Great explanation. I’ve done bicep Tenotomy surgery with rotator cuff repair. My surgeon explained for me but your way is much easier and clear to understand. Thank you so much!

Thank you for your videos. I’m a swimmer and having biceps tenodesis, capsular plication, and labrum repair next week…in Spain where I now live. It’s extremely stressful and scary to understand everything and properly prepare for the recuperation in a foreign language. Your videos have been a tremendous help.

I have one arm torn at age 45 and not repaired on advice of Orthopedic. I have the other ar torn later at 57, saw different orthopedic, and he performed tenodesis. NO COMPARISON. Tenodesis more stable, does not cramp under load, stronger supination, and FAR better cosmetics (yes, that matters). Unless you are 90 years old, DO THE TENODESIS.

Thanks doc. I did have an MRI, and my supraspinatus tendon is torn beyond repair because it is a very old injury of probably 20 years and the tendon retracted to the point where it is very short now. Again Dr. Orcutt thanks so much for your valuable time and expert opinion. God Bless you.

@@rotatorcuffexpert I’m doing way better than expected and I’m six days post op. Nerve block worked great, took the pain killers for maybe two and a half days. Used the legal cannabis with doctor awareness to help sleep. Switched to ibuprofen but now just Tylenol as needed. This really wasn’t as bad as I was imaging but I had an incredible team

I tore my dominant arm’s bicep a week ago, and have the Popeye arm appearance. I’m 52 and active in the gym, and just started getting comfortable with the idea of not having surgery. But now I’m questioning the whole matter. How long do I have before it’s too late to have a Tenodesis?

Typically it is easier to address the biceps within a month after it has been ruptured.

My bilateral amputee missing both my legs I just had my bicep tendon done along with my rotator cuff. Boy let me tell you I think my bicep hurts more than my rotator cuff. Once that block wears off you know it boy. My surgery was done by dr. Mark morrey at Mayo Clinic last Thursday morning

Hello @rotatorcuffexpert! According to my epicrisis I have a tenodesis subpectoralis with 1 sutter anchor. This was done because of supraspinatus tendon calcification combined with a Humeral head chondropathia type 3 Outerbridge, Glenoid chondropathia type 2 Outerbridge, SLAP lession type 2 Snyder. Does it mean it is the 3rd type of reattaching the biceps tendon to the lowest point of the humerus bone? Will the biceps recover fully then or I have to have lifetime restrictions using the biceps? I am 44. I appreciate your advice here.

ggochkov, I believe if I understand your description that yes, You have had a subpectoralis biceps tenodesis. The outerbridge grade refers to athritis in your shoulder and is not directly related to your biceps problem. A lot of the restrictions you may have will be dependent on what your surgeon saw and how confident they are in their repair. Good luck and let us know how you are doing in your recovery.

​@@rotatorcuffexpert thank you! They said generally no complications upon the surgery, everything looks normal by now. What I was concerned week 1 and week 2 before I started the rehab was a hematoma I had around the place of the anchor. Initially I had a drainage for 1 day post operation. The next day they removed it and one day after the hematoma appeared. They said it is from the anticoagulant and I stopped it. Hematoma have been passing away for a month. I have just finished 6th week post operation (2nd of May 2023). I have 12 kinesitherapy hours up to now started from 17th of May with a lot of progress in the movements of the shoulder. Will notify you of the progress of the rehab.

Dr. Dan Orcutt. Thank you very much for getting back to me. I really do appreciate it. I know that you are very busy. This is why I value your time and your expert opinion. Forgive me if I sound like a broken record or repeat myself. I just want to get a full understanding of my situation and what my best options are. You are my second opinion. I have been on Google and RU-vid every night this week for about 4 hours each night reading and watching videos of shoulders, doctors and patients and their experiences. Trying to figure out what to do. I will be getting my X-ray and MRI reports next week so that I can read the entire thing. I can forward the reports to you if you would like to see them. So once again, do you feel that going on 4 months is to long to get a good result with surgery about reattaching my ruptured long head bicep tendon? If I came in within a month would you advise or advise against surgery for cosmetic reasons? When you did attempt surgery for a LHBT how was the outcome? Thanks Dan Adamovic

