I had surgery about 2.5 weeks ago. Seated passive knee hang - Sitting at the edge of the bed. I have done the other exercises. Thanks for all the great tips, you're great! //Jeanette from Sweden

I think most helpful is learning to straighten knee with rubber band.

I had trouble trying to bend my knee to the point of tears during my sessions. The best advice my doctor gave me was to replace my chair with a rocking chair and gently rock whenever I was sitting. After a few weeks, the knee bending improved considerably and my replacement is 20 years old now and still functions very well. It is the no-tears method.

Had knee replacement surgery 10 days ago….. That heal slide maneuver is an absolute killer, and where you plant your foot and scooch your butt forward isn’t much better. I’m doing good for extension, but getting bend is extremely painful. I was looking at my rocking chair earlier today and thinking that might be a good aid. Thanks for confirming my beliefs. Going to try using a towel/strap on the heal slides to see if they become more manageable. Thanks for the helpful hints/pointers

My story. Operated on 6 wks ago, week one could'nt do much. on crutches .I was taking Palexia for pain, Endone and Tramadole just were'nt working. Week two.Still on crutches, Total leg swelling increased severely by mid week, had it checked out,and got a clean bill of health. Week 3, I had already stopped all narcotics and started to drive my car, I started my group physio, twice a week ,knee flexion was around 80 - 85 degrees. Week 4. Continued to drive car , I must stipulate, only when I really needed to, due to living alone, And only out of necessity. Physio twice a week and managed to ride stationary bike ,knee flexion 90- 95/100. Only using one crutch at this stage. At week 5, I had stopped using walking aids, Still doing physio twice a week, walking better unaided . I joined a gym so I can have use of leg press and stationery bike on off days to physio, Knee flexion has gone from 95 degrees to 110. I find cycling has helped a lot, along with the use of a thigh high compression stocking as well as using a tens machine to stimulate the quad and calf muscles.

@@daviddoniassr.2851 I had a partial knee replacement on my right leg two years ago and four weeks ago had my second partial replacement on my left leg. Don’t think that because it’s a partial it’s a cakewalk! It’s been a nightmare both times. The pain is absolutely horrible. I’m in physical therapy now and after four weeks I can get around the stationary bike 10 times with a lot of pain. my first partial replacement took me four months before I actually felt better. But now I have a small scar and a knee that’s 100% functioning like a new knee! Just hope I have the same result with the second one!

I had full knee replacement on November 9,2023. After 6 PT sessions I was able to get 126 degree bend on my knee. I am 63 and at 11 months post op. Never give up. you can do this.

I had TKR surgery yesterday and felt I was doing well. I managed to go up and down steps with a crutch, then did exercises. I managed a 70 degree bend but a few hours later I can hardly bend the knee and it's very painful. They say day 2 is when patients say 'what have I done?!' I've been given a cold wrap attached to a portable iced water box to bring down the swelling. I'm sure there will be ups and downs but It is worth the pain in the end. My mother had a new lease on life with her replacement when she was 80. She would march up a 1 in 6 hlll to her house, swim in the pool or sea 3 times a week and walked 3 miles a day. She's my inspiration to fight through the pain, remembering how bad my knees had become.

Wow it seems like your mother was a great inspiration! She was strong and resilient. But yes the beginning may seems tough but it does get better with time! Consistency with the exercise is key! Thats what PT is doing to you.Getting worse for you. No PT 3-4 weeks after surgery,then you'll be ok. These so called specialist are intrested in you to get worse ,so they can make money. F them. You have to recover from surgery,not f...g exercises. They f me up after my hip replacement. @ sessions and i told them to fuck off.After the second surgery ,no PT. F them.

@@jomc20 No PT after surgery for a few weeks. I dont know how people listen to these so called pt specialists. First you have to heal,you just got a surgery,than pt. Becuse of the idiots from PT, i could not bend my knee for 5 weeks. F them.

