Fan theory: i think medic modified that hat with engineers help to contain a device which emits a ray or waves that stimulate rapys Brain, taking him out of his natural carnivorous predator behavior and giving him even some sort of self conciusness and a certain degree of intteligence, at least enough to recap demomans "see"(for what reason ever, i also think he is under Demos handling because hes aside from heavy the physicly strongest of the team and can handle a rogue rappy). If the device is removed, the Brain need a some time to find back into it's natural way of behavior what is kinda a natural safety measurment so you can put the device back onto him. If not...
