Myofascial pain is a common diagnosis and is an accompanying diagnosis in many of the conditions seen in my clinic. Myofascial pain is pain arising from the muscles and soft tissue of the body. An orange is a useful analogy. The skin is the peel which once removed reveals the fruit (muscle) and the connective tissue, the little white part (fascia). The fascia or connective tissue helps give the muscle structure and can be a culprit in musculoskeletal pain.

Been having issues for years. I’m going to ask my doctor next physical. Tissues in legs hurt. Not all the time but when pressed on. The pain hurts but feels good at the same time. But I have pain everywhere also in arms, butt, and shoulders. I massage when I can. I’m not highly active but don’t just sit around. But with age I have slowed down. I do stretches and mild exercises but maybe I have to do more? Nothing seems to work. 40 yrs old and seems to get worse with age. Thanks for video.

I injured my shoulder 7 years ago was diagnosed with myofacial pain syndrome, I’ve done years of therapy, strength training, IMS dry needling, you name it. Along with heaps of medications, I have a TENS machine, along with a entire tool box of things to try and survive the agony. I keep up with my stretches and exercises give to me from my physical therapist and occupational therapist. I did a program on chronic pain and added in medication, body scanning, laughter, walking 45 mins a day. I also suffer from severe tension migraines which my neurologist prescribed me Botox injections along with IM injections I self administer, I now have scapular winging on my injured shoulder. I feel hopeless. Is it possible to have this pain for life?

Hi Ronnie. I'm sorry to hear about your struggles with myofascial pain. Scapular winging in itself is not that big of a deal. Scapular dyskinesia is incredibly common and poorly correlated with pain syndromes of the neck and shoulder. Stay positive and keep moving!

My left shoulder blade was covered in knots and now has spread all over my back into my legs. Pulling from all directions. Knots in jaw wrists butt knee I'm so tight some days muscles don't move. Wake up in the night so stiff like anstaye alot of pain. I'm in Ontario Canada and no doctors are helping me. Except pain med. I've spent a fortune on massage and everyday I roll on the ball breaking up knots. Almost makes it worse burning and can't move. feels like all my fascia swelled up and is constricting my whole body. Not sure how much longer I have to still walk

What happens if this condition is left undiagnosed and untreated for 30 years? Now physical therapy is impossible and the levels of pain from a few affected areas is off the chart and even causing fainting. How do I go about getting treatment? And what treatment is required now after all these years?

I've tried everything that is supposed to help. My body does not care. My muscles are full of marbles that are indestructible. I went Wayyyy to hard in my youth. I was extreme in Everything I did. I worked in tree removal for over 15 years as a no limit climber and was an absolute beast. I've experienced countless incidents of trauma. Now I'm mid 40s and keep telling myself "Well, the good news is I can't live forever" there is now no quality of life . Multiple surgeries on my hands and arms. Two very Invasive Carpal tunnel surgeries ,cubital tunnel and ulnar nerve transposition and then a posterior interosseous nerve decompression surgery. All those surgeries have run their course and the nerve damage is progressing. I've been advised that there is no useful reason to revisit the surgeries and just accept the loss of function as the nerves slowly die... I'm ok with that. It's the marbles that make me want to fall asleep and never wake up.. I've not experienced "comfort" in so long that I don't even remember what it's like.. and thank god I have no reference point because thinking of that would only make it worse. The depression and psychological effects are just as bad as the unrelenting pain that is existing. It never goes away, there is no break. It's ALWAYS there. My muscles feel like they are from a person half my size and stretched over my skeleton. I've been to an absurd amount of doctors and specialists and chiropractors. So much physical therapy it's insane. Every doctor/physical therapist is blown away how my muscles are so tight and filled with marbles. I've been apologized to many times with things being said like "I'm truly sorry you're going through this but there isn't anything I can do to help you. Your muscles haven't responded to anything that we can do" or some variation of that.. The only thing keeping me on this earth is the wife, kids, and grandkids. I can't imagine what I'm gonna be in 20 years other than an emasculated bitter Angy useless old man that no on wants to be around.. I'd rather be a memory of when I still had some of my soul left and something to offer. There is only so much "Man the fuck up" in ones gastank.. when that's gone the pain feeds off your soul.. stealing it bit by bit day by day til eventually there will be nothing left but an empty husk of the person you used to be completely unrecognizable from the person you used to be... . Sorry for the rant.

Good video, easy to understand. Though it seemed mostly aimed at inactive/sedentary people. I am a carpenter and have had issues with the scapula for 5 years and it must be overuse/dyskinesis or both 😊 thanks for the help on my little journey of understanding the problem

I think your assessment of the video is correct. In your case, it would be more about balancing use with your job and maybe focusing on different rehab exercises to better prepare your shoulder girdle for the work.

Very interesting analogy, question: could I have myofacial pain syndrome but no trigger points ?
@johnathanabrams8434

Myofascial pain syndrome is characterized by the Myofascial trigger point. It is the Hallmark for Myofascial pain syndrome. It can only be diagnosed by systematic palpation.

I'm not sure either I have this but whatever you think helps. When I wake up I'm fine but as the day starts I start feeling awful. When I lift my arms it feels so tight like I did 100 push-ups even putting my daughters hard in a ponytail is alot on my arm muscles. When I walk for about 5 to 10 steps my leg muscles feel like I did 20 squats and they feel like they wanna give out. It feels like a weakness but I can get up from the floor, chair and lift things with no problem. When I sit, lay down or even stand I feel it come on like a electricity going through my body. It seems like my body is gonna go through a shock. I don't get enough sleep at nights I get up with aching back and sometimes to my legs. I get the tingling that wakes me up and I try to sleep it off. I am low on vitamin d, iron, b12 and magnesium. I am taking vitamins to help raise my levels but the problem is that feeling and that whole tension electric shock going through my whole body.

Thanks for sharing your story, Stacey. This video is not intended for medical advice and given the complexity of your case you should consult with your physician.

Dry needling and muscle re-education is really all you need. The practitioner seems to leave out myofascial trigger points almost always become chronic and sensitize the central nervous system causing allodynia , hyperalgesia and the spread of pain to unaffected areas . Myofascial pain becomes increasingly difficult to treat is left untreated