6 wks out from surgery, Stay positive!. Don't believe everything you hear about how well people are recovering or have recovered..It's an individual process.The start of my recovery was a little bit slow,When I started group physio I found I was comparing myself against other people and thinking I was severely behind.After five sessions I am ahead of them in my rehab. People saying they were walking after a day is either very rare or BS!. The takeaway is Don't be hard on yourself ,stay positive, Don't put high expectations on yourself. Remember! any movement is better than no movement. Also I found the use of a thigh high compression stocking worn when sleeping really helped bring down the swelling, along with a Tens machine for muscle stimulation. I wish you well 👍

No PT for a few weeks after surgery. No more pain,then you can do PT. For me pt made my leg worse.

I had surgery about 3 weeks ago... And I was told to do the leg contractions only and I'm having some stiffness in my knee and pain in my calf and leg... But you actually knows what you're talking about... Some of what I'm feeling and dealing with you explained... I'm definitely going to use these... Thanks a million🙏🥰🙏

I actually quit going to physical therapy because of the intense pain. I was being subjected to toward knee straightening. The therapists were pressing on my kneecap until I shouted out in extreme pain, after which the quad sets were extremely painful and brought me to tears, then experienced anxiety in anticipation of my physical therapy appointments, and have been as of today Eight days without any PT appointments t At the 5 week point TKR which is tomorrow I am watching videos, attempting to find answers and solutions. Thank you for your videos.

You need therapy or it can it make things worse down the road. Find a new therapist or do your own but please make sure you are doing them. I know someone who walked in there tippy toes permanently due to failure to get good range of motion

Ice and elevation helps a lot, Take pain meds half hour prior to physio to reduce the pain level I realise this reply is a month after the event .hope all has gone well

PS the exercise which has been the most helpful so far is the lying down passive knee roll with strap; your suggestion to use the strap to coax the knee flex just a touch further is brilliant. Thanks again.

I’ve had both knees replaced in the last 4 months. This video is full of fantastic info. Remember the 3 priorities; Elevate, Range of motion and strengthening. I didn’t elevate after my 1st TKR and it made recovery very difficult. 2nd TKR was a breeze in comparison.

Hi Dr Mike Just to give you an update on how helpful and effective your exercises truly are: my surgeon was super impressed with my knee flexion and straight leg at my 2 week post op check up, after one week of doing your exercises. I have since started the hospital physio exercises as well but find your key exercises in this video the best by far. Btw, I am finding that by doing the ankle pops at regular intervals throughout the day, my swelling is reduced, if I don’t, it gets worse. I also do the icing and resting legs above heart level, all of which you advise. Thanks again so much for sharing all your skill and knowledge in this video. It is very much appreciated.

I'm 75yrs old and replaced my artificial knee after 20yrs. I have a porch swing which is great for range of motion bending my knee. I seldom sit in my swing so i have my coffee and do my exercises. I enjoy swinging on my front porch

I am glad to be here and to see your video, Had Comformis TKR surgery 2.5 weeks ago. Was terribly sick for 1.5 weeks devoid of sleep & illness interferred with exercises I had hoped to do some days. I was stunned when you mentioned 2 week range of motion scores. Just this week my physical therapist said something close to: "I would like you to be at 120 or 121 range of motion by the end of the 2nd week but you've been ill so much of the time you have been here," My score had gone from 93 to 96 to 101. I left feeling that I had disappointed her not being at 120. So this perplexes me. Sorry if name icon comes up2x. Some sort of computer glitch.

My biggest challenge has been a little different than others. I am concerned about hardware failure. How much weight can I place on my knee? I am 5 weeks post surgery, I have good range of motion, no pain, but when I start to climb stairs, I am still stepping up with my non surgical leg in fear of damaging something. I was told it takes 18 months for the the knee to completely heal. I am in PT weekly but having a hard time overcoming the vision of hardward failure. I appreciate all of the good information and demonstrations you provide. Thank you Dr. Mike!