Dr. Orcutt, I am 11 weeks post arthroscopic RCR and ACV debridement. I’m not aware that the surgeon did anything with the bicep tendon but I have a swollen bicep and it is painful. My shoulder and the bicep is so painful that I have been in tears doing some of my PT exercises. The swelling is not only in the bicep but has gone to the side of my breast on that same side as the swollen bicep. I have seen a lymphatic drainage specialist there times already with no significant decrease in the swelling. I also have nerve pain in my wrist near the thumb that goes up the forearm a little ways. The surgeon keeps telling me the pain will get better little by little but it’s only worse not better. It hurts worse now than it did 4 weeks ago. I think the pain I’m having is not normal at 11 weeks post surgery. I just had an MRI but the results are not in yet. I wish I could see another surgeon as this surgeon seems indifferent to all my pain and complications. I don’t know what to do. I’m so tired of this pain. It’s got me breaking down crying every day and nights when I can’t sleep because of the pain and worry. Any ideas?

Dr. Orcutt, I hate to be a pain but I feel so lacking in support from my surgeon. I’m concerned because of all the pain I’m having and how it’s now radiating everywhere. My PT said he really thinks the swollen bicep that is painful and the swelling that has spilled over to my breast as well as my nerve pain in my wrist and forearm is all caused by the recurrent tear of the supraspinatis tendon. But now pain is radiating up the trap muscle and wrapping around to the clavicle. That bone even hurts all the way to the center of my throat where it ends. Is this normal? Hey Cindy, So sorry that you are still in so much pain. Sometimes when the shoulder is not working well then the rest of the shoulder girdle (the muscles around the shoulder blade) begin to try to help. They then get sore as well. Definitely get checked out by your surgeon.

I am a 62 year old female and have a tenotomy scheduled as well as a spur removal and possible supraspinatus repair. Just wondering if the ‘popeye’ if it occurs, is very noticeable and also is cramping with occasional heavy lifting common?

I am a competitive bodybuilder and I ruptured the tendon on the long head of the bicep two years ago ( proximal end). I had surgery to reattach it and i often wonder which method did he use, drilling all the way through he bone and using a clip/peg to secure the sutures against the bone, or partially drill into the bone using a plug which expands at both ends. The bicep sits a little low on the humerus but other than that its fine. Now i have popped the tendon in the other arm so facing surgery again. Yes, as we get older these things happen. Good educational video.

Great video... Wish I seen it before my surgery. I'm 50+ My rotator was repaired including a tanotomy. I am very active around the house and i lift weights to stay in shape. Will I now have limitations as to how strenuous I use it? Should I be concerned with how it will perform or injury in strenuous activity?? Thanks for your reply

You should not have to limit your strenuous activities. Once it heals it should be as stable as a tenodesis. You may notice a difference in the shape of your muscle and the strength may take a while to return, but the majority of the strength should return. Good luck ! I hope this helps.

Dr. Orcutt, I got the MRI results but haven’t talked to the surgeon yet but it does show I have a full thickness recurrent tear of the rotator cuff (supraspinatis tendon), and the long head bicep tendon shows degeneration ( without tear). I’m praying the other surgeon will take my case.

I had a super-pec tenodesis with my labraal tear repair. 2 weeks later an accident caused the repaired bicep to pull loose. First few days were not so painful . . . but now sometimes the pain is acute! What do you do at this point??

That is a tough one. Typically we do not go back in. I would talk to your surgeon and discuss with them what your options are. Good Luck!

Hey Dr Orcutt, thanks for the great video. (Note - english isn't my native language). I'm 11 days post shoulder arthroscopy and miniopen subpectoral lhb tenodesis. I was told that for the cuff injury, my surgeon used a steroid injection. To give a short background - i'm 22 years old and a competitive powerlifter, and around 8 months ago I felt a "pop" in my shoulder while bench pressing. After getting an MRI and conservative treatment failing, I ended up having this surgery. Would you say that after full recovery i'd be able to get back to competing in powerlifting, or would I be at a higher risk of injury while bench pressing due to the bicep tenodesis?

Zeds, When all is completely healed it is hard to say if you "fully" recover.At your age we would hope that you will be close. Good Luck in your training!

Great explanation. I'm facing a tenodesis at 50. I'm very active and weight train. I do feel pain iny shoulder but the main symptom is an innervation issue in my pec major. Rotator cuff is fine (repaired in 2016 along with ac joint arthritis). I'll ask surgeon the exact procedure.

