I was stuck, everything worked except I had no modulation on TX output. As you demonstrated at about 4:00, the "Rear Select" setting was the issue. Once I switched it to "USB" it worked. Thanks!

Yes, but this PRESET mode assumes you are going to use the USB port for your data modes. It doesn’t work if you want to use the other ports on the radio. Also note that the USB port has a 16-bit A/D soundcard which is the same as your PC. However, some external data devices (microHAM for example) have 24-bit A/D soundcards.
Totally correct on the Microham Keyer 3 for FSK ie use Of FT 8, which uses microham USB Router with software to add as many Ports for use CAT x 2 plus Ports for all different PTT modes. I had to give Ports for Phone, FSK and Foot Switch plus another Port for Headset, and change FTDX10 setting to RTS for FT8 to work, mindful this how Keyer 3 operates.
you probably have rfi in your signal try separating your cables from the coax and wind a mix 31 around your usb cable as many time s as possible. lastly check you coax plugs are tightly wound.
If you use DATA-U or DATA-L a 3kHz BPF is used and you can go thru the mic, USB, or rear ACCY port. If you use USB only, you MUST use the MIC port for TX and you can vary the RCV BPF width.