Thanks for stopping by to watch and support the channel! It’s normal to not fully trust your knee in the beginning but rest assured that you are able to put 100% on the knee. Progressive weight bearing activities will be critical and focusing on eccentric step downs can make a really significant difference in building strength in your hips/quads to be able to independently use the stairs. Kaiser would not release me from physical therapy until I was at 120 degrees. I am going on 9 months post op, the hardest thing was trusting the knee, always worrying about a bad step forward or back. I started trusting the knee after I stepped in a large divit at a park, didn’t fall and no pain in the knee.

After two months, my knee was still swollen and hurting so I went to see my surgeon. He told me to get out of physical therapy immediately because going there continued to inflame my knee so I stopped and sure enough two weeks later my knee was bending just fine because all the inflammation was gone

Thank you for this video. I had my external fixation around my knee down to my ankle removed after 4 months and few days. Now my leg stiff and can only bend the knee 30 degrees.This video has been very helpful and started to follow this exercise routine today. Thank you❤❤❤

When I had my knees replaced in 2019 I had the surgery five months apart… I had to do PT twice a day for twelve weeks… I guess the heel slide helped a lot on getting a 130 degree bend in my knee. I also got the zero extension so I met my goal as a 62 year old Woman. I had to work hard for it but my goal was accomplished. The hardest exercise was the side leg raise…. That was a bear to push through…

@@nancypadulsky9683 I do them daily. The groin pains are so uncomfortable. That muscle is so weak now. In PT they massage it as well as my quads and hamstrings. When the quads get stronger, the groin pain will decrease, I’ve been told. These stretches help a lot.

Hi Dr. Mike, I am a pretty active 60 year old nurse. I had a Total knee replacement on December 21, 2023. I am currently 5 weeks post- op. I’m so excited that I came upon your videos. I feel I’m making progress, and your knee bends with strap at the thigh has really helped with my knee slides. My cold knee bend is at 93 degrees, after PT I was at 99 degrees. I feel my therapist is putting negative feedback that I should be near 100-115. It’s definitely still painful during exercises, but only taking Tylenol. What are your thoughts for knee stiffness? Don’t be hard on yourself just because you can’t meet a specific milestone! Focus on the exercises daily and keep trying various exercises that you can tolerate! Hi Dr. Mike, after my TKR, I have done 20 sessions of physiotherapy, but my knee is not bending beyond 65 degree. It is very stiff and extremely painful even after 3 months post op. My surgeon is recommending McKee knee braces, my physiotherapist recommending Dyna Splint? I don't want any óf this, what do you think, can I improve my bend without these brace and dyna splint?

@@JKHen22 Hi…I’m currently 12 weeks post surgery. Still going to PT twice a week. My knee bend is between 109-117* before and after therapy. Therapist is concentrating on strengthening my quads with knee press machines, etc. My only concern, is pain continues while walking. I still need Tylenol/Motrin about every 6 hrs. I need to get back to nursing, but know way am I ready for 12 hour shifts! Ugh…

I’m 4 weeks and 1 day after RTKR and as Dr Mike says the heel slide exercise is totally the worst for me. Thanks for the alternatives Dr Mike. As another person commented about a rocking chair helping I bought a secondhand one and doing that and as much as I possibly can 🙏🙋🏻‍♀️❤️👍🇬🇧

Im 41 and about 7 weeks post tkr. I wish i had found your video sooner 😢 some of these are really great tips that no one mentioned and would have helped so much. My strength is great in the leg but my range of motion isnt what it should be and still having too much pain.

The chair one truly works well for me. My operated knee was "stuck" + the sitting on chair, sliding body forward, getting knee to bend more, gave great release and more flexibility!!

Had TKR 2 days ago. I really appreciate your video. This makes more sense than some of the exercises in the book they gave me. For example everything is in the lying position. Sitting up for the straightening exercise makes sense.

