Hyaluronic Acid, also known as Viscosupplementation (Monovisc, Orthovisc, Synvisc, Durolane) is a popular injection treatment used in arthritis and other joint disorders. In this video I break my post-HA injection instructors.
Hyaluronic acid Injections or Viscosupplementation injections are a common injection option for those with joint disorders - osteoarthritis, patellofemoral syndrome, degenerative meniscus/labral tears and more. One of the major benefits of these injections are safe and do not impact the cartilage and can last quite a bit longer than cortisone.

Thanks for the info. My doctor did not say a word about after care or anything really. He just popped in the room and the assistant handed him the shot, he injected it and then he was gone - to the next patient probably.

Yes, I did not receive any info from the provider at all. I don't think the injections are very effective either. I still can't walk unaided.

@@grandmajane2593 there are some people who do not respond. I quote on average 70% of people with mild to moderate arthritis will recieve benefit. However I am very careful to inform patients of this so they don’t think it’s 100% effective.

@@SonamMD = My doctor gave me the first set of shots with great enthusiasm and critised another doctor for giving me a Prolia shot that didn't show improvement. Then when these shots didn't do anything he said "the gel shots will work!" So I had the gel shots. As he left the room he said ' it takes a little while for the shots to become effective". Its been over a month now and I admit there is a tiny bit of improvement. I can stand for the length of time it takes to brush my teeth. Before the shots I had to sit while brushing my teeth. So for the cost of the shots, I don't think they were worth it to me. I pictured myself walking unaided. I've learned a person has to really educate themselves before committing to a medical procedure that makes unreasonable promises. Thank you for your response. You have been very informative.

I got a single H. Acid injection. It resolved my osteoarthritis pain for almost 18 months. I’m going in for my next injection next month. I was never given any instructions for after.

What a great video! Thank you. I got my first HA shots yesterday. My doctor first shot lidocaine into my knees, then the HA. It was less painful or uncomfortable than the cortisone shots I’d been getting for years. After the shot my knees felt full or swollen, though there was no swelling. My knees weren’t painful the first day. They did ache and felt uncomfortable and stiff. This morning they feel worse. Still that dull ache and stiffness but worse than the day before. I want to start walking slowly on my treadmill but my knees and this video tells me I should wait. I will.

My osteoarthritis went from moderate (w/ a very painful bone spur) to bone on bone in 9 months! Even the Dr. seemed surprised. Anyway, this was very helpful, thank you! I get my first injection Apr. 11 and then 2 more the following 2 weeks. It appeared that many just got a single shot. I'm going to trust the process (1 ea week for 3 wks) because I'm not ready for knee replacement surgery.

I’m glad you found the information helpful. Yeah there are a variety of injection options. Some people still use the three shot approach. Some people say that it may actually be better, because you are injecting the knee, once a week for three weeks and allowing the product one week to take hold before you perform another injection.

I get 5 shots of Genvisc. 1 a week for 5 weeks. Every six months. It’s about 5 years now and so far it’s kept me from surgery

I will be having my third and last injection June 3 . This is my third grouping of injections . My knees are stiff , somewhat painful for up to 3 days post injection . I have been taking Curcumin , Glucosamine and a tart cherry condensed powder that are helping . My Meniscus was tore and I had surgery in both knees .

@@kimlittle2566 generally the pain is lesser on the treated knee. During the first two days, my knee felt heavier, the front part was a bit painful and warm but after four days, I was able to feel better.

Do not worry! They usually inject Lidocaine that numbs it before the Gel injection goes in and you will kick yourself for waiting so long! I have had 3 already and I will take that Gel injection anyway before I have another knee replacement!

