A deep dive into the wilderness medical backpack I carry with me while on a wildland deployments as a medic. I have transitioned this pack to be my only medical bag that is used for SAR, SWAT, Dive Rescue, and wildland deployments. The changes I have made are a result of several missions that caused me to rethink what I was carrying and what configuration it was in.

Anesthesia in a stick...5cc syringe. 3cc Ketamine, 1 cc Midazolam, 1 cc Fentanyl. IV, IO or intraNasal. In adults, 2 cc bolus and look for nystagmus...yup, you’re there. If you need deeper sedation? Push additional 1 cc and flush with NS, then reassess. I’ve done amputations and reduced fracture-dislocations with it...I’ve never used it all unless on deployment/prolonged arrival of the medevac bird. Consider adding TXA...(oh yeah, SWATdoc/ER physician/TCCC instructor/Combat vet). Good loadout.

we are using now propofol 10 + 2 ket mls combo. any benefits of ket/mid/fen combo vs proket?

Better analgesia and it lasts a bit longer. Propofol is gone pretty quick. That being said, try both but clear it with your medical control.

@@freeholdtacticalmed I am sorry my english is very rusty and I have probably misunderstood Propofol+ketamine combo is only for anestesia without dormicum. Works 10-20% longer then standart ket effects. Good for et/sga. Unfortunately in my job use o fen is very rare.

Outstanding kit, thanks for providing such informational content. If you continue operating in the wilderness environment, I'd recommend field stripping trash out of the MREs to cut weight/size and swapping the toilet paper for wet wipes.

Definitely would recommend adding suction. Airway has to be open and clear. Suction Easy is light and effective!

Difference between the civilian/LE quikclot and the military quikclot is the Military one is made in the USA and the Civilian/LE is made overseas.

Nice pack overall but, not every good for wild land specific use. Unless the fire is in Afghanistan there is no need for that many chest seals. You need way more IV fluid and electrolyte replacement like Liquid IV. Also, probably need a way to cool someone. Heat and Rhabdo are the 2 biggest concerns on the fire line.

Sam, I was watching another video from Live Rescue that was published a few days ago about the worst car accidents. I’m almost positive that at the 11:42-11:45 time stamp that you were the Paramedic assessing the patient. After watching as many videos of yours as I have, I can tell that voice from anywhere. I thought that was pretty cool to be able to see you somewhat in action, stay safe buddy.

Interesting to see an EMS agency still carrying a low concentration versed, I most commonly now see 5mg/mL concentration for IM/IN seizure control. Why the 1mg/mL?

No Kerlix, 4x4s, 2x2s, ABD pads? I have a medical bag set up similar to this one. I used a 5.11 Rush 72. Maybe one day I will do a video on it. Mine is more set up for a grid down SHTF/WROL situation tho. It has the contents of about 4 high quality IFAKs plus tons of gauze, tape, gloves. It also has saline and I.V. kits, Field Surgery Kit, Skin Staplers, and antibiotics. The admin pouch has boo boo stuff and otc meds. Way more stuff, but too much to list. I even have a collar and back brace from where i was in a gnarly car wreck where I died a few times and lost most of my blood. I collapsed a lung, tore my aorta, broke my spine in two places, broke 8 ribs, removed spleen, shattered pelvis and dislocated hip, broke femur, knee replacement, and broke foot. That experience was ass garbage.

I am currently in the Army and planning on pursuing the PA career path. Someday should I end up getting out, woking with wildland firefighters is something that is very interesting to me. Is most medical care provided solely by paramedics? Or is there a need/requirement in the wildland medical structure for PA's. Thanks!

im wondering about a fire shelter i know that on your your chest rig video you said that your not fireline medic but have the capability to become fireline medics but surly it would still be wise to have a fire shelter especially if you have to treat a patient that might be near to the fireline

Have you ever seen or used those TacMed phantom or ultralight poleless litters? We used them a lot in my WFR course and our instructor said he uses them a lot on his SAR's. It might be cool to do a review a $40 2lb packable litter that I don't see much of anywhere else on youtube

Hey man great channel! I’m thinking about getting a RATS but my concern is hydration. Is there a way that you can possibly slip in or attach a camel back. Trying to use this for more long distance foot movements.

Honestly that is one of my gripes as well. I do not have a good way to attach hydration other then putting Nalgene bottles on either side of it with a molle pouch.

Never bad to carry Narcan into the woods if you carry Fentanyl and Morphins... never know when somebody‘s getting too stoned to properly breathe. The level of treatment this pack can deliver is extremely high - even having a proper backup airway for your RSIs. Only thing I might thing about adding is - as you said - at least a colorimetric etCO2 detector (as long as you don‘t have the EMMA) and I would think about some solution for splinting a pelvis would be really great! As you mentioned, some fractures can really make you bleed like sh!t internally, and the pelvis is the leading structure that‘s gonna make you bleed out!

2 qestions Why do you carry 100mg/ml ketamine? seems a super high concentration especially if you are giving Ket for analgesia. could easily make a drug error. Secondly you said you currently have no ETCO2 monitoring, however you state you can do an RSI with this pack. I am not sure about the US but in the UK ETCO2 monitoring is essential when doing this procedure which is definitely justified. Whats your thoughts on this?

Even though this is from last year, if you're still using that flashlight (despite it not being bad or anything) you should look at an Olight Perun 2, it's pretty amazing and has a lot of power in a small package and the battery lasts a long time too unless you put it on full boost (lights the place up as if it was day time).. I've recently purchased one to use during night deployments or in darker places. You can also clip it onto your hat or chest so it allows you to work with both hands

Man I'm a line medic on a hotshot crew from Columbia gorge Oregon. You are carrying way to much weight. You should pair down or you'll be the one getting treat. Hydration and blister Care are main priority. Carry a box of jello mix for glucose to save weight. Extra socks for crew and extra boot laces. I carry a kings with mini bvm that's it for airway. Zofran and Imodium and your main narcs your ll use with minor pain management every once in awhile. Traction split is worthless, establish a distal pulse and go. I work with the sawyers most of the time as a wedge man and medic 2nd. Then you walk out with crew boss so your day is the longest so cut weight were you can.