I've had 7 knee surgeries from just a cleanup to an acl replacement to a total knee replacement to a shattered femur right above my fake knee as my last one which was the toughest one to get the movement back. I finally got the movement back with all these plus I rode my Can Am Spyder with my right foot gradually being forced backwards onto the rear passenger pedal. Nothing real hard but just gradually a little at a time. The vibration and leaning back and forth riding the bike was the best thing I ever did for all of my surgeries, it helped that I loved riding the bike but the pain was so much less than PT was.I also bought bike pedal extenders that let me leghthen and shorten the pedal radius for my bicykle that allowed my to gradually shorten the pedal radius while riding my bike as I could not bend it far enuf to ride without this, if I could send pics believe me these 2 tricks were a godsend to get my leg back, without them we could never seem to make any progress past a certain point. I've had so much PT I almost feel like I could work at a PT clinic (7 knee surgeries-2 shoulder surgeries including a rotator cuff and later a total reverse replacement on the same shoulder and a fractured elbow that required a plate and screws and finally a back surgery which was the easiest of them all.

LOL you sound like me I shattered left Tibia back in 89 Doctor put 2 plates 11 screws in there to hold it together and I was told I will be lucky to just walk again. I pushed hard and managed to get ever thing back working great it just took a lot of time. Fast forward over the years I tore up both my knees more along with other parts of my body. In 2015 the same doctor removed all my hardware since it was starting to cause me a lot of pain. then in 2021 he finely replace the knee for good. So like you I have had 5 surgeries on my left knee then 2 more on my rt knee a full rotator cuff tear with surgery and another surgery on my rt elbow. As for my total broken bone count I believe the number to be around 28.

Dr. Mike. I just came across your video. I'm trying all your suggestions. I had my TKR Dec 16. Discharged from rehab Dec 28. Started therapy sessions Jan 3. Did 4 sesions, then I developed sciatica nerve pain. Now its more challenging to do exercises. I had to reschd 2 sessions. I don’t want my knee and muscles to freeze up. I am day 6 after partial KR surgery - I very actively trained for this surgery by doing a variety of exercises to flex and strengthen my knees. My flexion is 100+ and pain reasonable without narcotics. I keep the ice machine on the knee most of the time including to sleep. These exercises are spot on- pushing just past the pain point is key to progress. I am encouraged I will be playing pickleball in a couple months 😊

My TKR was 3 weeks ago. Physical therapy is very hard but has been very useful for me, especially doing the floor hurdles. Without this exercise, I would never have been able to get in/out of my car. i finally was able to pedal a bike completely in forward direction a week ago.

I'm 5 weeks out and feel like such a failure. When I get up in the morning, it's like I never did a single excersize. I can barely bend my knee or straighten it. I am having such a hard time. Will try your excersizes. My PT is with the surgeons office, and basically, they go by a computer program. 😢 very helpful just do the excersizes for what day you are out from surgery. Will try your excersizes and hopefully they will be better.

Hang in there and stay positive! It’s not just a physical recovery but also a mental recovery! It takes a lot on your mind have to restrict activities for the first couple months and not being able to do the things that you normally want to do. But it DOES GET EASIER! Take the small wins and focus on setting mini goals! Don’t compare yourself to anybody else but your own journey! It’s your story and now here for you to tell! 72 years old. I had right TKR 3 months ago. besides therapy, I used a Continuous Passive Motion (CPM) machine; up to 2 hours at a time 3 times a day (for 3 weeks). 1st few days set it at 60, 70 , 80 degrees. keep moving up, if it hurts, back off, but by end of 2 weeks I was at 110 degrees on the machine, at 6 weeks at 123 degrees and released from PT. also used an ice circulating cuff.

