Why Plasma Zinc is the Best Marker of Zinc Status

Short Answer: Plasma zinc is the single most important marker of zinc status. Urine zinc may decline faster in deficiency. Hair zinc increases with supplementation but does not decrease in deficiency. Plasma zinc is decreased by inflammation, oxidative stress, the ovulatory and luteal phases of the menstrual cycle, probably pregnancy and oral contraceptive use, and any kind of stress that leads to an adrenal output. If plasma zinc declines despite supplementation, corroborate that this reflects zinc status by showing normal hair zinc and low urine zinc. Use symptoms as the ultimate arbiter of whether zinc supplementation is helping.

Been waiting for this one. Great episode for sure. Zinc finger motifs and hormone resistance would be a great topic as so many are on hormone replacement and clinicians need to understand this mechanism imho

Yes I doubt there are many clinicians who know about the zinc finger motifs in the nuclear receptors.

Almost all labs don't make a difference between plasma and serum zinc, and even seems that they don't know a difference between two when asked about it

Get it written by the doctor on the order and tell the phlebotomist. The phlebotomist is in control of taking a plasma or serum tube.

@@chrismasterjohn Thanks. Bonus question if I may - how long to wait after diet changes to get a blood test done? Not just for zinc, but in general

Same, they just tell me they don't do. Is it a different method to measure, or they should do the same thing that with serum but in a plasma tube ? Here in Dubai, they told me nobody does plasma zinc in the entire UAE.

Excellent info ! I was just looking at the Quest test code online for ZINC ..that was after Lab Corp said plasma or serum. at least Quest says preferred is plasma. But now i know to definately talk to the person ..before they start the draw.

When I go to get my bloodwork through walkinlab (at Labcorp or Quest) I do indeed see both an RBC option or a Zinc plasma option available at both places. They do make a distinction here in the US.

I think I figured out why my zinc was low and my copper stays elevated. I get worse when I add zinc, clostridia takes off like wildfire based on my 16S rna microbiome and OAT testing. I think the zinc can feed clostridia microbes. Copper is very slow to go down even on a very low copper diet over the last year and a half. Thank you🙏 I've had high copper for almost 5 years I didn't check before that.

Any thoughts on the relatively recent reference range change LabCorp made for plasma zinc? I tested at 53 ug/dL last month and they didn't flag it as low. IIRC. the low end of the reference range used to be 56 ug/dL and now it's 44 ug/dL. I definitely feel like I'm deficient at 53 ug/dL. My goal is to get it up to around 103 ug/dL and sustain it long term.

It was insane before and now it’s more insane.

What are your thoughts about the zinc (sulfate) taste test?

Not validated, probably very confounded yet with some utility.
