Historically cortisone shots in the knee have been one of the most common treatments in orthopedic surgery. We still use them for patients with knee arthritis, and occasionally we will try one for a patient with knee pain and swelling for other reasons. In this Ask Dr. Geier video, I discuss the pros and cons of cortisone injections as well as what they actually do.
Please note: I don't respond to questions and requests for specific medical advice left in the comments to my videos. I receive too many to keep up (several hundred per week), and legally I can't offer specific medical advice to people who aren't my patients (see below). If you want to ask a question about a specific injury you have, leave it in the comments below, and I might answer it in an upcoming Ask Dr. Geier video. Rawya in North Carolina asks:
I have a degenerative posterior horn meniscus tear in my knee and a small ulcer in the bone. It has been almost nine weeks. I’ve tried anti-inflammatory medications and ice, and I saw several doctors. The knee is swollen and uncomfortable. Would a steroid shot help? Or would extraction of the fluid help relieve the pain? Are there any side effects to the cortisone? Or would the swelling reoccur, and if so, would it be more severe?
Cortisone is a steroid that acts to decrease inflammation in the knee. In some ways, it’s much like taking a large dose of anti-inflammatory medication like ibuprofen, but the effects work only on the knee since that is where it’s placed. Cortisone doesn’t help the injured structure within the knee heal or return to normal, but it can help to decrease pain and swelling.
In this video, I share my thoughts on when these treatments can be a good idea. For what problems are cortisone shots helpful, and when would they not provide much relief? Are they harmful to the knee? Can I get too many cortisone shots? And what other options are available?
Please remember, while I appreciate your questions, I cannot and will not offer specific medical advice by email, online, on my show, or in the comments at the end of these posts. My responses are meant to provide general medical information and education. Please consult your physician or health care provider for your specific medical concerns.

I was diagnosed with osteoarthritis of the knee over four years ago. I had arthroscopic surgery to clean and smooth out inner knee. It did not help. Since that time I tried various supplements with only modest results. I also tried prp injections and some stem cell treatment with only minimal success. Finally I decided to try a cortisone injection from my VA orthopedic doctor on May 1 of this year. After 3 days much of my pain and stiffness vanished. It seemed to good to be true. But here I am 6weeks later and I have no discomfort in my knee. I do not try to overdue it. I have a night job where I can sit down often after walking some. I go to the gym 5-6 days and do swimming and biking only. No excess walking or running. Now that the pain and stiffness are gone it has allowed me to do more intense bike workouts and interval training. This is bringing my weight down and helping me even further with my knee. The Doctor at the Va told me that I could get these shotsup to three times a year if I needed. It is the best treatment I had yet. And I hope that with the increased exercise ability and weight loss, that I can prolong these effects. I just turned 62 and have been an athlete all my life. now I concentrate on low impact swimming and cycling. Perhaps my case is somewhat unique

@@CBT5777 I had both my knees replaced the following year. the right in February 2019 and the left in November 2019. Has been the best decision. The modern Knee replacements are much safer and easier to endure. But I have to continue my own physical therapy routine such as using the gym weight machines to strengthen the muscle surrounding knee. Also I have to do some of the stretching routines I learned after the surgery. Also it is important to lay flat on bed and elevate each knee 6-10 inches a couple of times daily, or weekly to promote alignment. I can bicycle 5-10 miles if I want. Walk with no problem. But no "high impact" like running, jumping, tennis, football or rough sport activities. You can use the cortisone until it doesn't work. Also you may want to try the synvisc gel injections. But, do not fear the modern knee replacements with a well qualified Orthopedic MD. They can last up to 20years or more, and no more intense pains. After 3 weeks you can start driving to go places. First 7-10 days are the hardest. Just get set up and plan for the recovery.

