Insulin IQ community member, Pam asks why using MCT oil, which is not an exogenous ketone, still raises her ketone levels. Dr. Bikman explains that MCTs and shorter fats are absorbed directly into the bloodstream from the gut, bypassing the complex process longer fats undergo.
These fats go straight to the liver, which must immediately burn them due to their short carbon chains, as cells cannot store such short-chain fats. This increased fat burning in the liver results in higher ketone production, making MCTs highly ketogenic.

I make my own mayonnaise and use MCT oil to make it. Since I make my own bread and buns it gets used for sandwiches, etc.

Isn't MTC oil a component of coconut oil and would coconut oil also boost ketones.

Those would be oils saturated oils such as coconut oil and mono saturated (one bond) olive oil. Poly saturated oils (seed oils) contain lots of carbon bonds and therefore are easily broken down and inflammatory. At high temperature, olive oil unlike coconut oil, will turn into a trans fat.

But what if you (over)consume MCT and produce more ketones than what you need for fuel and/or building brain tissue at a given moment...? Are they recombined to any sort of fat and stored or are they expelled out of the body? Just curiosity.

No, they are eliminated with urine and some part via breathing. This is how the urine tests for ketones and breathe tests for alcohol work.

According to a paper titled Metabolism of ketone bodies during exercise and training, DOI:10.1113/JP273185 "The Metabolic actions of ketone bodies can alter fuel selection through attenuating (decreasing) glucose utilisation in peripheral tissues, *anti-lipolytic* effects on adipose tissue and attenuation ( a decrease) of proteolysis in skeletal muscle." Similar to how Insulin behaves. Because is is a fuel, it keeps more fuel from being used or made until it is used up or excreted. "in contrast, ketogenic diets _suppress fatty acid synthesis_ enzymes..." Ketone bodies as singaling metabolites, doi: 10.1016/j.tem.2013.09.002 IOW, being ketogenic turns down the body's fat building enzyme, so while you may not be breaking down fat, from exogenous ketones, you won't be building it. and unused ketones are not recombined as far as I've read, but i'm always looking for new studies to prove that. That's why I love following Dr. Bikman, and modern UTD studies.

@@anastasiag299 thank you very much! 🙂your answer leads me then to my last (ignorant) question: ketone stripes won’t then measure ketone levels “actively in use” by our body cells UNLESS you are over consuming MCT or for example over consuming your own body fat, driven by gluconeogrnesis… The stripes work only because of the excess ketones that you are excreting and that you can measure, no? (I am sure that there is an easy explanation and I am making a mistake, I am just curious)

Three Ketone measurements: 1. Urine Strips measure Acetoacetate (AcAc). 2. Blood Meters measure Beta Hydroxy Butyrate (BHB). 3. Breathalyzers measure Acetone. AcAc and BHB are the Ketones used/burned. Acetone is the final unusable waste product of Fat and Ketone burning. Depending on where you are on the metabolic spectrum, you may excrete AcAc as Excess Krones, breath out Acetone from burning Fat AND Ketones, and have BHB circulating in your blood to where it's needed. Measure what's important to you.

If my A1C is 5.8 (pre-diabetes) and fasting glucose is 95 mg/dL, but my fasting insulin is low at 3.7 you/ml and Triglycerides is 40 mg/dL, HDL is 64 mg/dL, Triglycerides/HDL is below 1.0 at 0.63. My Homa IR is also below 1.0 at 0.9. Do I have insulin resistance with the high A1C despite all other markers are normal?

Excellent info & question. I assume you were fasted for 12+ hours. With the elevated A1c & glucose, in the absence of carbs, your body may be using excess protein to make glucose.. Dr. Paul Mason has good videos regarding having elevated A1c levels while on a keto/carnivore diet. He talks about the importance of the lifespan of the reticulocytes and their role in the A1c number. Also, if your BMI is 25 or below and you have high LDL cholesterol, you may be a Lean Mass Hyper-Responder. All the best on this journey! :)

Is MCT Oil good if your blood sugar is very high? I eat mostly low carbs and high fat diet but my doctorcsaid its not good for me. My BP and BSugar is also high I am doing everything possible to bring it down but it won't come down at all I do intermittent fasting as well mostly 18/6 hrs per day. Sometimes 16/8 but my weight is not coming down as well. I am sitting down typing this in tears. Because my doctor said on my next visit he will put me on meds for diabetes but I have no symptoms of diabetes and I am frustrated they don't listen what you are saying you know your body they don't. So I don't want any wrong diagnosis. I have digestive issues due to not having a gallbladder also I had a complete hysterectomy without any HRT treatments. I am thing its is hormonal issuse but doc doesn't believe it. Whst else can I do. Thankyou so much in advance

It is possible you are eating too much fat, such that your body does not ever need to use your stored body fat reserves for energy. Dr Bikman says to first prioritize protein, cut carbs, then lastly fill in with fat. Prioritizing protein also helps significantly increase satiety & thereby reduce the desire to overeat. Keep tweaking. Don't give up!

I suggest you get yourself a new doctor., who will listen to you and give a second opinion. You don’t say how old you are but, I would have thought HRT would be needed after hysterectomy.

Perhaps try some longer fasts as well? I don't lose weight if I am drinking butter coffee... I drop the fat content of my foods so I burn my own body fat... just a suggestion. Good luck... My HA1C was rock bottom last time I checked. I have dropped in excess of 30 pounds in 2 years... fasting was the answer for me.

A friend of mine had his gallbladder removed and his doctor told him to strictly reduce his fat intake as much as possible since bile is required to digest fat and not having a gal bladder means bile production is minimal to none.

MCT oil is derived from coconut oil. MCT stands for Medium Chain Triglycerides which are fats.

@@MrLuba6a iirc butter has about 5 - 7% MCT, Coconut oil has about 15%... MCT oil is around 99% ... so you get MCT either way, but higher percentage means lower calories per ketones.

simply the concentration. Having coconut oil does contain c8 and c10. But the % is mild. MCT oil is concentrated and is pure 100% c8 and c10. nothing else. If your still confused just google medium chain trigs and mct oil. Its a bit complex.

I have to agree about this. My body hates it. My ketosis, fasting, weight loss goes pear shaped when I introduce mct oil. Just won't have a bar of it. Goes straight into the storage tanks that I can not utilise. Its not quite as black and white as the research suggests.

That’s incorrect. There are good oils and bad oils and our body actually needs some oil to process certain fat soluble vitamins and nutrients in the body. There are also oils in natural foods, such as oily fish, nuts etc so it’s a fallacy to say our bodies needs no oil at all.