These exercises are very good, but can you advice me what i can and can do when my other knee is also a problem ,( not the one operated on. ) Both had arthritus and I was born with my knee caps not central. Im trying stave off further surgery as long as possible. 61 yrs old

I had 1st total knee replacement on February 14th, 2024. 6 weeks ago yesterday. 6 week check is tomorrow, and I will be scheduling the left knee replacement. I hope next month. I did pre surgery exercises prior to the surgery faithfully. I have been going to physical therapy 3 days a week for 5 weeks now. I actually got 130° bend 5th week. I also was able to ride stationary bike 8 full minutes at week 4. 3 weeks of high pain levels, but by the 4th week, it felt so much better almost overnight. However, sleep is still difficult. Icing it definitely helps more than I thought it would. I can't wait to get left side done now. I feel way more prepared and know how to get through the process better, I hope. Lol

You are lucky! I am at five weeks and only at 81°. I push and push and it hurts like hell. I fell last week and bent my knee all the way back. Oh my God did that hurt ouch it was a self manipulation lol. I go for my check up tomorrow. Can’t wait to see what’s next.

@ImThePronounPolice oh no! Yeah, I've even had my 2nd knee replacement surgery 10 weeks after 1st. It's been 6 months now. I walk half mile every day and have zero issues. Rocking chair is great physical therapy and I actually swam at community pool all summer. Stay at it, every day all day.

This seems to be the same pattern I had the last time - other than falling and bending my knee all the way back. I was slow to get my knee bending but now it goes all the way back. they say you forget how horrific the surgery is when you schedule your second one. That happened to me. My other one was two years ago.

Dr Mike first of all I would like to say thank you so much for making this video. I'm 57 years old and I am 9 days post up right knee complete replacement. The swelling is so bad that whenever I try to do any of these exercises I can only move a little bit. My question... How long do you think the swelling is going to last? Because I feel like if I didn't have all that swelling I would be able to move better. I'm working with and in-house PT for the next two weeks and then I'm looking to go to Helen Hayes rehabilitation near my home for outpatient but I'm so fearful that the new knee is going to form scar tissue because I'm unable to really have much range of motion due to the swelling. I will try some of these exercises, thank you so much...

Swelling after surgery varies from person to person and can last between 3-6 months depending on how much activity you’re doing. It is part of the normal healing process and by only being 9 days out you will likely see improvement in the next 3-4 weeks. Hang in there and take a look at my other video I made on how to manage swelling! I go over various techniques so that many people can find which method works best for them! These exercises are very helpful, thank you. I have no problem with extension, but my biggest obstacle for flexion is intense pain, just below the crease of the knee on the outside, which I guess is the hamstring. There is a palpable knot there. Doing the heel slides is excruciating, but my physical therapist insists on continuing to do them. I feel that they’re actually causing damage and slowing my progress down. After being pretty successful with a cane, I had to go back to the walker after my second physical therapy session. Do you have any advice for dealing with this? I have already told the therapist that I’m not going to do any exercise that causes that much pain. People around me keep insisting that I have to go through that pain in order to regain my motion range. I was at 0-110 degrees when I started physical therapy, so I don’t have that far to go. I really don’t want to be subjected to this brutal therapy anymore. I did almost no exercises for those first two weeks and came out with a good range of motion. I’m tempted to just let it be and just use gentle work at home if anything, but the prevailing wisdom seems to be that that is not going to happen. my experience from past hamstring injuries, though, is that ice and rest are the keys. Not overexerting I don’t know what to do.

If I were you I would still focus on your range of motion exercises and maybe explore other stretches that aren’t so painful. There are many different exercises to help with improve knee flexion. Also I would recommend exploring some soft tissue massage if you haven’t already done so. Addressing any tightness around the fascia and tissue can make a dramatic difference with pain and your range of motion. I like these. I have 110-115 knee flex and almost 0 degree flat, if I work at it. (Still hurts tho, dang it). I never took any pain meds. I just can't see that being good. They had me doing the ankle pumps 15 minutes into the recovery room. Walking at 30 minutes with a walker, and home in two hours. My PT people must have watched your videos. But it is also very good to see them done from someone else. I need to practice the balance more but it is better than before the TKR. Just not as good as I would like yet. Swelling is almost gone but I should be elevating better. I'll keep watching these and your other videos. Great channel. Much appreciated Dr. Mike

A month after my TKS I'm doing good. Best exercises is stretching the leg as far as you can, ice sessions, massaging the knee for blood circulation. This is my second TKR and it's a grueling surgery but it's worth it in the end.

