Theoretically you could have been insane at the time of the crime, but sane during the trial. So if you were found not guilty by reason of insanity then you would be released immediately after the verdict. But in practice most people end up in a mental ward as bad as prison for decades.

Obama is still called "Mr. President" in his interviews for example, not "Barack" same with Biden, when he was seeking re-election he was called "Mister Vice-President."
