Hello Doctor, I am young 34 & having the lower back pain with sciatica symptoms from back to knee on the right side from last 2+ years. Hanging on the bar & some icing is helping when having an episodes but have this nagging pain on a regular basis. I just got the MRI results : “L4-L5: The disc is unremarkable. There are mild facet joint degenerative changes. Mild bilateral neural foraminal stenosis. No spinal canal stenosis” my doctor is recommending either epidural or oral steroids to start with. Wanted to understand if this is curable without surgery and what are my options

I had MRI that showed annular disc tear, bilateral neuroforminal stenosis, displaced nerve root etc and its been 1 yr 4 mth later and im just have some diagnostic procedure done on s1 s2 s3 . For a year and 4 months my pain management doctor has been performing radio frequency ablations on the upper part of my lumbar and not addressing the actual MRI findings. Sad

Hello Sir. I am a 35y old wrestler who was injured in practice by having my neck cranked downward into my chest. I was given an MRI with results being "Mild central protrusion at C5-C6 level with mild left neural foraminal compromise at this level". This was 2 years ago and when it flares up my symptoms are stiff neck with burning shoulder blade. I took 3 months rest initially, quit wrestling which was my pride, and returned to doing medium weightlifting and cycling. I recently stopped exercising for a month and upon restarting (elliptical for 30 minutes) felt the pain return for a few days... is this a likely permanent thing? Is lifting moderate weights a very bad idea? Thank you for your time.

Greetings Mr. Jim ross, From the limited information that you have provided here in the comment. It's a clear case of disc buldge that is compressing both the thecal sac and nerve roots as well. Preferably it's better to avoid high impact sports as per the condition now. But on considering your age and fitness there's always an option to strengthen your supportive tissues of neck: muscles , ligaments etc. Suggestions: # You may start with isometric neck exercises and posture correction exercises to reduce the stress on the spine # And once you are confident about the neck stability you can start with weight lifting with lower weights upto 2.5 kgs just to get the shoulder muscles to get toned. # And always be consistent about the exercise as in your case there is a clear history of trauma and related issues. There are a multitude of factors that can affect the mobility of your neck. We may briefly categorize them into the following : 1. Muscular factors 2. Spine related factors 3. Myelopathies(involvement of nerve compression) In your case we need to have a clear discussion to rule out the factors and find the exact cause. A surgery is suggested when there is a noticable narrowing and nerve root compression causing severe pain or associated symptoms. Sir, im suffering by this issue since past 5½ years. I'm literally living in pain always. When the pain was started, i was suffered only by low back, now suffering by knee pain & feet as well.. i dont wish to take pain killers, almost all are prescribed such tablets only.. can you pls suggest any specific excercises/food that which will reduce /manage my pain..

Greetings Riyaz, The condition that you have might be because of a nerve root compression associated with a herniated disc. As you have a history of 5 and a half years, many factors would have added on to the problems in the intervertebral discs. A thorough assessment of your spine health and related structures are mandatory inorder to give you any suggestions over the same. sir, i am sending my MIR report , let me know if i require surgery because in MIR report it is writen .There is resultant narrowing of the spinal canal seen at this level causing compression of traversing nerve roots and impingement over the conus medullaris. Greetings from Proanatomy, It means the narrowing of the intervertebral foramina( the openings on either side of vertebral column) that allows the exit of spinal nerves arising from the spinal cord. You can get a clear picture about the intervertebral foramen in this video itself. And if it is narrowed it can in turn cause the compression of nerve roots that are exiting through the same. Bilateral means - the intervertebral foramina on both sides are narrowed and thus affecting both the left and right spinal nerve roots. Hope you got a clear picture of it. In simple terms it's a sign of degenerative changes setting in on the vertebral column. Where we could clearly mark out the fatty infiltrations into the vertebral bodies on assessing the MRI report. Remember that the fatty infiltrations that you can find on the vertebral bodies particularly are a clear indicator of degenerative changes of the spine.