If I was a line medic I would forsure cut weight but I am a line ambulance medic, meaning I am not hiking all day and am only really called in if things are really serious. Might have to hike 1-2 miles but overall we are supported by a helicopter or ambulance.

Just a thought, seams like plenty of potential for inhalation injuries on the fire line so I think an intubation and cric kit is a wise choice even for those hiking 20 miles. Supraglottic airways are nice but you may need something more. Although I don’t work on the fire lines, so you guys are the experts on this subject.

I'm curious about the practice of "drawing what you need" out of a vial as opposed to drawing it up and diluting it to a push dose or maybe drawing up the full vial in a few syringes- it seems keeping multiple partially drawn vials is not the best idea especially since you have removed one major safty feature (the cap) which offers protection from the elements as well as an additional level of checks and balances with color coding (although it can be argued people take shortcuts with cap colors and drugs like atropine and fentanyl can easily get mixed up). But I am wondering if there is a perspective about this aproach. Also it seems like diversion could also be a lot more of an issue of you dont draw it all up and then theoretically the syringe is contaminated as soon as it comes in contact with the patient.

So pre drawing narcotics is super dope if you know you are going to use them. Pre drawing does potentially have an infection risk. Probably most medics dont do it because messing with the tamper seal can be a huge no no.

@@bdogg20101 I buy often from amazon, but only from brands I recognize and only things that are priced where they should be (anything that is way cheaper than usual should be a red flag).Regular gauze and things I will also get from amazon. I even bought my med bag from there and it's a beast of a bag. But things like BVMs, OPAs, NPAs, chest seals, and whatnot I buy from chinook medical. They have a lot of great supplies, with my favorite being a coban roll that has a gauze pad built in so it's like an israeli bandage but even easier especially if someone without training grabs it out of my pack. Edit: Removed recommendation for Recon TQs

I agree with most of what you said but steer clear of Recon TQs. They have HUGE QC issues and the internal band does not run through the entire device which causes uneven pressure. The internal band also lacks the same welding as the actual CAT so it is broken more easily, making the Windlass useless. Not to mention the patent infringement, threats of lawsuits to anyone who talks bad about them, and anecdotal reports of the rivets in the metal windlass breaking. It isn’t COTCCC approved for a reason.

@@bdogg20101 Yeah, I absolutely love my bag. Have you ever used the pocket BVM? I saw a photo of a large tear in one along one of the folds so I was afraid to go with one. But as I do more research into med bags I see more and more people swear by them. I really, really don't do well with vomit so the thought of my BVM breaking and having to do mask respirations for someone is my biggest nightmare lol, even if I do have a valve.

@@PrepMedic while ya have a Coyote Pack strictly filled with mostly/some Emergency Medical Supplies I've got the LA Police Gear Original 3 Day Backpack OD (Olive Drab) Green which'll be filled with a bit more variety than that which I plan on turning in2 as much of a Urban Wilderness Everyday Preparedness Survival Pack as possible storing in the interior/inside & attaching a mish mash (mixture) of some Solution Dyed Ranger Green Style Color MOLLE Pouches 2 break up the OD (Olive Drab) Green of my Pack 4 some Urban Appearance/Look 2 it & some OD (Olive Drab) Green MOLLE Pouches on2 the exterior/outside 2 keep a bit of the Wilderness appearance/look of LA Police Gear Original 3 Day Backpack OD (Olive Drab) Green at the same time!

Hey boss, we are looking at consolidating our Narcs down to one storage option. Less moving parts on scene. What was the brand and model of the blue Narcs bag at 12:12? Also, do you have any updated rigid options for storing Narcs. We keep ours on the medic in a safe, but when we administer, we take them out and have to keep track of the case/bag. Thanks for the info!

What are the most common injuries in the backcountry? I feel like people either pack a trauma room or boo boo band aids, not much in between for individual carry

I've been a diabetic since I was 12 and ever since my first visit they've told me glucagon is sensitive to light so I shouldn't open my glucagon container unless I need it but I noticed you keep yours in a clear container. So I guess my question is do I really need to worry about light denaturing my glucagon?

It can certainly damage it especially over time. Best to store it in a cool dry and dark place unless it’s not an option.

I have a medical bag set up similar to this and I used a 5.11 Rush 72. It's a clam shell opening bag.

Thats a lot of kit to leave at the door when you stack up to go into a house and leave it unsupervised especially if it’s at your personal expense

Still no hydrogel dressings like BurnTec (even US Marines IFAK does have it) - in my opinion for wildfire kit it is a must. It does not take a lot of place. Is there any reson for not having it?

Mainly because it isn’t how we treat burns prehospital. Moist sterile dressings with non-stick capabilities. I have a small burn cravat and a large burn sheet.

@@PrepMedic I have to disagree, according to research it is usefull, especially that in wildfire scenarios where we have to take into account possible prolonged care due to limited access to patients. This may or may not be related, but I live in California and would a P100 respirator help with inhaling the smoke/ash of a wildfire? (or even a house fire?)

P100 will protect civilians from most smoke and ash particulate matter in the air, yes. No civilian respirator will offer you much meaningful protection inside a burning structure, except maybe for aiding your escape, but normally you're better off just leaving ASAP than stopping to put on a mask. The dangers of an active structure fire are carbon monoxide and other hot gasses that will a) poison you and b) burn your airway respectively. No filter-based respirator will help you with that.