I've been suffering from left knee super stiffness and sharp pain from getting up from a sitting position, or even trying to criss cross my legs. Ibuprofen only gave minimal relief. I had my 1st cortisone shot yesterday since I couldn't deal with the pain any more and it has been the miracle injection. I woke up like a new person with no pain. I hope it lasts because I prefer to change my diet with anti-inflammatory foods in hopes of reducing inflammation, moving forward since cortisone does destroy cartilage with use over time. Thanks for this video.

I get shots in both knees every 3 months for the past several years. Usually by the time I get to the office I'm at a full 10 pain. I love the pain of the shot at that point (lol) because it's much less than the knee pain. The shot numbs it of course and within a day or two my knees are completeley pain free. I go from a cane to running and jumping and climbing ladders. I'm 55 with very little buffer left and my awesome Dr. and I agree we'll do shots until they don't work... then surgery, eek. By the way, when I got my first shots from this guy I looked away and didn't feel them at all... he's good.

3 months is too often, cortisone weakens the bone and if going down the road of knee joint replacements then we need to increase the periods to as long as possible between the treatments. I'm 58 and get 9 to 11 months from the treatments.

Thanks for the information Dr. Geier! I have slight swelling in the right side area like upper calf & the Dr. said it’s slighht Ortho Arthritis. Just wanted to know is arthritis can it occur on the side by the knee? I don’t have pain in the knee- just slight swelling around one side of the knee. I took Naproxen - no side effects. But, I’ve noticed - after I stopped regularly taken Naproxen - the pain comes back after a week, of non usage. Otherwise I am in great health. Thanks so much.

I've had cortisone shots for 4 years every 4 months. Worked ok until the last time, but I think it was because I did too much too soon and missed a step. Now I'm getting a gel injection. I have also had cortisone in my shoulder and have them in my heel every year for plantar fascitus, and the doc says I can get them as much as I need to...no surgery ! It only hurts for a day or so and then I'm good for months!.

I'm 57. I'm a carpet installer, and karate practitioner. The X-ray showed that I have very significant arthritis on the lateral side of my knee. I went to bed fine one night. Woke up I couldn't walk the next day. I got the cortisone shot and I'm back to where I was, even better walking and going to the gym and work. The only side effect that I had that got me concerned was a racing heart on the second day. I went to the hospital concern. They said it was the body metabolizing steroid fluid that was put in my me. That was controlled with anti anxiety med.

Hello. I have started using metholprednisone. But I do have high blood pressure issues, so I only use a short dosage. I have bone on bone arthritis on one side of my knee. so I know that I am likely facing a knee replacement eventually. That said I want to lose more weight before I decide on either a knee replacement or possibly an experimental stem cell bone regeneration treatment, I am considering next a cortisone shot and will discuss it with my new doc. Thanks for posting this. I have been looking for more info on these shots.

I’m pretty active and maintain a healthy weight.. however, have not been able to do much since my knee started hurting some 7 weeks ago. It,s driving me nuts. My doctor has suggested physio, bracing, strength training and rest. My doctor suggested the hyaluronic acid injection which I’m scheduled to get next week in her clinic, without ultrasound. I am scared of needles and would like to know your opinion if getting HA injection is the best course of action in my case. I can email you my MRI reports if you like. Thank you so much!

I had a swollen prepatellar bursa on my knee. They drained it once but didn't inject cortisone. I iced it and kept it wrapped but a month passed and it returned. So he drained it a second time and then he gave me a cortisone injection. I personally don't take medications much. So for me the biggest noticeable side affect has been the light headedness. I just noticed today that my calf below the same knee has skin discoloration. It's dark brown and kind of random shaped. It isn't dark brown like melanoma. It is kind of a lighter brown splotch with a few parts boiling up, like I was burned there. Is this related to the coritsone? I am in my late 30's. My knee isn't swelling up again so that's nice. But I am getting stir crazy not being able to exercise. And always having to keep it iced.

