“A lot of women's health stuff - there's no easy diagnosis. So very, very often women's health things end up getting put into, you have fibromyalgia, you're a hypochondriac, or something like irritable bowel syndrome.” -Dr Karen Tang
Dr Karen Tang is a gynaecologist who specialises in the treatment of pelvic pain, endometriosis and fibroids. She also does a great service educating the public on social media, raising awareness of women’s health issues.
In this episode, I ask Karen how people can figure out what’s causing their pelvic pain, what surprising habits may be causing problems, how patients can better advocate for themselves, and how she’d reimagine sex ed.
Stay tuned to find out:
- Why “stabbing butthole pain” has gone viral, and what it actually is
- Why endometriosis is so commonly misdiagnosed as IBS
- How female anatomy, especially the uterus, does NOT look like it does in the textbooks
- Why we should teach children the proper names for their reproductive organs
- Why you should get a pelvic floor PT!

The "normal" vs "not normal" problem with gynecological issues is so true. Even with pregnancy, I remember the doctor's face when I told her I'd been managing morning sickness by sleeping 14 hours a day, and that meant I could keep down almost half of what I ate and drank. Everyone said it was horrible, so I thought that was what they meant. She looked at me and spoke slowly, "You know that's not normal, right?" Nope, I did not know that. We have to talk more about reproductive health, and we need to be specific.
@rebeccabilly7466
@rebeccabilly7466 16 дней назад
I nearly died because everyone dismissed my concerns about "morning sickness" when I wasn't able to keep any food down. I was nearly in my second trimester before anyone took me seriously. By that point, I couldn't keep down any water and was having dry heaves in between. I had lost over 15 pounds and was severely dehydrated.

Exactly. I always hear "come prepared with a list". I do just that, they listen to maybe 3 things, spend about 15 minutes with me, either throw meds at me, give me an "I dunno", or order labs seemingly just to pacify me, and send me on my way. Nearly every time, I'm mid-sentence when the doc is beginning to walk out the door. I try asking questions or raise my concerns, and it's like they're inching closer and closer to the door to leave. It's so frustrating! If you (doctors) don't have time to actually listen and *try* to get down to the bottom of what's going on, then don't bother to book the appt! I feel you! ❤️‍🩹

I’ve actually seen that written in my medical records when I recently did a special information request as I was finding the way I was being treated was off. I am someone who is proactive about my health and when something isn’t right I don’t automatically go to a dr to figure it out I think of the what could it be and eliminate what seem totally wild and not logical to my presentation. I have a small list of potentials of what it could be so to help the dr in the 10min appointment. Then I get the dr asking me to say it in my own words. They are my own words 🤷🏽‍♀️🤷🏽‍♀️🤷🏽‍♀️

I started at 9, and pretty quickly it got uncomfortable, then painful. But had abuse at home, so figured they didn't care (broke my coccyx when roller skating; kept that secret). Then I finally escape, and go to the Dr. They didn't care either! Actually got mad at me😡 Mad for not remembering when my period was (because it was more like when it wasn't), got mad at me for wanting attention for pain (because of money, my Dr in my teens threw samples of birth control at me. I was scared of it, so she put me on Toredol- that saved me in highschool. After that, the marine dose of Ibuprofen and Excedrin together. Barely functioned with it). I don't know if either are THE cure, but might help? My mom had endo that was improved when she had a hysterectomy. I was told a partial is the best option in order to help with hormones. The OBGYN and I came to the conclusion that I might as well go that Dr route rather than going through a laproscopy to try to determine if I have endo. All I know, is that I feel like I just want my "guts" and uterus ripped out bc it feels like it's doing that internally anyway. Try seeing if you have more/ better options. Wishing you all the best

Some doctor's misdiagnose or don't listen to people. A family member had diarrhea for many days. The GP asked them to take a blood test in A&E. A&E made him stay in hospital. The gastrointestinal doctor made them stop their GVHD medicine when my family member said they need for GVHD of the liver. Their blood test showed abnormal liver readings (because of GVHD). The gastrointestinal doctor didn't even think to call up hematology because they had no clue. They asked my family member "what is GVHD?". Well listen to your patient and look it up. Look at their medical records and history. Imprisoned in hospital for 7 days. Misunderstanding.....