Thank you for this video. It was easy to understand and made me feel hopeful and at ease for my injection. I’m at a moderate to severe level of arthritis in my knees and not qualified for replacement at my age. So the shots are my last hope as of now. Wow, this video was so helpful! I'm actually a little sad seeing that it is under 8 minutes, free for me to view, and I received way more information than I did at a 2 hour Dr. visit (however, an hour & 45 minutes was waiting time) that cost $45 co-pay! :)

See my provider didn't give me any post care instructions. I switched from silcone to h.a. because he very briefly explained it's natural and would last longer. But damn does it hurt! Here I am 8 hours after injection, where I immediately went to work. The injection itself hurt, as if the h.a. was pushing my knee cap to the brink of my knee. And I'm sore and angry #AF right now. I got a 16 hour work day, when I should called out of work to heal for at least 48 hrs. Be weary out there, this pain is more present and annoying than the pain I was having without it.

I got my 1st one April 11th and my 2nd one is on Monday April 22nd. I did really well with Bone on Bone. I also do Chypo and Red Light Therapy. These are all 4 your Core and your JOINTS. I have Beautiful Legs and No One Is Cuting My Knees😊 My Hip Replacement Went Very WELL😊😊

Interesting video - I received my last shot today. My second I had a lot of fluid in the knee. I was told to take it easy so I was surprised I got fluid. After my second shot I decided to rest more - so today they did tell me that I had a little of fluid but not enough to get out. I did rest a lot and only walked if I needed to. My aftercare was just too rest And take it easy. I did workout yesterday and did a whole hour on the arc trainer but in 3 rounds of 20 minutes. They did tell me that I should be feeling more of the effect in 3 weeks from now since it really starts to work 6 weeks after the first shot. I did 3 shots of Euflexxa. He did tell me I should be able to get back to running again. So I’m hoping it works

Thanks for te video! I had a cartilage repair surgery, but after 7 months I forgot all about it and was moving very heavy stuff. Since 3 months I have a lot of pain and stiffness, and I lost all my power in my leg. I think an injection is the next step but I’m not sure which one as I see so many different stories on the internet.

The internet can be a great resource for many things. But I find often things can also get lost when it comes to medical information. It’s always worth consulting with your healthcare provider first and then making the decision. At that point, you have all the information.

Thank you for this vital information. I wasn't aware of the rest for 48 hrs, and I'm getting my gel injection tomorrow and had plans to go back to work. Concerning because I work at school, requires walking and standing for long periods. Not sure if I can request the day off, not to mentioned I'm getting the injection for 3 weeks in a row.

I still work full time , it sounds like ill need to take a few days off as Im a rural mail carrier. Standing for 2-3 hours and getting in and out of the mail truck to deliver packages . Long 9 hour route. My knees are hurting everyday , I take tylenol all the time , it just takes the edge off.

I find that sometimes they can be equally as effective. Also in many patients, one may work better than the other. In these cases, it is not uncommon for me to try both.

I just had both of my knees done today and they hurt so bad a couple of hours afterwards! It feels like my arthritis on steroids! I can barely walk and have to use a walker to get around. Each step and even laying stationary is excruciatingly painful! I am hoping that this is the storm before the peace.

Pain is not typically this bad in people post injection, I would encourage you to follow up with your provider. Things like the injection technique and approach also matter. However, this requires an in person assessment for a proper exam.

I had three gel injections one a week for three weeks. After second injection I had knee pain all week when I got the third injection by my doctor the pain was extremely bad I was so bad I was crying and wrenching in pain. I stayed in the room for a while with the nurse checking on me then the doctor came back to check on me. The doctor said he had not seen anyone have that much pain with the shot. It has been 3 days since the last shot. My knee is still painful I have been taking nsaids and tylenol.

i just had the injection(HA + Steroids), not only that doesnt hurt at all but there is nothing special you need to do after the injection.....you should be able to carry doing whatever you want to do IF the pain is not above 4 from 1 to 10

Im glad you had a good experience. Instructions can vary post injection for sure. These are my personal recommendations for my patients but I always advise people to follow the advice of the person who performs their injection.

Thank you so much for this awesome informative video! Do you see better results with HA shots or plasma injections? Do they use the same agent to deliver them into your knee? I have some allergy to eggs, and therefore the liquid agent that delivers the medication can sometimes make me feel poorly. The effect on my knees, however, has been terrific. Just wondering in your experience if the plasma has at least as good results in your patients? Thank you so much for these great videos!

