MEDICAL ANIMATION TRANSCRIPT: Your surgeon will perform an aortic valve replacement if your aortic valve is not functioning properly. The heart is a muscular organ that pumps blood through the body. It consists of four chambers. The upper chambers are the right and left atria. The lower chambers are the right and left ventricles. The right and left atria connect to their respective ventricles. The right ventricle connects to the pulmonary artery, and the left ventricle to the aorta. Valves are located at these connections to ensure that blood moves in only one direction. The parts of a valve that open and close are flaps of tissue called cusps. The aortic valve is located between the left ventricle and the aorta. When the heart contracts, the aortic valve opens and blood is pumped into the aorta. When the heart relaxes, it closes to prevent blood from leaking back into the left ventricle. A normal aortic valve has three smooth cusps that open widely and close tightly. Your aortic valve may be leaky or narrowed, causing your heart to work harder and disrupt your blood flow. A leaky valve does not close all the way, causing blood to leak back into the ventricle between each heartbeat. This condition is called aortic regurgitation. A narrowed aortic valve does not open all the way, which restricts the amount of blood passing out of the ventricle into the aorta. This condition is called aortic stenosis. You may have been born with an abnormal aortic valve. For example, your aortic valve may only have two cusps instead of three, so it may not open and close properly. Your aortic valve may have become damaged during your lifetime, a condition called acquired aortic valve disease. Causes include rheumatic fever, which can be a complication of untreated strep throat; endocarditis, which is a bacterial infection of the valve; and sclerosis, or hardening of the aortic leaflets. After a period of time, a faulty aortic valve can cause the heart muscles to become enlarged and weakened. Depending on the nature of the valves defect, the walls of the left ventricle may become abnormally thick or thin, and the problem can eventually affect the right side of the heart as well. Patients with this condition, known as heart failure, may experience shortness of breath; chest pain, or angina pectoris; dizziness, lightheadedness, and/or fainting; swelling of the ankles, feet, and legs; and fatigue. An aortic valve replacement is performed to restore the valve to normal function. Before your procedure, an IV line will be started in your arm to provide fluids and medications. A catheter will be inserted into your bladder to drain urine. Aortic valve replacements are done under general anesthesia. This means you will be asleep for the duration of the operation, and a tube will be placed through your mouth and into your windpipe to help you breath during surgery. Once the anesthesia takes effect, your surgeon will begin by making an incision in the chest. The traditional incision is made in the middle of the chest overlying the breastbone, or sternum. The surgeon then separates the sternum to expose the pericardium, which is a protective membrane encircling the heart. Your surgeon will then open the pericardium to expose the heart. At this point, your heart will be connected to a heart-lung machine. Tubes are used to reroute your blood into this machine, which takes over the functions of the heart and lungs during the operation. Once the heart-lung machine has taken over, your heart will be temporarily stopped & the blood inside removed. This keeps the aortic valve still & allows your surgeon to see it. Your surgeon will open the aorta to expose the aortic valve, make an incision around the edge of the damaged valve, & remove it. Your surgeon will then carefully sew the replacement valve into place & close the aorta with stitches. At this point, blood will again be allowed to flow into your heart to check the function of the new valve. If needed, an electric shock will be given to restart the heart. Once your heart is beating on its own, it will be detached from the heart-lung machine. Generally, temporary pacemaker wires will placed on the surface of the heart to ensure the heart maintains a normal rhythm during recovery. After surgery, you'll be taken to the intensive care unit where your vital functions will be monitored. You will still have a breathing tube in your throat, which will prevent you from talking. Several tubes will be in place to drain blood & fluid from your incision, & your bladder catheter will still be draining your urine.

Viewers should know: * the heart lung bypass tubes are larger than the illustration indicates. * the sternum is not just 'separated' but sawed lengthwise. It is repaired with stainless steel wire, so there is no danger of coming apart. * yes, an electric shock is applied directly with internal paddles to restart the heart. Occasionally, when the body is being weaned off bypass the heart will spontaneously restart, but that's the exception, not the rule. * DO your recovery rehab as if your life depended on it, because it does!

@@robertf6409 --- The recovery starts as soon as the patient is awake and alert. People awaken at different rates. Some bounce back, others take more time. Look up "Post Bypass Surgery recovery" and you should get a general idea. The whole idea is to get UP as soon as possible. Some are up and walking within the first 12 hours. The postoperative risks go down the more active one is. Pain is managed, but there is discomfort. There's no way around it: when you have surgery, you're going to have some pain.