This year I got to see two doctors for my heavy periods and overall check-up. Both I payed to visit, can you guess who's price was higher?)) One was saying my uterus looks all "knotted" and I am entirely unhealthy, and I will for sure unable to get pregnant without her assistance with the organ "that bad looking". Another one was attentive and knowledgeable, but kind, she provided me with all the options for my near future and showed real care for my symptoms without judging or comments on how ugly my body parts were 😂 interestingly enough she didn't say I will need her help and wanted me to be able to be well on my own, without paying for anything extra. She also has one of a very few RU-vid channels on women's health in my mother tongue. The visit to a second one was so validating. I wish every woman would have an access to a real healthCARE provider. Thanks for the video!❤

I’d definitely second the advice to seek a second - or even third or fourth - opinion. And if you’re referred to a Specialist who doesn’t take you seriously, tell your GP. They may be able to get you referred elsewhere. (It’s also important for them to know who not to send patients to.) Happened to a friend who was referred to a (female) Gyno who refused to offer any treatment for her severe fibroids without approval from said friend’s partner - openly Ace, Aro, and happily eternally single friend - because she didn’t have children and might change her mind about wanting them. Thankfully her GP was horrified and disgusted and found her a Specialist prepared to treat her as a person, not an incubator, as quickly as possible.

Aspirin was first made in the late 1800s.Prostaglandins were discovered in the 1940s and it wasn't until the 1970s that aspirin was found to be a prostaglandin inhibitor. The endometrium produces prostaglandins which cause smooth muscles to contract. The uterus and intestines are smooth muscles. Some women are more affected than others. In my late 20s I was diagnosed with IBS and psychological reasons for my crazy bowel symptoms by a gastroenterologist after a gastroscopy and colonoscopy. My GP was outraged by the “psychological reasons” diagnosis. The female gastroenterologist did not take any gynaecological history, which included cripplingly painful periods, nausea, diarrhoea for at least ten years by that point. It took until I was 42 to be diagnosed with advanced endometriosis during other complicated gynaecological surgery (it ended up being endo, as well as known adenomyosis, a fibroid, and pelvic floor prolapse). I’m making sure my teenage daughter does not go through the same sort of thing. To be fair her medical practitioners do not consider cripplingly painful periods to be “normal” or acceptable and she had an endometriosis diagnosis at 18. This means her condition can be proactively managed, hopefully without the complications I had.

Nothing is worse than having some sort of awful pain and going to the ER for it. If you're a woman, they seem to automatically discount a lot of the pain. If you're a man, and you say you have level 6 pain, they start you on oxycodone or something of that level of strength. But if you're a woman, and you report level 6 pain, they give you a Tylenol. Maybe two. Not Tylenol 3, the ones with codeine, nay nay! But plain old acetaminophen. If you complain the pain is worse than a couple of Tylenol are good for, they start looking askance at you, and begin to consider you a drug seeker. After that point, they'll treat you like a junkie looking for a fix before they even give you Neurontin.

I found if I stretched my body weeks 2, 3 and 4 and had a treatment once a month it reduced some of the pain, as my tight body was more flexible during the movement and softening in week one, therefore I had more flexibility to allow the movements/softening. For me it really helped, it didn't take all the pain away, it took the heat out of the pain I sincerely hope this helps others too with horrendous pain. For me, stretching my glutes, quads, itb, hamstrings, calf's and upper back really helped. If I couldn't be bothered to stretch, I'd be sure to do my calf's always. Sincerely hope this helps someone as it really helped me, I had didelphys uterus and a septum so needed all the help I could get (two wombs) fortunately I had a hysterectomy, everything out, thank goodness and don't had the horrendous pain anymore, I now look back, I so should have spoken up more about my pains and issues I experienced, and the horrendous heavy flow.

