Rotation exercises for the treatment of rotator cuff problems is one of the worst exercises you can do.
It may not hurt right away, but over time getting stronger in the rotator cuff muscles gradually feeds into the root cause of rotator cuff tears.
This is NOT understood by healthcare professionals! Those that give this exercise to their patients are not intentional trying to hurt their patients, they just don't have the updated training on this. They don't understand muscle imbalances and how to correct them.
If you've got a rotator cuff problem, make sure to do the right exercises and movements that work on correcting the root problem.
In this video, Dr. David gives you 3 reasons to avoid this one rotator cuff exercise.

This man is a Saint. For nearly two years, I've been suffering pain in my left rotator cuff. I watched one of his videos in which he explains 3 simple exercises and introduces the benefits of hanging yourself, and in only two weeks I can feel a great improvement. Now I am free of the chronical pain I had, and only suffer a bit when puting the sholder in an extreme position. Still working looking forward for a full recovery. Thank you very much for your help!

Aren't the pure axial rotation exercises you are criticizing isolating the infraspinatus and subscapularis? Strengthening these should help stability without straining the supraspinatus, I would think. Makes sense not to rely on rotation for strength as primary therapy, but in combination with shoulder blade mobility couldn't they contribute? And what about isometric rotation vs isokinetic?

I am contemplating signing up for your recovery course. I have my first surgical consultation tomorrow and we'll wait to hear from the surgeon regarding exercise. I have a full width full thickness tear as they said, two big one small tear but as still rather do exercise over the surgery

@twobraincells4364

My guess is that here in this segment he is pointing out a subtle detail that makes all the difference in terms of how one approaches and conducts physical therapy. The rotator cuff muscles...because their primary function is to provide stability.....that "stability" is best trained by exercising and insuring the existence of a fullest possible range of motion under very light or no resistance. Once a fullest range of motion is achieved under light to no resistance, I think it is important to begin to train those much larger muscles in the region responsible for many of the common and natural movements that stability is critical for. In this case....I would train all three heads of the deltoids and the traps.....particularly the lower and mid traps...of course all muscles of the body should be trained proportionate to the most efficient firing sequence to off load and distribute weight and tension in a manner promoting longevity and safety. This was an excellent differentiation lesson...and one I am in the midst of implementing for my own recovery. Good luck.

You will have to wear an arm sling/brace for about six weeks. Once the pain has decreased significantly, you can perform exercises to strengthen your traps and deltoids. This website has a list of exercises to do.

Looking forward to seeing your other videos dealing with shoulder pain. I'm 66, and a couple of years ago, i was dealing with right shoulder pain. My physician gave me a cortisone shot 14 months ago and no more pain! 8 months ago my left shoulder started giving me pain. I tried cortisone shot and this time no success. I cant sleep on my lefy side and reaching past shoulder height hurts. I'm an avid golfer and i can still play, thankfully. Im going to try your exercises and see if that will help.

"You're training the rotator cuff muscle to become predominantly a rotator muscle" -- Of course. Operate any useful weighted lever like a hammer or shovel and you should realize the rotator cuff is designed to rotate. Throw a ball, crawl on the floor, or practice martial arts. You should not fear training rotation. In fact, I'd recommend researching clubbell exercises and working towards mills and reverse mills, which are ancient exercises not covered by modern band or plug-yourself-into-a-machine exercises at the gym. If you jump the gun in any exercise when you haven't earned the right to progress (with added resistance or the next exercise in a logical sequence), then of course you can harm yourself. But the human body does not strictly move in just combinations of front to back, side to side, or up and down. We rotate. Train with resistance below the threshold you'll hurt yourself and slowly over the course of years work your way up. I've been training with clubbells for a few years now and I'm still in the lower weight ranges of what you can buy online. That's fine because I'll eventually work my way up, but I will have given my muscles, connective tissues, and nervous system ample time to adapt. Also, I recommend not segmenting workouts by muscles in like a bro split, upper lower, or push pull legs day thing. Think about it in terms of movement patterns like throwing patterns covered by clubbells, weighted carries or ballistics covered by kettlebells, safely going down to and getting up off the floor, crawling, walking, running, swimming, etc. The human body is a complete complex system so you should train it as one complete complex system.