Interesting question. With PRP there is no agent to deliver the medication since it is only your own processed blood (plus some anti-coagulation likely sodium citrate). The HA may have some additional additives, but this will depend on the brand and it is best to check the product monograph for this information.

@SonamMd in your experience are the results equivalent to performing HA injections? I’ve read a few studies but they concentrated on combination therapy. I’m aware of PRP not being covered by insurance but some providers accept care credit. Have you noticed a difference in your practice? Also, what are the ha typically covered by insurance? Would getting cryotherapy a week after injection speed healing time?

Depends on which brand you are getting. For example there is one brand called synvisc 3 which are 3 injections of approx 2mL completed 1 week apart. You have to do the 3 injection to finish the series. However, the majority of us use brands that are single shot and should last for many months before repeating.

Advil with Ha is okay. The reason why I don’t advise Advil with PRP is because Advil is an anti-inflammatory. With PRP, we are stimulating an inflammatory reaction as that as the mechanism of action. Additionally, Advil will affect the function of platelets. Because PRP uses the platelet aspect of your blood, I want to make sure that those platelets are functioning well. For this reason, I advise, avoiding NSAIDs 5 days before and five days after the injection.

That’s a great question. The key to answering it is understanding what we are treating. In most cases I’m using HA to treat arthritis. Unfortunately, as of right now, we have no cure for arthritis outside of a joint replacement. So these injections are means to help people manage their symptoms.

not sure what i get injected, but its 1 shot a week for 3weeks, and im told not to wear my knee brace for at least 3 days, and my knee feels great for 6 months or so, these are provided by the VA.

This tends to be one of the options that we can consider. Either this or PRP. Depending on patient preference, insurance coverage, of whether or not they want to try a full natural treatment (PRP), we make the best decision for them. I find HA injections for shoulder arthritis quite effective actually.

My knees still hurt two weeks post shot 3/3. I had this injection 7 years ago and my knees have been fine. Now I'm worried. I did to a 5k 4 days after my 3rd shot. Did I mess something up. I have no s/s of infection. My knees just ache and they didn't before

Hey Dr Sonam ! i have been injected hyaluronic acid for chondromalacia patella 6 days ago. I have played golf yesterday and i had absolutely no pain. before the injection i had pain playing golf. Now during the night i woke up and i had terrible plain in my knee. what can possibly be the reason to that ?

Are you asking if a steroid injection is better in the long term? I actually encourage people to defer cortisone for as long as possible since it can have negative effects to the cartilage. Hyaluronic acid is made in the body and is in our joints naturally. These injections work to lubricate the joint and create healthy joint fluid which contains important nutrients for the cartilage. HA injections are better long term than cortisone.

I tore my meniscus three years ago. Fluid accumulated, so I had an aspiration then and again three months ago because of tightness and limited range of motion. I had a cortisone injection each time. I have no pain, but it can feel unstable at times. I never heard of a hyaluronic injection as a treatment. Would it be helpful? Thanks.

Sometimes the instability means that the meniscus can be getting stuck in the knee. In cases like that surgery may need to be considered. I perform HA injections for meniscus tears of the knee as well.

Hi, Dr. Sonam. I’m 64 years old with an arthritis knee. My previous doctor recommended cortisone shot when he diagnosed me back in 2021. I have had one shot yearly since then. I’m now living in a new state and saw another doctor who has recommended Orthovisc, 3 x 2mL syringes. Will this injection work on someone who has had cortisone shots, especially at my age?

Is hyaluronic acid part of the substance inside the synovial fluid? As i have knee pain, stiffness & knee joint effusion and knee fluid aspersion was suggested, isn't it contradicted to hyaluronic acid injection?? .if swelling is due to too much fluid because of some damages, and if HA injection is for arthritis bone on bone as a result of not enough fluid ?? Does that mean if you have swelling, you can't have HA injection?? I'm confused.