What are your thoughts on tenodesis vs labrum repair? I had labrum repair done but I'm having other issues and biceps pain occasionally.. going back in and doc said he may do a tenodesis this time.. will I still likely be able to be high demand baseball and swimming/tennis with a tenodesis if all else goes well? Regardless of supra pec or sub pec? Thanks

Mark, a lot depends on your age and the condition of the biceps. I think having a biceps tenodesis regardless of where it is "tenodesed" it can help with pain. I am having a bicep tendonesis In a few weeks I previously had surgery in my shoulder originally was supposed to be that but the end up preparing a rotator cuff that was torn in 2 spots I've had an injection because I still have pain in my shoulder almost 2 years later from original surgery and the injection made the pain relatively go away and they're saying that I need the bicep tendencies surgery I do manual labor is it going to hold is there going to be any issues how long is the recovery timeCan I damage the repair longevity wise

Hey Matt, those are a lot of good questions and I hate to say it but "it depends". In general I will protect my patients with biceps tenodesis for 3 months. If all things go well my people are released from restrictions at that time. If it is well healed it should be ok to performed manual labor. Good luck and let us know how you are doing.

thanks a lot for the amazing videos. i went through a rotator cuff repair 2 months ago but today the ultrasound showed it went retearing of the supraspinatus. when do you usually do the second repair after the first surgery? how far from the first operation? thnx a lot

I try not to do the second at all! I typically do not decide on repeat surgery that soon. I usually wait at least 6 mo, unless there has been a significant trauma/re-injury. Also I would require an MRI to confirm the Ultrasound. Good Luck!

Can a subluxation of the biceps tendon out of it's bicipital groove without any tear or rotator cuff year can be replace on its own? Or just being replace in is groove?

Hello Doctor, thank you for sharing! I had a slap repair surgery 2 days ago, and I found out after that the doctor performed a tenotomy. He said the first 4 cm of the biceps tendon were slightly damaged. I asked him why he didn't do a tenodesis, and he said the tendon will eventually tear from the new attachment point, and he does not recommend it. He simply attached the tendon with a staple 5mm from it's original attachment point. He said he'll do a tenodesis if I want it. I don't know what to do. I'm 43, and I always need to lift heavy because of my work.

RC, I'm a little confused about what actually happened. In a SLAP repair, the biceps is repaired where it normally is, along with the superior labrum. Releasing the biceps is a tenotomy and reattaching the tendon (where ever you reattach it and with what ever you reattach it with ...screw...staple...) is a tenodesis. So it is confusing to me about what surgery was actually performed.

@@rotatorcuffexpert From what I understood, he fixed the labrum with a screw. The biceps tendon was half torn, so he cut it off completely, and left it in its groove. He "stapled" it close to its original labrum attachment. This part doesn't seem like a proper tenodesis as it's not where it's supposed to be attached, right? I have the Popeye shape. I'm really confused if I should fix this immediately with a proper tenodesis before the biceps settles in its new shape. He said the first 4 cm of the tendon are not in a very good shape. Would that affect the decision to choose tenotomy over tenodesis?

I still am not sure I completely understand, Did you have this procedure done in " the states"? If the tendon was badly diseased perhaps the surgeon did not think it would hold in the tenodesis.

@@rotatorcuffexpert No I've done it abroad. He said the first 4 cm of the tendon were slightly damaged, and have the white fuzzy stuff on the outside because of friction. When I asked why he didn't reattach it in the humerus, he said he doesn't usually do that.

Im a 21 year old who just had my 5th surgery on my right arm. UCL reconstruction (2015), 2 ulnar nerve surgeries (2016 and 2018), slap repair (2020), and I am now 9 days post op rotator cuff, biceps tenodesis, and posterior labrum repair. So the problem that I am running into is that I can’t use the abduction pillow due to the problems of ulnar nerve in my elbow. My arm is not able to stay in the sling for prolonged time due to the pain from the nerve. My DR was okay with me not using the pillow but I have also had to take my arm out of the sling at some moments. I wear it whenever I move around, or sleep. My question would be about the dangers of injuring the shoulder again and sort of predicament that I am in. Thanks.