My knee was stucked at 75° after 4 months and the doctor suggested me to use the JAS-EZ , it's a brace with a ratchet the resembles a medieval torture device, it's painful and so far getting close to 90°, I do all those exercises that you showed as well, but it's going to be hard

Oh forgot…..next knee is scheduled for Nov 7th for same total replacement. Have to get my new knee well enough to help with my next knee. Your opinion please if I should push new surgery back until my new knee now is at a certain percentage recovered? Everyone’s recovery is different but I would say that it’s not abnormal for patients to have back to back surgeries like yourself. In fact some of the patients I see have both knees done at the same time! Lol pretty amazing! If everything is going well as planned I think you’ll be doing better than you expect by Nov 7th. For the first 2-3 weeks I usually recommend icing for 3-5 times per day up to 20-30 minutes at a time with a skin barrier to protect your skin. Then as time passes and you have less swelling and pain you can reduce the frequency of the ice! Thanks for watching!

Supine hip stretch! Aah 😊. 4 weeks post TKR and really feeling it in the hips the past week! THR a year ago so don't have much muscle tone to work with, but ROM better than expected 🙏

Thanks for taking the time to watch this one and I’m sorry if I wasn’t clear in this video! All of these exercises demonstrated should be performed for the first 2 weeks. You can still use some of them past the 2 week mark but you will want to eventually progress the exercises if they become too easy for you. Hope that helps!

First of all very excellent video the way you explained and exercises are really effective. It's been 3 months i had my femur fracture surgery with rod with my knee got dislocated, now my femur is ok but i can't bend my knee it's very tight and painful to do lost quad muscle too. My doubt is can i do same exercise with same intence or you suggest different?

I also use these for all my patients that have fractured their hips because they have to go through a very similar rehab process! I’m 14wks out from TRKR. Heel slides produce horrible pain in my hamstrings since 3wks after surgery. They can get me to 120 with manual manipulation. Sitting is uncomfortable to my hamstrings and hamstring curls are uncontrollable also. Been complaining to my physical therapist for the past 10 weeks.

Great video, I’ve just had meniscus tear surgery nearly three weeks ago but while he was in my knee he drilled some bone to cause bleeding. I’ve got a lot of swelling and bruising and it’s just knowing how far to push it while the knees so swollen!

Hello Dr Mike, i am post op 2 weeks of a tkr. The hardest thing i am going through is getting my thigh muscle to just work. I had PT today and it was really hard to march in place. I can extend my leg to a zero, and bend my leg backwards to 108. Lifting the leg is a challenge if im laying flat or sitting up. In a book i was giving from the home health nurse said i could rock in a swing to help with my thigh muscle. Apart of me doesnt trust the new knee but each day i can do more and im worried im going screw up the hardware or my incision will split open. Is there something i can do to help relax my thigh muscle so it doesnt feel like my bones and muscle is screaming? Thank you

Thanks for watching and most importantly for taking the time to comment! The best way that you can relax your muscles is by using ice/elevation to minimize the swelling and to keep working on your flexibility which in the long run will elongate all the tight muscles surrounding the knee. There is a high likelihood that the stiffness is causing you to feel this way. Take it one day at a time and focus on stretches at least 2-3 x per day for the first 6 weeks if you can. Hope that helps!

Thank you so much for this video. My husband is post TKR May 2023, and he still is not quite straightening to zero. His outpatient PT has ended, and he's taken a break from home exercises. But now I'd like to start him doing your sit-up technique with the rolled towel under his ankle. I'm telling him that with enough time doing that exercise once or twice daily, that should get him his last couple of points to zero. Am I right, or is it too late?

Thank you for watching and yes please share these with him! It’s definitely hard to tell when is too late. But the best way for him to restore his motion is daily mobility with the exercises! Those last few degrees to get to zero can be stubborn but he shouldn’t give up. Also take a look at the video I made for those who are past the 2 week mark!
