You could also go to medical school and be an ER doctor. There are fellowships in tactical medicine for ER doctors. Johns Hopkins has one. You could also do anesthesia and work in a trauma hospital. As an anesthesiologist, you'd probably get paid 6 or 7x a paramedic. I respect paramedics. They have a tough job but they don't get paid well at all. You're young and have plenty of time. Whatever you decide to do, make sure you get the good, the bad, and the ugly from someone in the career path you are considering. There is a high rate of burn out in many roles/specialties in healthcare.

Awesome video Sam! I've used my NAR bag for years and agree with you about using clear organizers inside. I switched mine out to LBT/mystery inserts so it's easy to see once the bag is opened in low light situations or if someone else is trying to find something. Sam imagine the future with all the stuff in this video being fitted to the suit .the entire suit is the medic. Think gauntlets think integrated thigh pockets. Even shoulder really just the walking medicine cabinet on you. Our modern society since the last ice age is only 12000years so everything and anything towards the wearable medic suit is useful.the other side to that is the operable vehicle that is also packed with stuff unlike a standard ambulance style it contains operational capacity and is fitted with everything for stabilization purposes. I’d ditch the weight of that and leave it in the truck where you’ll do all the advanced work. Just something to maybe consider. Still a well organized kit. I just personally would leave advanced airway and most IV stuff in the Vic. Focus on trauma if you’re out with guys, then when the shooting stops and you can actually treat your pt you have that ready waiting for you. Curious for your thoughts?

28 npa is always my go to. I always tell any students I have, that a 28 will usually fit about anybody and if your in a hurry not to fiddle with that sizing they get taught in class.

Organized and to the point. From one medic to another. I like the bag, respect your layout, and like you, I HATE that you cant see through those pouches! Im a big fan of rolls - grab out or pull open and the roll folds out with everything right there in front of the face, no digging or dumping. I agree that its a backup - main go to items staged on the body - and more importantly - on the body of the team members themselves! A big boy can have his own large size Igel in his IFAK or vest so long as we know where it is lol Im also keen on a small wounds kit in there for all the little stuff we encounter constantly on training (bandaids, ointment , moleskin, etc.), and of course everything must be waterproof (mud)and the damn thing must float if dropped overboard. (floatation material as the back padding insert). Just some ideas tossed out there. Great job!

Hey Sam, I was hoping I could get some career advice from you. I’m a new paramedic at a really progressive agency that uses non-sworn SWAT medics. They aren’t supposed to carry weapons, but I think some do “under the table” regardless. My dream has been SWAT since I was a kid, and I was planning on moving somewhere the SWAT medics are sworn, (Cypress Creek EMS, etc.) so I could actually be an operator on the team; Not just a guy who waits in the truck for the scene to be declared safe. But here’s my issue with moving. My job right now will max me out at $108,000 base salary after seven years with the SWAT and Critical Care. That is unheard of anywhere else, and it’s really tempting to make that kind of money in a low cost of living area like the Midwest. I think Cypress Creek will max at around $75,000, and that seems like a pretty common salary range for Texas. I know you moved from a sworn position in Ames to an unsworn position in Colorado. Does it bother you being unsworn as a SWAT medic? Do you still get to clear rooms and do tactical shooting drills and that kinda stuff? I’ve always wanted to be an entry team member, not a guy waiting in the truck. What would you do if you in my shoes? Thanks man.

@@squidwardtortillini8816 Hello man, at least in Venezuela, having a paramedic attached to the SWAT team is very useful as the healthcare services may not be as close as needed or with the tools required to do specific jobs. Although if the paramedic gets shot down, the other SWAT members may have to solve the problem themselves, at least some are EMT. Check out the SWAT team I met: BAE CICPC

For my church I have build 10 comprehensive trauma kits with 5 kits in two separate large backpacks for an congregation of about 60 people. I already have 1 hypothermia foil blanket in each kit with a few extra. Do you think it would be beneficial to add 2 to 4 ready heat blankets to provide additional hypothermia protection. Patients requiring EVAC may be waiting 20 minutes plus depending on the EMS protocol for entering a warm casualty collection point.

Love the videos @prepmedic! A question for you though. I an currently an EMT and I am planning on attending medic school once things open up again. I would love to keep a bag similar to yours for the sake of preparedness. Where abouts do you get some of your iv equipment? I was under the impression that mag, dipenhydramine, and even saline were controlled substances and not something that you can keep around the house.

A controlled substance and a RX only medication are two different things. I couldnt have these meds if I was operating on my own, they are technically owned by my department but because we all have our own kits and periodically respond in POVs we are permitted to stock some meds.