Wow that is a lot of surgery on one arm!! The best thing to do is continue to communicate with your surgeon. It seems unlikely that being out of the sling by your side would be damaging, but that really is dependent on your surgery and your surgeon. Talk to your surgeon! Good Luckj!

What are the chances of getting a bicep deformity if you are having a bicep tenodesis below the groove? I am suppose to be having the surgery done and I am so nervous about having a deformity. What is the likelihood of having a complication??

You can have a deformity after any biceps tenodesis or tenotomy. You are more likely to have a deformity after a tenotomy. There are varying degrees of deformity, many will not be noticeable except to the patient. Good luck and let us know how you are doing in your recovery!

@@rotatorcuffexpert I had my week follow up visit today. The doctor said i had something called Buford complex in that shoulder. But everything is looking good, he didn’t have to do anything with my rotator cuff. I go tomorrow to start physical therapy, been doing the stretches at home. Started the codman workouts today. I wasn’t in a ton of pain thankfully, they gave me pain pills but I didn’t take them at all

No it is not a must. Some depends on the surgeon and some depends on the what all was wrong. It is a some what controversial in Orthopedics, whether to do a tenotomy or tenodesis. There is no right answer. Hope this helps!

Obviously talk to your surgeon. When the biceps is not stable in the bicipital groove the ligament can be repaired ( i think this is quite rare) or the biceps needs to be moved out of the groove, a suprapec or subpec tenodesis is what most surgeons that I know do. The biceps procedure will not affect the strength of the rotator cuff. As far as the biceps, the data is still not clear which procedure is truly superior. Most of us shoulder surgeons will tenodesis in younger active people because it just "feels like" the right thing to do. Both procedures are supported by the literature. Hope this helps

Just had shoulder surgery with Bicep tenodesis. My bicep looks very bulged near the elbow. My doctor stated the muscle will look like this until I start therapy to improve muscle form. Does this sound right? Will you be able to feel the upper tendon at all when you move muscle? Did they not tension the muscle correctly when attaching? Thank you

A couple of years ago I was tightening the faucet on my kitchen sink and there was a pop in my shoulder and it hurt so bad I flung myself in the floor. It hurt so bad the next day I went to the ER. They did an x-ray and said I was fine. As some time passed I noticed a knot on my arm. There wasn’t any pain then. As more time has passed I researched and found that I’d torn my bicep from the shoulder. The longer I go the more painful it’s becoming. Mainly in my shoulder. The bicep is more a feeling of aggravation. I’m 61 and active. Have I waited too long to have it repaired? What is the best way to go to eliminate the pain? I have chronic pain in my spine so any pain I can eliminate will help with my sanity. I’m also very self-conscious of the bulge in my arm if that matters. Getting rid of the pain in my shoulder is my main goal. Thank you!

So the biggest thing that I worry about after a biceps tendon tear is did you also tear your rotator cuff. The front of the rotator cuff is right next to the biceps. It may be too late to fix the biceps, but your pain may also be coming from a torn RC. I would recommend seeing an ortho and getting an MRI of your shoulder. What is the absolute longest time that you can wait to have a bicep tendon that ruptured reattached? I am scheduled for surgery for bicep tendon reattachment and it will be going on four months. Considering the time that is elapsed, can I expect good results or should I not even subject myself to surgery?

Dan usually if it is completely ruptured in the shoulder we do not try to reattach it because it will heal in its new position. However I have tried before. I would not wait longer than about a month. Thats just my opinion. Hope this helps!

age 45.. just tore my long bicep head tendon at the shoulder doing preacher curls. Seems like a freak accident cause I'm in good shape. After xrays MD doesnt believe anything else is wrong. He said just let it be, get used to the bulge, get PT and in short time I can start working out again. --I'm confused. How can I work out again if one head of my biceps isn't attached anymore? For sure I can never do curls again right? What about overhead presses?

The biceps is attached in two places (that is why it is call bi) the long head attaches in the shoulder joint and the short head in front of the shoulder (coracoid process). I always worry that the patient tore the front of the rotator cuff at the same time. I get an MRI of the shoulder of all my patients that have a proximal biceps tear to look for this. Thanks Dr. Orcutt . There is a sharp pain in my biceps sometimes when I do PT... I only had the supraspinatus repaired during the rotator cuff surgery. But don't know why my biceps started to hurt the most.

