In this episode of The Metabolic Classroom, Dr. Ben Bikman challenges the traditional view that saturated fats are the primary cause of atherosclerotic plaques and heart disease. He asserts that while plaques, or atheromas, in coronary arteries are composed partly of fats and foam cells, the exact process of plaque formation remains speculative. Dr. Bikman emphasizes that anyone claiming to know the definitive cause of plaque formation is likely overstating their knowledge. Foam cells, which are fat-laden macrophages, play a critical role in plaque development and are consistently present at the sites of these plaques.
Dr. Bikman explains that inflammation is a significant factor in atherosclerosis, and C-reactive protein (CRP), a marker of inflammation, is a better predictor of heart disease than LDL cholesterol. He describes how macrophages engulf oxidized LDL cholesterol, turning into foam cells and secreting pro-inflammatory proteins like CRP. This process is driven by the presence of oxidized lipids, particularly those derived from omega-6 polyunsaturated fats such as linoleic acid, which are prevalent in modern diets due to the widespread use of vegetable oils.
Ben highlights several studies to support his argument. A notable study from 1979 by Brown and Goldstein showed that macrophages only consume LDL cholesterol when it is oxidized, not in its native form. Another study from 1998 found that oxidized LDL containing specific bioactive lipids, nine and 13 HODE, is particularly problematic. These oxidized lipids are derived from linoleic acid, not from saturated or monounsaturated fats. Moreover, historical dietary studies, such as the Minnesota Coronary Experiment and the Sydney Diet Heart Study, revealed that participants consuming more polyunsaturated fats had higher mortality rates than those consuming saturated fats.
To conclude, Dr. Bikman argues that the traditional belief that saturated fat causes heart disease is flawed. He points out that recent studies, including a correlational study published in the British Medical Journal, show that refined grains, not saturated fats, are more strongly linked to heart disease and overall mortality. He suggests that the real dietary culprit is the overconsumption of omega-6 polyunsaturated fats, particularly linoleic acid, found in processed foods. This shift in perspective underscores the importance of reevaluating dietary guidelines and focusing on the types of fats consumed.

In 1999 my cardiologist told me emphatically that my vegan diet CAUSED my heart attack I had in front of him during a stress test . He was the smartest cardiologist I ever knew, he stopped my early death and made me stop being a "stupid vegan" and return to eating meat and fish. He was way ahead of his time in this regard.

Medical students with Dr Bikman as a professor are extremely fortunate. He is the best at explaining things and knows more about metabolism than probably anyone on the planet. To me, he represents hope for the future of medicine.

If you look up the side effects of eating just rabbit (it has no fat!) they call it Protein poisoning. Protein poisoning is when the body takes in too much protein with not enough fat and carbohydrates for a long period of time. Other names for this are “rabbit starvation” or “mal de caribou.” These terms came about to describe only consuming very lean proteins, such as rabbit, without consuming other nutrients. We need Fat!

I also do Mike Mentzer consolation workout routine once a week to stay strong for health span, because what's the point in increasing life span if you can't enjoy it up into that point.

This is really tough for the layman because there are brave doctors willing to buck the system and admit that the fat centric model of CVD is not accurate, yet all the medical licensing organizations still cling to it. So it gives the one the feeling that you are experimenting with fringe theories by not going by the established “guidelines.”

To this very day, the great majority of American primary care doctors and cardiologists, under the spell of the AHA, STILL warn against eating too much saturated fat. Wouldn't you think that our medical professionals would keep up with the latest developments in the world of metabolic and cardiovascular disease? When Dr. ATKINS, about 50 years ago now, wrote his ground-breaking books the Sugar Industry used Congress to grill him ( I have the transcripts still!) with an eye to lifting his medical license, shameful! Another great video, thank you! A bit confused though… In a study (Gertz et al 1991), the researchers discovered that the composition of “plaques” was as follows: fibrous tissue (87 ± 8%), calcific deposits (7 ± 6%), pultaceous debris (5 ± 4%) and foam cells (1 ± 1%). So, surprisingly, foam cells only a very small component of the atherosclerosis plaque. Also no mention of Lp(a)! Are we sure LDL found in foam cells isLDL or is it also Lp(a)? Another study (Rath et al 1989) mentioned: “High serum Lp(a) also led to a significant increase of apo B In the arterial wall. This Initial study showed that Lp(a) accumulates in the arterial wall, partly in the form of lipoprotein-like particles, therefore contributing to plaque formation and coronary heart disease.” What are your thoughts about this? (references from Malcom Kendrick's book “The clot thickens”)

Ancient human diets would have included nuts, poultry,, and pork which have significant amounts of Omega-6 Linoleic acid. It may be that consumption of Omega-6 has increased and the Omega-6 found in processed foods has been oxidized through industrial processing.

Main issues seem to be elevated glucose causing damage to glycocalix. Also glycating ldl making pattern B. Small ldl can get in where u have damaged glycocalix. Also oxidising omega 6. Akso fructose aging good tissue 7x faster than glucose.

I've eliminated all processed foods from my regular diet, and feel great. But it's quite obvious that the animals today have a much higher fat content than the animals a few decades ago, which has to be a contributor to the increase in fat. Obviously frying food in oil is the worst, but what is the consequences of modern animal agriculture and higher fat ratios?

But Omega 6 and Omega 3 fats are the only essential fats which we need to consume, we cannot make them. We do not require mono or saturated fats at all in out diet, but out metabolism does require Omega 6 and Omega 3 fats. It's not the polyunsaturated fats which are the problem, it's the industrial processing of them. They become technically rancid (spoiled), and this causes metabolic damage.

Dr Malcolm Kendrick has put forward his thrombogenic hypothesis for causing atherosclerosis. It seems like you're suggesting people such as him are speaking from a false sense of authority? I'm currently of the opinion that his is the best model, is this incorrect? Or are you speaking of other "authorities"?

Dr Bikman, I have high blood sugar and BP can you please tell me why my BS is high i watch everything I eat which are low carb under 50 grams of carbs per day I do intermittent i eat no sugar never had a sweet tooth. Intermittent fasting 18/6 per day but I cannot lose the weight its only around my tummy! I am thanking that maybe its my thyroids because I have no gallbladder or uterus due to a complete hysterectomy. My doctor only wants to put me on meds! Please help I even follow that new scientists who talk about eating salads, and veggies first then proteins and carbs last❤

A deficiency of iodine and selenium can cause issues..that correlate with thyroid problems. Also carbs could be decreased lower..I do 20 or under grams per day. Until I cut out all grains, my weight loss stalled. I make cloud bread now..very simple..using eggs and cream cheese.

Just share my own experience...I have about the same or a bit lesser carbs/daily like you. I do Zone 2 running (very easy, slow running, some people or most walk by me) at least 3-4 times/week, 40 min each. I also walk after meals for 1/2 hr. Every night before bed I meditate about 10 minutes. My BP was 130+/80 now every am is around 117/68 most times. The meditation for me helps a lot for BP. Went to see my nutritionist, even though I've gained 3 lbs since I've started the new meals plan, my body mass fat is decreasing (as I've gained more muscles) and BMI is still the same 21.3. If you can't do Zone 2, please try meditation before bed.

Bacon fat is actually mostly unsaturated. It's the stearic acid and palmitic acid that are saturated. Oleic acid, the most predominant fat in pork, is unsaturated, and so is the linoleic acid.

Study your basic chemistry. Saturated fat has no double bonds for oxygen species to attack. Polyunsaturated fats such as linoleic acid (Omega-6) have many double bonds susceptible to peroxygenation. The information you've been getting makes no sense.