The first doctor I talked to is a gyno. I told her about my symptoms: I felt like I was going to die every time my period starts, I had pain near my bladder, diarrhea, occasionally I would pass out and this had been happening since I was 12 years old. I told her I may have endometriosis she said there will be no further discussion if I am not on Birth control. I started taking the pill and it caused me to have a seizure. I also apparently have interstitial cystitis (IC) and the BC was causing the pain near my bladder to worsen. I’ve talked to several doctors and no one cared. I recently went to a pelvic floor specialist and she’s been the first person to affirm my suspicions about what’s going on with me. I felt like crying, I’m really grateful for her. I hope PT works out for me because I am really young and I can’t live the rest of my life like this.

My last one told me there is absolutely nothing wrong and she WILL NOT see me again because my MRI came back clear. my symptoms have been going on for so long, its getting worse as i get older. im 34. I absolutely LOVE that you talk about anatomy. Cause I had my gyno surgeon show me pics from my inside pre and post removing my uterus and endometriosis in the pelvic floor. And he was sitting with me for an hour or so as he was just there for emergencies during doctor strike, while I was recovering from my ureter re-implantation (as an unfortunate accident of said prior surgery). The pictures were so amazing! Even though it looks different than "normal" when your belly is inflated with gas so he can see stuff and safely endoscopically operate. But I love those pictures so much. I had so many questions and even though I had gotten a lot of knowledge before (Thank you, Mama Dr. Jones!). It still looked so unexpected XD And I'm still finding out more due to my urinary and GI struggles now. Like that there's a bunch of glans along the anatomy of the vagina that can cause trouble that I had no idea we had there. I mean I was aware that most of us do have little understanding of our anatomy until we need to have it (usually as we get older n struggle). But after diving into the topic more and seeing those photos from within me, I felt genuinely criminally under-educated on even the basics. Like .... damn it I should at least have seen photos from inside the body of all major human organs at least once in school!!

Pelvic Congestion Syndrome is quite painful and debilitating…. I could not even sit upright … I had to lay down to bring a small measure of relief. When I got diagnosed… the doctor said it was the worse she has EVER seen. After menopause… I am better. The only option for me was total hysterectomy and I was so trying to avoid that. Basically I severe varicose veins throughout pelvic floor and around my organs. Pain would radiate down my legs and abdomen and would cause me to break out in sweat .

I should make an appointment to see my gynecologist. My periods are long and extremely heavy. Even using Ultra tampons I have to change every hour for 2-3 days plus another 5-ish days of mostly heavy period (but more like changing Ultra tampons every 4-6 hours). Lots of big clots, super awful cramps that make it nearly impossible to do anything or go anywhere for the 2-3 days when it’s at its heaviest, awful painful diarrhea every period, super tired but trouble sleeping, shooting pains/spasms that make it hard to move, and migraine headaches. Basically totally miserable and life interrupted every single month. If my gynecologist doesn’t take me seriously I will find one that will. I will try to record the symptoms the next time I have a period to help communicate the problems I am experiencing.

I had very similar symptons to you. Only difference is after those initial heavy 7 days or so, I kept bleeding for a week and then some spotting before a few days of peace then my period started again. That was my normal so I never went to the doctor for it. After all, even when I went, they said it was normal. I spent years with severe pain right under my rib and severe abdominal pain. Ultrasounds showed I have some fibroids, biggest was almost 6 inches long. Did surgery and the biggest ones were actually double that size. I had fibroids right under my rib cage squeezing my organs. Keep changing doctors, I went to about 7 or 8 doctors before one even did an abdominal exam on me. I never even realised my upper abdomen pain was a gynecological issue until then.. I wish you luck.

Girl I have/had some of the symptoms you have. Fibroids and endometriosis was the cause for me. It’s hard to see endo in any tests but MRI would the better one and even on MRI sometimes you can’t see it. Good luck. Get tested soon because blood clots can be a serious sign.

Another overlooked cause is celiac disease. Had extremely painful periods since 12, among other issues. Because there's no money in celiac disease (can't prescribe pills, treatable only by abstaining from gluten), educating those affected by celiac disease is also something that isn't pursued. Celiac disease affects every single system in the body. Always had diarrhea and extreme pain for the first two days of my period. Always had to call in sick.