Can you still have full mobility lift weights get arm cut and lean if you get the tendoesis done? Will it prevent you from doing certain workouts?

It is hard to say. There are so many variables that come into play in the preoperative, the operative, and the postoperative time period. The idea is that we get you back to doing most normal daily tasks. Check with your surgeon, he/she can give you a better idea for you personally.

I’ve had a surgery on both shoulders and my orthopedic said that when he looked for the LH bicep in each arm they could not be found... but I have no deformities in either arm ..is that unusual?

I've had this done, and it seems my bicep top head seems to be sitting lower making me feel like it's in my inner elbow, it's uncomfortable and has caused me for over 10 years to not be able to go back to the gym. I like to lift weights to keep in shape, but because of haven't been able to do so i'm now diabetic. I want to get back into the gym to body build again. What is your recommendation?

I don't exactly know which procedure you had done, but I would recommend going to see a surgeon for an evaluation. There may be other things that can be done. Just one more opinion Dr. Orcutt. What would be the odds of a successful surgery for a long head bicep tendon being reattached if 16 weeks has passed based on your expertise and your experience as a surgeon and what you have seen? Would you even recommend surgery to a young athlete at this late stage? Maybe me being 56 and waiting 3 months now and by time surgery is performed it would be 4 months this might be the reason my surgeon strongly recommended that I leave it alone? What is your take on these questions? Thanks again for everything Dr Orcutt. I really appreciate your input, time, and helpful advice.

Dan, you are persistent! If someone theoretically came to me 4 months after a complete biceps tear (long head). I would get an MRI to make sure the cuff wasn't torn. If it was ok, I would recommend against surgery for the biceps.

Dr. Orcutt, I had rotator cuff repair surgery on 8/27/2021. I have lost 2.5 inches of muscle mass off of each arm in 4 weeks. I have read that muscles atrophy can begin within 8 hours of surgery. The info points to the release of the hormone cortisol. Can you speak to this in the forum? Apparently, pro athletes suffer muscle loss with injury and surgical remediation. Thanks.

yes the response to the hormone cortisol, due to the surgical stress your body is under after surgery is real. Also just disuse can create a quick atrophy of muscle. It never comes back as quickly as you loose it, but you can get it back. Once you recover from your surgery and are able to exercise again, you can regain that mass. Good Luck!

My Doc told me I need a rotator cuff repair, labrum repair and a tenotomy would probably be best for me as well. I am 53, but I am a maintenance man and sometimes have some rigorous situations and also overhead work ( ceiling grid and tile install, pulling wire, some wrenching on pipes etc). I am worried about losing strength and having a weird looking arm. My doc is my age and said if he needed surgery, this would probably be his choice for himself. I was wondering if that is good advice, and why is the tenotomy necessary? I broke myself from slipping on the ice, I failed to mention that !

Joe, a tenotomy is quicker and easier than a tenodesis. If it is a long complicated RCR often a doc will do the tenotomy so there is less "work" being done on the shoulder and a decreased risk of stiffness (not sure I agree). Also many surgeons don't think a tenotomy is necessary for anyone (I dont agree). Im 53 and I would want a tenodesis.

I had bicep tenodesis done 15 months ago. was doing ok for awhile but never pain free and in recent months more pain. I can't reach behind my back again. Hurts to lift arm while dressing. Im 54 but still very active and it's very defeating feeling 😞

Im a tree surgeon ie arborist ... I had a fall at work i had a 4cm tear in my superspinatus a, tear on infraspinatus tore pectoral bicep tendon and sub scaplaris tear .. and a torn lat. My consultant said it was the biggest tear he had seen in many years Because of my job they did a full monty repair on all the damaged tendons ect but i have a fair bit of pain in the ociptal groove .. I lost also half the cartilledge in my shoulder socket it shattered on impact i fell from about 20 feet and the shoulder took the brunt of the impact ... I can now at 4 months raise my arm above my head rom is about 80 percent of the pre accident range but the shoulder feels very fragile" .. im 54 now but my job is high demand im on ropes climbing pulling lifting logs ect daily... the accident put me out of work now for 4 months iim still in recovery but i have pain in my ociptal groove my teres major and minor and long head tricep my question is will this improve over the next 6 months to a year ... Whilst i know im getting older i was gym fit and active pre fall and im hoping i can go back to the gym after healing. Thank you very good video..👍

MRI, as it is being explained to me, shows I experienced a proximal LH biceps complete tendon rupture which happened about three weeks ago. No pain, no bruising, only a biceps deformity. Surgeon advised against a tenodesis but said he would perform the surgery if I wished since I am very active even at 56 years old. I am scheduled for surgery in one week. What are the chances for a return to full activity?