I am a diabetic, I've had plenty of hypo scenarios, I have never been so combative nor seen a pt so combative and hypoglycemia that SWAT would need to be called. That is absolutely ludicrous. I get pretty combative and I've awakened to PD being on scene but I cannot imagine a scenario where SWAT would ever need to be present for any diabetic I've worked on or myself

Sure. The medics attached to SWAT teams usually come one of two ways--sourced from local fire/EMS like Sam, or an officer who goes to paramedic school. There are also cool opportunities some places, like here in Arizona, the Department of Public Safety Ranger helicopters are staffed with a paramedic who is also a DPS officer so they can perform rescues as well as law enforcement functions. The opportunities are out there.

hey sam, Im trying to build a ifak for mountian biking. I honestly have no idea what i to stock it with. my first thought was the solo ifak from nar adding a sam splint, otc meds, and chest seals. But all the addition misc items got me stumped. I've got my cert in fa/cpr so that rules out any of the fun stuff. Cant stab my buddy with an ars needle for fun. jk.

First: I am no medic or have any kind of medical education. I am just trying to learn some things out of curiosity. So, one question: you say that the trauma patients don’t need fluids, but blood and that the dilution is unwanted (correct me if I understood it wrong). But do you carry blood, or have it accessible in a vehicle? Here in Germany, ambulances and medics don’t carry blood, especially not the specialized units that go out into the wild etc. AFAIK here they actually do the opposite: they give saline to avoid a volume shock in case of massive hemorrhage. Could you explain? Also, don’t take this as criticism, I am really just trying to learn :)

The use of blood products by EMS is not yet common in the United States. It is starting to gain traction, but typically it is only routinely carried by extremely "progressive" services or specialized transport teams. It's not that increasing circulating volume is bad, (we call it fluid resuscitation), it's just not the first priority for a trauma patient. Whether there is a lot of it or less of it, blood has the same oxygen carrying capacity, regardless of how much fluid is also circulating with it. So the first priority is to stop the loss of blood immediately as best you can. If you fail to do that, the patient will die no matter how much IV fluid you put in, because IV fluid does not carry any oxygen. I don't want to put words in Sam's mouth, but if you have a trauma patient whose blood pressure is low, the absolute best thing for them is blood, to replace what they've lost. But if I don't have access to blood, my first priority needs to be keeping the blood they have left inside their body no matter what. If the patient has a blood pressure that is low but still enough to keep him alive, and I stop those things to put in fluid to raise the blood pressure to closer to normal, I'm not helping in the long run. Or worse, if I can't stop all the bleeding immediately, because they have an internal injury, it just basically chases out what's left circulating into their abdomen where it can't do any good. Obviously if you cover your all your other bases you would consider it, or if someone is crashing, you do whatever you have to do to keep them alive. Keep in mind also that unless it's a mass casualty situation, if someone is hurt that bad, you're doing what you can on-scene then hauling the mail to the hospital. The only thing that can really save a person suffering from severe trauma is a surgeon.

Felix Kuhnert Absolutely everything ^ that guy said. Also, I can’t accurately say how many, but there are plenty of EMS systems in the US that still have normal saline for fluid resuscitation in their protocols. Why? Unfortunately It takes a long time to change protocols even with solid evidence.

Chris Kotterman well but assume I get the bleeding under control, the patient will still die of the shock due to the blood loss. If you then don’t carry blood or other fluids, you are kinda screwed, aren’t you?

@@nighthawk9264 Yes. EMS does carry fluid, usually normal saline, and will deliver it if necessary. Just not blood products in most places. If a patient is showing signs of hypovolemic shock, then obviously you want to do what you can to bring volume up. But hypovolemic shock is a late problem. You have to lose about 20% of your blood volume, so unless you're bleeding into your abdomen, it should be a preventable problem if you have the other tools available. But it's not unavoidable in all cases. That's why he has a bag of saline in his bag. An ambulance has plenty of fluids available. Sam is working in the time between injury and delivery to an ambulance. In that time, fluid resuscitation will likely not be a priority, so you wouldn't carry a ton of fluids with you. You don't want to stop other efforts to take the time to put in a line to deliver saline just to prevent a possible hypovolemic shock that hasn't developed yet. It's not an argument against fluid resuscitation. If you need it you need it. It's just about how much of a certain thing to carry given limited space and the difference it can make in patient outcomes during the time you're treating them.

Felix Kuhnert This varies from situation to situation. There is evidence that supports practicing permissive hypotension in trauma patients (maintaining a MAP of 40-60). This has proven benefits, but not forever. If your transport time isn’t incredibly long you could get by maintaining safe hypotension and maybe using pressors as opposed to saline.

You need a physician who acts as your medical control and authorizes their use. With that you also need a pharmacy agreement.
