mmmh, I find the process ok, and that may be part of the PMBOK, but I really disagree with creating a specific document for it. for me, an assumption that you're not sure of is a risk. so it will be in the risk register. (a proof of that is that the fields you mention are exactly the ones appearing in a risk register)
I agree. I am not answering the question 'should you use an assumptions log?'My preference is to use a RAID log to store Risks, Issues, Assumptions, and Dependencies. But, whilst every assumption carries the risk that it is incorrect, the fundamental action for an assumption is always the same: to test it and either confirm or reject it. A simple assumptions log would be a table with some of the following fields: - unique ID or reference number - a description of the assumption - the aspects of the project it impacts upon - the implications of error (perhaps as an impact score) - the person responsible for reviewing it - a due date for when it needs to be validated/disproved - a current status (maybe a simple set of status like (unknown, unlikely, likely, confirmed) and - the date of the last update