Mr Arrowsmith, I am always careful when people say "full-activity". Most people do well with repair of an isolated biceps tendon/tenodesis. But alot depends on how badly the biceps was torn and the quality of repair. Take a look at this video.

Saw my orthos doc today, he recommended Long Bicep tenotomy if the rotator cuff is not torn just frayed. If the rotator cuff is torn he is recommending rotator cuff repair with tenodesis. Question how far post op can people just put on a t-shirt?

Hi Cornelia! After rotator cuff repair you can put a tee shirt on within the first couple of days. Your arm will be down by your side so it will be a little bit of a challenge to do it, but with a little help you can get a tee shirt on. Good luck in your recovery. Let us know how we are doing.

I had distal clavicle resection surgery 7 weeks ago. For 2 years I’ve distal bicep pain. I finally have had an MRI yesterday. Of course I don’t know the results. My doctor isn’t sure if the pain is coming from my neck or the distal bicep. She was concerned that I might have a hard time with my shoulder after surgery and the position I would need to be in to get an MRI of my distal bicep. The shoulder did fine. I was almost crying yesterday because the distal bicep hurt so bad when I got out of the mri machine

Hi Doctor. I have a concern that I’d like to discuss with you. I had a rotator cuff arthroscopic surgery ( supraspinatus partial tear, labrum tear, repaired with a patch ) and a biceps tenodesis ( biceps tendon re-attached to the bicipital groove ) recently done 06/06/24. For some reason I feel like “tightness” in front of my shoulder, a constant discomfort/pain in that area specially during the night and when I remove my abduction pillow from my sling. My PT therapist wants me to stop using little by little the pillow attached to my sling. I cannot tolerate the sling for long periods of time without it. I tried several different methods like ice/heat, narcotics, anti-inflammatory medications, positioning etc. Nothing helps. That discomfort is always there. Is this something normal? What else can you suggest? Thanks in advance. 😊 ( I had an open biceps tenodesis, they made a small incision close to my armpit. I’m a “healthy” 36yrs old female )

@@rotatorcuffexpert thank you for your response. With PT my shoulder tightness has decreased. But now I have a new problem. I came out of the sling 06/25 finally. Now, I noticed increased swelling and pain that is not relieved by medications, movement, nor ice/hot. The pain and swelling is constant but it gets worse every night, specially when I wake up in the morning. I have swollen elbow and hand, fingers included. My fingers are so painful that I can barely move them, and i cannot keep my arm straight from the elbow swelling. I’ve been trying all recommendations, prescribed and OTC remedies, lots of ice and heat, even compression and the problem still there. Sometimes my arm feels warm to the touch. I’ve been complaining about this with my clinic and no one seems to care. They treat the problem as “minimal”. Based on your experience and knowledge, could you please provide some information to me, what can I do, is this issue normal? Please, this is driving me insane. Thanks in advance. 🙏🙏🙏 I cannot sleep at night. Also, my shoulder joint hurts. I’m taking oxycodone 5-10 mg and it won’t help.

I had a tenodesis done about a 1 1/2 years ago. It was anchored just below the groove. My long head bicep has a little deformity at the top. Is this normal? The Doctor said the anchor is still there and everything is fine. I thought it would look totally normal.

Squall, It would depend on what actually failed and why. In general my most common position for biceps tenodesis is suprapec. This is the position I do most all of my tenodesis. I have a partner that does most of his subpec. It just depends on what the surgeon feels most comfortable doing. It shouldnt matter if it is for a failed SLAP repair or no. Hope this helps.

Does the tendon actually grow back to the bone or does it just hang from the suture strings? Another question do you get phantom pains from having the tendon removed?

I have a massive three tendon cuff tear and am being told that the longhead bicep tendon is subluxed and has to be cut in order to repair the cuff. Does this have to be done as I don’t want Popeye bicep.

