Ex-KKM doctor here. overworked, peanuts pay. Will be leaving to work in australia next week. Stark difference in pay and work life balance. Will be getting 8k++ AUD per month (post tax), working 4 days per week 3 days off. My suggestion to those interested in medicine? Dont. Not worth it nowadays.

In a nutshell government Servant will always suffer with poor salary if compared to private sector. Poor work life balance. I really pity those contract doctor whose salary still stuck at the basic level. FYI I'm doctor who doing passive call paying only rm130 for 32 hours working. Despite being almost as hectic as active call which pays rm220. Can you imagine that? And shift system is the best but cannot be done due to old school mentality and low number of doctor (except in emergency or anesthesia department) And I previously work at district hospital doing 10-12 of active call per month. And no appreciation by the patient always complains here and there not understanding our situation which is usual fate for district hospital. Doing work but never being appreciated instead being complained is the worst feeling.

A lot of doctors nowadays don't really teach juniors like in the traditional way where seniors pass down knowledge; have disciples etc. I think ALL doctors should be first and foremost TEACHers to their peers and the general public because of the ever growing amount of technology and equipment needed for these doctors to master. A compassionate doctor is good but one that works/strives for the best of his community health as well, leverages the knowledge he's received. I feel part of the reason medical education will always be expensive is cos the latest and greatest specialized and somewhat custom medical equipment is always being created and improved so they need to be purchased and doctors need to be trained in them so more training equipment also need to be bought repeatedly. Besides the doctor's ROI on their education costs, the public/School's ROI on the student's education also needs to be calculated to determine the bonds of students in the profession whilst providing a decent pay. I think an "evidence-of-educating-others" evaluation KPI should be a necessary indicator of a doctor that values the growth and knowledge of the profession. The more skilled and knowledgeable doctors running to the private sector for a higher pay is also a problem. A decent equitable pay to ensure security and a worry-free live so they can focus on their charges is also necessary in public health and a bond period for ALL medical education worldwide to serve the public should be necessary considering the situation in UK, Korea, US etc. the more and more specialized equipment and knowledge required to educate them also don't come cheap. but I feel the medical profession shouldn't be one that is profit driven. If all skilled doctors aim to make as much money as possible especially in furthering people's vain projects, plastic surgery beyond reconstructive, dermatology that go beyond a normal healthy skin, and more towards an aesthetic driven profession, I think we have forgotten the meaning of medicine and what doctors are supposed to help. E.g. a bond period in public service including pupillage period needs to be served whether in the educating country or worldwide between countries it should be a requirement to serve the public as doctors. More and more doctors also totally abandoning the profession to become influencers not even in the industry that I'm beginning to suspect more and more they just wanted the doctor title(prestige) to transition to other industries as a testament to the rigorous education they were able to endure. I feel medical schools should filter out in their interviews those especially with intentions not really to serve the public. It'd be a waste of school space and contribute to the faster wear and tear of the equipment and wasted use of medical consumables and resources e.g. Cadavers and destruction of specimens. Someone literally DIED for their use and there's no price that you can put to that. A wasted medical enrollment IS a WASTED medicated spot. I rather we have a doctor passionate about the improvement in general and specialised public health, rather than one just to fill the numbers and waste these priceless resources. I think people forget that doctors and trainees use priceless resources that cannot even be recorded in accounting fairly unlike in the education of other professions. The same lens cannot be used on this profession. I feel the medical profession worldwide should be heavily restricted in privatization of their expertise. Additionally the medical records of individuals in private institutions are isolated from those in public, so there could be conflicting treatments or health condition or double prescription not communicated.

Millennial here. I noticed government doctor working condition when I was a little kid when I was admitted to the government hospital. My ambition at the time is to become eye specialist doctor but knowing the doctor working condition, I change my mind to become other than doctor. Fast forward, I'm an FPGA engineer now in a MNC and has working hour flexibility + WFH. Paid handsomely too. I hope that our health care system improves. I just saw one of my friend got an offer letter for permanent position in government hospital. I guess only time will tell.

Now low pay low staff count , they need help, according to some friend I know, they(in government hospital)still use hand written form in 2024 .... Terrible! Any bid for change is met with back in my days,.... And with if you don't like just quit...

Addendum: 1.) Oncall rate is not RM 80 /hour It is RM200 total for weekdays, RM220 total for weekends or PH. Which divides into RM8.3/hr for weekdays and RM9.1/hr for weekend and PH.(imagine not even double pay on PH😂) 2.) No 2- the RM80/hr is the locum rate which are hard to come by. Maybe you wil get 4 hours every month or other month(depending on specialties) - that amounts to around RM320 every month/other month #sadtruth

When i was a medical officer serving in the rural area of Sarawak without a hospital, I had to do oncall for 6-7 months per year. No life. I got sick every month but had to continue my oncall as my clinic only has 2 Drs, and my partner was postcall.

The oncall rates for all other departments in the hospital is still RM 200 from 5pm till 8am for weekdays RM 220 from 8am till 8am for weekends which is around RM 9 per hour You can work as a part timer at any store and earn more than a DOCTOR.

Many ppl in Malaysia’s NA are doctors. They all got meth problems. They need to take meth to survive the long work hours/harsh conditions. Quite unfortunate. I’m glad I never became a doctor despite my mum pressuring me because I just don’t want that kind of life for myself.
