Short answer - I'ts just a design thing. Explanation - In Nekravol stream Hugo Martin said, that golden skull is just a cool decorative element without any lore meaning.

But later, when you hear it from game director's answer on a livestream, that this skull doesn't have anything related to the lore and they just put it here so it looked cool - all theories and mystery get debunked. Sad, but fun.

Honestly I just think it’s an Easter egg 😁. I like the theory that it could be a night sentinel that was put down mid transformation and given the honor of a gold skull as a remembrance.

I think it was going to be an asset for a bigger part of the game, something like the skull switches. However, I think it was scrapped, and they decided to just leave it in.

I think the wretch having a connection to Davoth could have some weight to it. When comparing Doom Guy's praetor suit in Doom Eternal to Davoth's suit, the praetor suit feels like a more refined version, which was what I always had the impression of when TAg 2 came out. Davoth's suit is far bulkier and he needs multiple equipment launchers, where as Doom Guy should be more nimble in combat and only needs one launcher. (It's an odd detail to include unless it were meant to imply some sort of history). Perhaps the wretch used to design Davoth's armor, but later switched sides during the revolt and used his prior experience to develop a superior suit of armor for Doom Guy. Davoth was furious when he found out the wretch was helping his enemy, hence why Davoth holds such a grudge against him. "The one who shall not be named." To my knowledge, it's never stated whether or not the wretch is still alive either. Unless Doom Guy is really that good with tools and metal work, the wretch is presumably still alive since Doom Guy had his new praetor suit in Doom Eternal. The other praetor suit from Doom 4 can be found partially disassembled in the Fortress of Doom, so they're two separate suits of armor (not one being overhauled between games), and no one else could have built it. Maybe Doom Guy paid a visit to the wretch between games via the slipgate he has in his house.

Maybe it’s the betrayers sons’ skull before he too was betrayed and became the Icon of Sin. That gold covered skull could be what is was before the transformation or perhaps that’s where the vestiges of his life is stored? Maybe the betrayers wife was part demon? Their son may not be 100% human. I dunno. It’s a long-shot.

Well here is a theory. It has a slayer mark.... its a dead slayer.. or someone like it.. or a horrible enemy that got gilded because of their greed. Who knows what kind of greed.