So this is a bit more complex of a question. HA does make up a part of synovial fluid. However, when arthritis is severe, it can cause a build up of joint fluid and this can cause swelling, pain, stiffness. In many cases, I will aspirate out the knee prior to putting in the HA injection. In cases with a lot of swelling I may also inject a mixture of Ha and cortisone. The amount of fluid I inject in is usually much less than what I take out. The other thing present in joint fluid is white blood cells and inflammatory markers. These are not present in the HA we put into the knee.

After diagnose stage 2 osteoarthritis at the age of 48 mostly how long does it progress to next stage? And is it definitely progress to next stage even if really take care of food, excercise ?

Its hard to so say how fast it may progress. Knee arthritis can stay stable for many years. The most important things will be to continue exercise, strengthening, and a low inflammatory diet. There is some evidence that arthritis gets worse in those with metabolic disease.

You can get the injections as many times as you like. Ideally we want it to last at least 6 months given the cost. But this injection is quite safe to the cartilage, so theoretically it can be repeated whenever. You just need to account for the risk of infection with repeated procedures.

I personally counsel patients that HA will last from 6-12 months. I haven't personally heard of a product that can last up to 10 years in the joint as of yet. I definitely will repeat these injections every 6-12 months if they are effective in my patient population. @@annydobon8106

I just had a hyaluronic acid yesterday. It was so painful that I couldn't walk. The nurse took me to my car. By the time I parked in a street. It was a struggle to my gate and to my home. There is stiffness, and I can't straighten my knee. I have a physio in 8 days. Resting at home. Back to work in 5 days. My job is a lot of walking up and down the alimak. Would there be improvements?

This is not a typical response I see. Most of my patients walk out of the room quite comfortably. They may feel a bit stiff but that’s about it.

I had my second gel shot a week and half ago. I’m having a lot of pain going to and down the steps but not while I’m walking without steps. Not sure if this is something to be concerned about or if I should just keep putting braces on to try and help until the gel shot really takes affect.

I would recommend booking a follow up for assessment. This depends on technique and your exam. I warn patients it can take up to 4 weeks for the shot to take effect.

This depends on which HA injections you received. Most are a single injection but there are some which are still a set of three injections. I generally counsel people that they will begin to see effects at around the four week mark after the last injection, the last injection could either be a single injection, or the last injection in a series of three.

I would clarify with your doctor. But I advise patients to take 48 hours off of anything after their injection and gradually get back to activities over a 5-7 day period.

I usually recommend my patients try to time their injection so they are not working the day of or after. While it can be done, it may just flare you up. I like my personal 48 hour rule. I’d always check with the doctor who’s doing your injection just to be sure.

I got the full Synvisc treatment in the left knee Tuesday due to the insurance not covering the 3 injection approach. It has been pretty sore and I’ve not walked for any distance since. I’m looking forward to good results. I’m thinking of returning to my daily walks of 4 or 5 miles Monday, does that sound reasonable? Thanks for this vid from a new follower!

Some people report weird sensations when I inject them. For example, if i inject the shoulder, some people feel a coolness down the arm (not a nerve like pain). And this can be referred. In other cases, its due to the injection technique which I cannot comment on.

I had my acl reconstruction and meniscus repair surgery done 1 year ago Till last month i Had 5° of extension lag and flexion also not achieved full Did mri found arthrofibrosis in knee or cyclops Got it removed a month ago Right now extension is 0° in surgery knee and -4° in unaffected knee 2 days back i took HA injection as told by my doctor. What benefits can i expect…? Would my flexion and extension improve ? And is such injections only given to arthritis patients or even young people like me who are in there 30’s

I usually recommend people waiting at least 48 hr and getting back to activity slowly. That’s my advice generally. However it can change depending on individual circumstances so always check with your doctor.

You can get 3-5 every 6 months. Cortisone only 3 per year. Stay away from cortisone it damages cartilage.