@@gouravdwari2072 An ice-cold potassium-rich fluid is sent through the heart's arteries. It's the potassium does the trick. "plegia" means paralysis, as in quadri-plagia or para-plegia of limbs, so this is cardio-plegia.

THANK YOU. I had this operation back in April 2019. From an early age I was always short of breath. I could not play running games or do physically demanding sports. And I was always having to lean against rails and walls to support standing. My problematic heart condition appeared quite suddenly when I suffered a small stroke at age 51. I recovered quickly but I noticed I was gradually suffering more headaches which grew worse and lasted longer as time went by. Eventually I was having headaches 24/7. For relief I was over using headache meds. Finally, I saw a neurologist who'd picked up evidence of my stroke from five years earlier. A condition he described as gliosis. I feel a whole lot better mentally knowing that having had the operation I have extended my life. But that comes with the knowing that at my age the rest of my body has deteriorated. On the one hand aging has limited my movements on the other my mental capacities had gone through the roof thanks to the sudden, and new to me, increase in oxygen to all my organs. Clarity has been the greatest gain.

We need to appreciate the effort of the scientists & the doctors that saved many lives without having a rest or giving up. My mom has undergone this type of surgery right after I was born more than 30 years ago & she's had medication since then & doing well, I'd thank God & every person who contributed to such noble job & discovery because they gave me a chance to live in my healthy mom's arms bringing her back from death certainty.

I had this surgery in April 2023 in Stanford by an amazing team. My family doctor noticed a change in my heart murmur and suggested I do another Echo Cardiogram. The first Cardiologist told me my Aortic Valve was stenotic and it was pretty severe. They were hoping to do TAVR but the cardiologist at Stanford brought in his surgical partner to explain why I wasn't a candidate. Open heart surgery was my best and in reality only option. I will do my first annual Echo Cardiogram one year after surgery later this month. I was "saved" by doing annual physicals and by my family doctor who recognized the difference in the sound of my heart murmur.

I just went through this surgery in July I almost didn’t make it. My heart was outta my chest for 5 days cause it was swollen from the infection i had on the valve. I was on ecmo machine basically life support all the doctors and nurses told me I was very lucky cause I was the sickest patient in the hospital. I’m only 24 years old this was the first time I had to be in a hospital ever in my life it was so unexpected. I just wanna thank dr jones from Kaiser on sunset they saved my life. I’m happy to be able to share my story after watching this video 🫀💜

My dad had this surgery 8 years ago and enjoyed it so much he’s getting it again tomorrow. He got the mechanical valve the first time and I guess he got an infection and has bacteria built up on it and it needs replaced. He’s going for the pig valve this time around so he can stop taking blood thinners and if it needs replaced they can go through the groin instead of cracking him open.

It’s tomorrow. After a 7 hour surgery he is now part pig. Should spend a few days in the icu, a few more days in a room before he gets to go home. Then 12 weeks of some chest compression thing and a pillow.

I had a bicuspid aortic valve because one cuspis was damaged. I also had and ascending aortic aneurysm. So I have a total aortic root replacement in 2018. Neat video to watch.

Hi there I know it’s an old comment, but I have the same condition,have to get a metallic valve replacement and aortic root replacement..can you tell me how was your recovery and what where the main difficulties faced post surgery?

@@mylifecliched Having my sternum split open was rough. I slept in a recliner for probably a month because I couldn't lay back and get up. I needed help sitting on the toilet and getting up from the toilet. I couldn't hold my newborn daughter. I couldn't drive or really do anything for about a month but after about 6 weeks you'll start to feel "normal". They say it takes a couple years to fully recover. It's been 4 years since my surgery and I'm just now feeling fully recovered. I had other complications though because of my autoimmune disease. You're going to do great though. Those first couple months will be hard but after that the recovery will get substantially easier.

my grandma had this 2 years ago, she's gotten one since the one she had was leaking and thats not good especially in the hot summer of 2019 so i had to drive her to the hospital for surgery on the hottest day of the summer, trust me i haven't been more afraid of her health in my life

@@johnj3577 she was 68 and the recovery went very good, she didn't have issues about the artificial valve but did need at home care because that's normal here in the netherlands so she had a bed from the hospital in her living room for about 2 months

Can this operation be done with "robots" ? Not sure if this is the correct term but I`m referring to the type of procedures where surgeons just make a few small incisions through which they insert tubes containing a camera and all the necessary tools for the operation.
