue with compounded hormones is it’s not regulated so no guarantee dosage you’re getting is accurate. They focus too much on testing vs focusing on how your body feels/reacts to the hormones.

I don't think this would work with someone with adhd because from what I understand, the type of estrogen in birth control pills or HRT does not affect the dopamine levels in the brain that fluctuate due to imbalanced estrogen which is what women with adhd suffer from.

Hi Dr Hirsch, could you please tell me if it's technically possible to be in early perimenopause at age 52? My doctor prescribed the birth control pill for perimenopause but I haven't started the treatment yet because I'm wondering if it's truly possible at my age. I'm a non smoker but exposed to public second hand smoke (live in Europe). My periods are regular and have been heavy for a couple of years now due to uterine fibroids. Hi and thank you! The average age of perimenopause in US is 47.5 and menopause is 51.5, but that is just the average. I have women who are not in menopause until age 55 - which means you could be in perimenopause. Since I have not seen you personally, that is what makes it harder to say, but age wise is a definite possibility

@@heatherhirschMD thanks for your feedback Dr. Hirsch. It's so confusing as a patient in the world of perimenopause. I tried micronized progesterone day 15 to 25 for 6 months but no change in heavy bleeding due to fibroids. Next doctor says try birth control pills with combined estradiol and progestin to to block ovaries and hopefully reduce the heavy menstruation. Another doctor says I'm too old at 52 to go on the pill and should just treat fibroids with embolization. It's difficult to know who is right... Tough being in perimenopause and unable to set up an appointment with you. This video has helped me better understand my options, nonetheless! Thank you for your uplifting demeanor.

Weight gain, dryness especially during periods (all cavities and hands), night sweats during period, irregular periods (spotting and coming often a week earlier, lighter but length is the same), decreased drive, etc. Joint pain... They tested me for sjögrens syndrome and that was negative... I asked my Dr. On my one ovary because I am having glandular issues (MGD) and I heard glands can actually utilize other hormones from glands... He told me my one ovary by itself would be compensating for the missing one but that's different from some things I've heard.

I am 38. I still have regular period. However, my mother has history of early perimenopausal- when she was in her 40's. I saw my OB-gyne last week and have my lab test. My (Follicular Range) FSH was 4.6 and my Estradiol was 22. Is this something I should be worry about due to my Estradiol was near the lowest normal limit? I have a prescription for Microgestin 1/20. If I am not going to take the 7 placebo pill, will I still get my period?

My doctor told me that your levels are not created to be completely stable (especially women), because it is actually dangerous to not give your receptors and your body a break from the hormones. Your hormones are either going up, or they are going down. The increase is great, but it’s natural and even safe for those “down periods”. Hence why pregnancy and contraceptives increase the risk of blood clots, constipation, etc”