Im 50yr and just found out I had a proximal biceps rupture, didn't even realize although I've had shoulder/ rotator problems and pain for years... when I think about it there was a point of sharp pain then much of the entire issue and pain cleared up years ago... I'm an avid jujitsu player and rock climber and surprisingly don't see a huge difference. Other muscles will compensate and fill in some of missing volume from popeye syndrome if you put in the work..

Bob, yes If you are a well seasoned athlete much of the lost supination power can be compensated for with other muscles. And depending on your body shape the popeye may not be that noticeable.

As a person in their 30s, my surgeon says he is probably just going to do the bicep tendon whether it needs done or not. Should I refuse? My MRI Arthrogram shows healthy looking bicep tendon.

If a biceps tenotomy is performed (release of the biceps in the joint) it retracts out of the shoulder. That changes the shape of the biceps muscle so it looks different. Hope this helps Jay.

@@rotatorcuffexpert thanks I appreciate it. I've been off pain meds since day 3 they're extra careful now with everyone getting addicted. I'm definitely not one been on morphine pump when I had transverse myelitis when I was 19 I'm 40 now. But I understand their precaution. I went a day without the pain killer and I felt it wasn't bad enough to warrant a refill. Is the procedures I had a big recovery?

Myrical, I think anytime you have shoulder surgery it is painful with a signficant recovery. Give yourself time to recovery.

I understand Dan. It is difficult to find the biceps once it is healed in, and then re-tensioning it is tough too. Even after biceps tenodesis you can have some deformity of the biceps.

Howard, many surgeons will allow early range of motion after these biceps procedures. It typically takes 3 mo for the tendon to really heal in. Before the tendon is stressed the surgeon will evaluate the healing of the tenodesis or tenotomy on phyiscal exam.

This is a great video - VERY EDUCATIONAL AND INFORMATIVE Thanks for posting this! Just tore my proximal biceps tendon off today doing a sudden heavy lift of an object to get into the bed of my truck. I’m a 61 year old semi-retired optometrist but more importantly I’m also a novice bodybuilder, since 1985. I am not a typical 61-year old guy as I’m blessed with good health and a muscular physique . I love working out and this injury is potentially psychologically devastating to me. It’s not very painful but it looks horrible ( Popeye deformity) so I’m hoping it can be fixed. I need to be able to resume bodybuilding. BTW I’ve been talking low dose testosterone for about 10 years ( TRT ) and don’t know if that made my tendon more prone to tearing. Any support or advice will be welcome.

Braveheartman, thanks for the kind words! Sorry to hear about your injury. You should be able to find someone who will do a tenodesis for you. Also many times when the biceps is torn so is the anterior of the rotator cuff. It is unclear if testosterone in that dose predisposes people to tendon tears. Just seeing your post now. I’m also 61, in great shape, and two days ago I tore the long head bicep. And, it is devastating like you stated. Not the pain, but the deformity. What did you end up doing? Did you have the surgery? I have an appointment to see my orthopedic surgeon in one week.

It depends on where and how the biceps tendon is injured. If it is along the biceps tendon an open below the groove may be better. If it is with a rotator cuff tear then arthroscopic may best. It really depends on the specific details of the tear. Hope this helps.

Definitely discuss this with your surgeon. Typically a type I tear is not treated with a tenodesis, especially in young patients

Hey terri tednosynovitis is essentially tendonitis, inflammation of the tendon. Some tendons also have a layer around them (synovium) so if that is inflammed as well we get synovitis. I had surgery yesterday on my left shoulder due to a work injury and my surgeon fixed two small rotator cuff tears, did a clavicle resection, took care a couple of bone spurs and did a bicep tendonesis. My arm is still numb from the nerve block but I am doing well and go for a P.T evaluation tomorrow morning. He said this was definitely from the injury and will see me on Nov. 2nd. Thank you Dr. Typical advise to treat long head biceps tear non-operatively. How often are cases of doing surgery after years passed since the tear occurrence and why people have to do that ?

Ya Sim, Many docs perfer to not fix an isolated long head biceps tear. My only concern is to make sure that the rotator cuff wasn't torn at the same time. Make sure you get it checked out. Most people do not have surgery after years pass.

@@karnagefordayskdog7620 Hi not deformed. But I’m still experiencing tight bicep. (Addition to above surgery I had bursectomy and subscromial decompression as well at same time.). No visible difference with no surgery arm. Good luck for your surgery.
