My current patient has muscle rigidity in her legs and knee pain. I cannot get the top leg bent to roll her at all, and can only get the ankle crossed over occasionally. (Also she's not in a hospital or twin bed which makes everything more difficult) Right now, I'm rolling her away from me from behind using the sheet and inching a support pillow under her little by little. Please tell me there's an easier way, it's uncomfortable for both of us

I'm my grandmother caregiver...She is hospice.Is it normal to be afraid to touch her.She is in pain alot.I feel so bad.

Care is always primary - if she is too painfuyl to touch, you need to speak to your hopsice nurse or medical director to adjust pain medications. Consider pre-meidcating 20 - 30 min prior to having to provide care. ALWAYS expect there will be some discomfort if a patient has been in one position for more than an hour or two - they will be stiff no matter what and you still must give the care. Once care is given if your grandmother settles down, then her pain medication is at a good level. If she isn't comfortable - in a new position - moaning, grimacing etc - she needs more meds. Absolutely - works for any patient - no worries about the diagnosis. Please try it - she will be more comfortable and you won't hurt yourself. I care for my mother who is on hospice. She is a fall risk so I’ve been using pillows on both sides tucked under the fitted sheet. It has worked well and provides comfort for her and peace of mind for me.

Yes, oral gurgling is the most upsetting to everyone and most hospice nurses don't know how to manage it... we have a couple videos. Gotta get the patient flat and almost on their belly to get t he secretions out of the throat and mouth before sitting up...

I found that care consistency was the most difficult when multiple different caregivers were sent to care for my mom!

YES, it is one of the most important questions to ask - first the hospice you choose - how often and how consistent will the primary nurse visits be? Also when hiring outside help from an agency, you MUST ask if the same team of caregivers will be sent. I had a patient last month that had... and I am NOT exaggerating... 24 different caregivers over 25 days of service. The shifts varied from 8 hours to 10, 12 and even 24 but a different caregiver mix every day! Completely unacceptable!

It's up to the medical director, the patient and the family - depends how deeply declining the patient is... some stay on a long time based on their desire. Also depends on the admitting diagnosis and the reason for the blood thinner - might be related to another diagnosis that is not contributing to the terminal condition. Good question. Depends on if vomiting or nausea - 1 Tablespoon after 30 minutes of no nausea or vomiting - wait 30 min, if still fine, may increase to 3 Tbls and again wait.