I would love doctors to spend more time with their patients. How do we get that to happen? ... There is a known issue of it taking years to obtain a correct diagnosis of specific conditions within women's health. Anecdotally, I ended up seeing PAs because they seemed to have more time & attention during the visit and ordered the necessary tests (that Drs didn't order) revealing my diagnosis. Unfortunately needed to get referred back to Drs for specific interventions, and yet again they are flying in and out offices.

I have deep penetrating endo and because my periods weren't exactly strong it took over 5 years to just get someone to do MRI and say they found "accidental finding" of adno. Then it was 2 years for laparoscopy from which they woke me up because it was now too extensive to then 3 years post lap still waiting for NHS to train neurologist that can operate on me. I have lost all quality of life, lost my job, all my hobbies etc. can barely walk... The gap in women's health is shameful. Especially when we're gaslight to make it our own fault and it's not that bad...

my sister has endometriosis. she had no diagnosis, until she was 27 and had to get an emergency surgery for it. my other sister and i both have suspected endometriosis, from 2+ professionals each (gyno and gp). but there's really no treatment or diagnosis, not even management happening. my sister recently got a copper IUD and this seems to have helped her symptoms. i haven't done this but i've been through several oral contraceptives to no avail. either intolerable side effects, or my periods change nothing at all. it's just so frustrating that nothing can be done until it's severe and emergent. you get referred to gynos only to hear "yeah, you probably do have endometriosis. but there's not really any way to be sure without opening you up. so i really can't say". and my sister just getting an IBS diagnosis. even though all 3 of us have had chronic UTIs since the onset of puberty, we've always had similar cycles. the list goes on. in a way, i wish i needed an emergent surgery so that it would all just be over. at least for a while

Do you know anything about the Myfembree medication Dr. Karan? I've been taking it for endometriosis for about 4 months now and plan to be on it for a year. Symptoms did not improve after laproscopy, so this is the next (and last??) step (can anything else be done if Myfembree fails me?). My surgeon diagnosed me with endometriosis, but I am a little skeptical because it did not show up in lab testing of my tissues/cellular level. He did tell me he scraped off some "inflammatory patches" though. Symptoms include heavy pelvic inflammation that radiates into the lower abdomen and my hips, painful deep penetration for sex (v or a), and continuation of odd cycles and spotting days. I started Myfembree at a weird time in my cycle, but then again I am so irregular I don't know what my real cycle is. So I took it when I got it, as per his instruction, but the pamphlet says a certain timeframe. I have been bleeding literally half time since taking it, but it is going down and he did say it should stop my period/bleeding altogether. The pain is a bit better, but the inflammation is still there. But in any case, I hope I can live pain free after this. My surgeon told me that Myfembree will kill all of the endometriosis for good, a once and for all kind of drug. I hope he's right. Interesting side effect for me though: If I take it in the morning and not at night, I get so fatigued I fall asleep face down on the bed or couch in my day clothes with the lights on a few hours later.

We actually now have a test for it though I understand it's new and thus not yet common or approved in many countries. But my surgeon recently got informed on that method on one of their conferences, so in foreseeable future it might become a standard. And yet, it seems said test won't likely help patients with certain GI problems. It appears to be the type that can tell that sth. is not alight but not why. Still a way to better know when to investigate further. That said, it's because it's freaking difficult to "scan" for something that normally appears in the body and then it can appear elsewhere and you do not know where and it can be very small. Also when are you gonna scan when ppl can have no periods for several months any have no idea where they're at in the cycle, since We CAN already "scan" for it, it's just that the official "scan" for it is laparoscopy aka surgery, which is obviously expensive. Other than that, it seems you are overestimating what screening even does in medicine. most of the time they are merely getting indicators of something, by something else getting out of wack and not even the "thing itself". 😅 Which can sometimes lead medical professionals down the wrong investigation route with rare stuff or not yet well researched stuff. Imagine you tried to find out what pipe is broken in a big building, on a moonless night without any artificial light other than a puny mini-flash light that illuminates an area about as big as your hand. And you cannot go inside. That is how "helpful" tests are for conditions that are good at blending in with regular anatomy 👀
