A serious incident occurred in Chelyabinsk on September 9, 2010 during the takeoff of the Yak-42D (board RA-42385) of the IzhAvia airline.
As it was later established, shortly before this flight, the local service center was replacing the main control switch of the stabilizer located at the helm of the aircraft commander. As a result of a mistake, the switch was set upside down 180 °.
Before the flight, the stabilizer was shifted to the takeoff position by the co-pilot from his workplace, and the incorrect setting of the switch on the commander's helm was not detected. When, during takeoff, the commander tried to move the stabilizer in the "Dive" direction, this led to the opposite result - the stabilizer switched to pitching, up to the maximum value.
When the crew tried to cope with the current situation, the aircraft reached supercritical angles of attack. Pitch up reached 40 ", roll reached 86". The commander switched to control of the stabilizer from the reserve system, restored the aircraft's controllability and made a safe landing at the departure airport.