i am 15 and i have had a cortisone shot in my leg for more than 7 times because i have very bad arthritis. with my doctor, she did not use anything on me, so it took around 45 minutes because i didn’t let her put it in me because it hurt so so so much. when she did, i screamed so loud that people on the two floors below us heard. if your going to get the shot, i really recommend that you get it under local anaesthetic or laughing gas, it does not matter how strong you are to resist the pain, causing the build up of tension in your body and then screaming actually wears you out that you feel very tired after wards, trust me i’ve had it 7 times and the pain just gets worser and worser and worser. not better, and me personally stressing myself before i get it because i’m in so much fear of the pain makes me so tired afterwards and psychologically i don’t feel like the medicine is working and tell myself that it was just a bag full of pain, getting some numbing cream really helps , or not being able to feel the pain really helps.

I just had one done yesterday on my left knee. My OS said he doesn't like putting them into young adults (I'm a 19 year old college wrestler) but in my case, he thought it'd be worth a shot. I had a discoid meniscus which was taken care of with a lateral meniscectomy and since then, my knee right where the meniscus is has always had swelling and extreme pain; especially when running and even biking. As of today, I definitely do not have anywhere near as much pain and hopefully it is just that the meniscus had some swelling which can be taken care of without another surgery! I also had one done on my shoulder after having 2 labral repairs (SLAP Lesion and Posterior tear the first time and SLAP Lesion that extended anteriorly the second time). I almost tore it a third time and the pain was bad but after that cortisone shot, it's made a world of difference! Hopefully those are the only 2 shots I'll ever need just to get me rolling back into action!

Jean Paul Marrero Hello Jean, I as well had a discoid meniscus surgery which was done last September. 8 months later I am still in a lot of pain probably more than before the surgery. Is your knee better now? My doctor keeps saying it would take over a year for it to start feeling better but I am desperate. Apparently a discoid meniscus is not very common so I cannot find a lot of information online about it and its recovery time. I am actually going in for a steroid shot today to alliviate the pain and enjoy the summer a bit.

I had multiple times of X-rays and mri scans doctors said there was no tears in knee and no fractures. The doctor said at the time of injury the bone was hit hard but no fracture. Recently they said it could be ruhmetoid arthritis because they can’t find no problem in the reports. There was swelling and even the fluid reports say no infection. They are prescribing steroid injection will that be fine.

This is coupled with arthritic knees. My orthopedic surgeon also suspects an ACL tear in the right knee. I wanted to avoid surgery.Had a shot of cortisol in both knees. Hello Dr. Geier, I have some concerns; I have had a Baker's Cyst drained twice in the past year. I started having issues with the same type of pain again but now in both knees; yesterday I was told I had torn cartilage in both knees, some fluid and inflammation (more in left). I was given cortisone shot in both knees and today I am in severe pain and can barely walk or bend my left knee without being in tears. Please Help Me, is this normal?

My doctor closed during covid-19 when I was scheduled to get my second ever cortisone shot. The first one lasted eight months. I found another doctor who said no, recommended physical therapy, so I went to a third doctor. That shot lasted about 7 months and I just got another today because it wore off immediately one day. The first time it didn't hurt getting the injection and my pain was gone in knee. The second time it hurt getting the injection but last about two days. Hurts really bad from the injection I got today but I expect the pain will be gone in a couple of days. I was told I need double knee surgery in about 2 to 5 years so I'm just trying to maintain in the meantime.

Hi there, I received a cortisone shot for occasional knee pain for the first time at the end of June. I had an effusion 4 days later, 24ccs of cloudy yellow fluid. Tested negative for gout and infection. MRI shows only 'minimal chondromalacia' 2 months later, my knee is way worse and I'm in constant pain. My dr says that I must have arthritis now and to see a rheumatologist. Can cortisone shots actually make the injury worse?

I'm 19 and I ran track and field for a decade as a jumper and sprinter even making it to a college level, ive recently found out I have major arthritis in my left knee. I couldn't walk for months and still get pain when I attempt to jog or even slightly jump up and down I can barely handle my body weight. I thought I had a torn meniscus but that's not the case and my doctor recommended a cortisone shot. Should I take it or would I be more at risk? Any helpful tips on how to deal with arthritis in the knee or how to strengthen against the pain? Any answers would be appreciated, Thank you.

