Moving care from hospitals to community is easier said than done. Hospitals are doing everything they can to facilitate the discharges however patients frequently fear of being discharged too soon, don’t have resources at home to take care of their health after a major illness, don’t have social support or require rehabilitation after major illness/procedure before they can return to independence at home or at the community level. Unfortunately without some sort of social care programs it’s nearly impossible to discharge patients quick enough from the hospital without running into trouble with regulations or even worse running a risk of being sued by the patient or patient’s family. End result: discharging patients is extremely complicated and is getting harder in current economy when many cannot afford a private home health aid in order to safely return home after a hospital stay.
