Erectile dysfunction has many causes. Structural changes of the erectile tissue are among them. Dr. med. dr. phil. Stefan Buntrock, urologist from Göttingen, explains in this video on UroChannel, how penile corporal fibrosis affects potency. Fibrosis is the result of chronic inflammation which is part of the wound healing process. Atherosclerosis, diabetes or trauma such as prostate surgery, may trigger wound healing. Under certain circumstances, wound healing doesn't stop and goes on and on. This is a problem, because the smooth muscle of the corpora cavernosa will then become fibrotic. As a result, function is lost as the corpora will not be able to inflate properly. Venous occlusive dysfunction is one of the consequences. It is important to keep a healthy lifestyle with physical exercise, a healthy diet and absolutely no smoking. Avoid to become overweight or obese at any cost, because the metabolic syndrome is a major promotor of chronic inflammation which in turn is a risk factor for penile corporal fibrosis. Once the damage is done, there are more or less only experimental approaches to revert the process. Stem cell therapy is currently under development. Platelet rich plasma injections, the so called "P-shot" likewise is considered experimental. Since PDE-5 inhibitors such as Viagra and Cialis don't work in corporal fibrosis and neither does injection therapy, the placement of a penile prosthesis is the treatment of choice.