Hi, would love to hear from others. I’m a 54 yr old female. Ver active since middle school, track team to long distance running through out high school and collage. Later in life I took up on rock climbing and trail walks jogs in my home country in Israel. I’ve seen many doctors and they pretty much give me the same diognose, physical therapy and cortozone shot. I have a re tear in a bucket handle tear, a lateral displaced meniscus and a partial tear in my Acl. I’m in constant pain even in my sleep with most of the time swelling. I am taking over the counter intinflamitory meds but no relief in site. I’m desperate and want to get the shot just for relief but in long terms I need to know what to do. I miss my runs and jogs and wanted to compete in the Jerusalem run marathon. It’s a up 40 k run. I usually run 8-10 k a day. Any suggestions would be appreciated. Looking into medical replacement 🤷‍♀️

I had a cortisone shot last year because no one could figure out what was wrong with my knee (had x-rays, MRIs, bone scan, etc). The shot did absolutely nothing for my pain...so I got all the negative effects and zero positive. I later decided to try a diagnostic scope, which found a torn meniscus flap and a medium defect in my femoral cartilage. The surgery actually made my knee much worse - now the edge of the resected meniscus scrapes against the femur defect (stabbing pain every time I turn). I had 2 months of physical therapy right after the procedure, which worsened the pain even more. 7 months after the scope, I can barely walk and am getting pretty severe muscle atrophy. Stupid ortho doesn't know what to do.

Just had one done today. Ive had them before . My recommendation is that if your not having surgery and you are going the physical therapy route or rehab try to get as much done as you can. In the past I have had the shot and felt good so I thought I was fixed. Well it will wear off. I was right back to hurting again. After the injection take advantage of it feeling good to lose weight and strengthen your quad muscles. Then when it wears off you might be in better shape and less pain than before . IMO

Hi Dr. Geier, I recently got a cortisone shot in my knee, but there's no improvement, same pain. It concerns my left knee, more specifically the right side of my left knee. That's where the pain comes from. I told the doctor before he started the procedure and pointed my finger to the pain location, because he was about to insert the needle into the left side of my knee. All he said was: Yeah, that's the meniscus.... And then he administered the injection, without having any result at all so far. Was he right to inject on the left, or should he have listened to me and injected on the right side, as I suggested to him ?
@DrDavidGeier

As long as the injection gets into the knee, which going in at the outside (lateral) side of the knee should, then the medication will move throughout the patient's knee.

Hello Dr! Thank you for the video. I have a question, hopefully you can answer. I am 35 years old. I had a fall in January 2016, I had recurrent swelling, pain. My Ortho MD aspirated fluid several times, gave me Orthovisc injection series. It helped me for about 2 months. And everything came back again. 6 months ago I had right knee arthroscopy with partial menisectomy. I have been to PT several times. But my knee isn't just like it used to be, I know it will take more time to heal, but it still hurts for me to drive long distances, going down or up stairs, can't squat or do any lunges. What would you recommend in my case? I have bilateral knee arthritis (I was morbidly obese for years & had RNY in 2011) and my doctor says it could be the arthritis bothering me but I know how my knees were before the injury and I know it's not coming from the arthritis. Thank you!

I have had many cortisone shots in my knees, just had one on Tuesday and they work for a short time. I have also had Cinvist and Monovicst all with some relief. I also had meniscus surgery which again helped for a short time. My concern is these shots I had Tuesday were from the same Doctor I have been seeing through all of this. That night I had a horrible headache to where I had to sit in the dark with a cold pack on my forehead. I was also very dizzy and weak, I laid down and after sleeping it was almost worse and I had to stay home from work the next day. The odd part is my knees felt good. Could this be from the shots? I did call my Doctor and he did tell me to rest and ice it.