A comment: I am a 71 year old retired carpenter and much of my work actually needed these awkward movements when working with sheet goods overhead. I'm trying to rehabilitate both rotator cuffs, especially the left, with success on the right but little on the left. I have been using the very exercises you suggest eliminating from a rehabilitation program - I'll do so and see if I have any improvement. Thanks much.

Hi Brian! We're sooo glad you found our RU-vid channel! We want to help you rehabilitate your rotator cuffs and get back to doing the things you love! Then it guides you to correct the underlying root muscle imbalances so that you can fix the source of the problem (rather than just treat the symptoms) and get long-term relief from shoulder pain. OK, so I have a 1.6cm. Mildly tendinopathic and frayed infraspinatus and subscapularis tendons and my " Orthopedic surgeon MD" gave me a cortisone steroid injection and a set of 8 exercises, two of these exercises are the Internal and External Rotations with small weights 2-5lbs with a towel under the armpit lying down on back and side. You're saying he's wrong? The other exercises he recommended are (Cane stretch, Back Scratcher, Pretzel stretches, Wall Walk, Side Raise, and Wall Pushups) What is your opinion.? I don't know if you answer questions on your Podcast.

We're so sorry to hear about your experience with your shoulder surgeries! It sounds like you're feeling understandably frustrated and confused about why the bone replacement happened without your knowledge, and we empathize with your concerns. We know that this is a challenging situation, and unfortunately, we don't have specific information about why the procedure was performed or why it wasn't documented in your records. It's concerning that you're now experiencing issues with your right shoulder as well, and we understand your desire to find answers and resolution. While we wish we could provide more insight into your past surgeries, it's essential to consult with a healthcare provider in your area who can review your medical history and offer personalized advice. If you're interested, we do offer Virtual Consultations where we can discuss your symptoms and provide general guidance on managing shoulder issues. Our Virtual Consultations are conducted similarly to a Zoom call, and we will send you a link that will automatically connect you with one of our doctors. This consultation will allow us to gain a better understanding of your condition, discuss your medical history, and provide personalized guidance tailored to your needs. Once you have purchased the Virtual Consultation, we recommend sending us an email with your questions, any records you would like to discuss, and a brief description of your medical history at least 24 hours before your appointment. This will help us prepare and ensure we can address all your concerns during the session. Please refrain from sending this information before purchasing for security purposes. After your Virtual Consultation, you will receive a recording of the session, and you will have 30 days from the date of the consultation to ask any follow-up questions regarding your session. You can email us your questions, and our doctors will get back to you at their earliest convenience. Once you purchase your consultation, our scheduling coordinator will contact you to find out the best time and date that works for your appointment. As a bonus, we're currently offering a sale on our virtual consultations with $125 off. Use the code MAYFLASH at checkout. The sale will end on 05/31/2024 To learn more about our Virtual Consultations, including cost, please click here: www.epmanualphysicaltherapy.com/virtual-visit/ Additionally, we have resources available, such as our rotator cuff tear help playlist, which may offer some relief for your symptoms. Do you know of any good physical therapists near Philadelphia?
@epmanualphysicaltherapy
@epmanualphysicaltherapy Месяц назад
Hi Sleepyhead, Dr. David's methods are specialized and may not align with traditional or commonly practiced approaches within the field of physical therapy. As a result, it can be challenging to find therapists who follow similar methods. You might find it helpful to visit the website of the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT). They have a "Find a Fellow" search feature that can assist you in locating physical therapists who have advanced training and specialization in manual therapy techniques similar to those practiced by Dr. David. Here is the link to their search tool: aaompt.org/Main/Public_Resources/Find-A-Fellow/Main/Member_Resources/Find-A-Fellow.aspx?hkey=108c9a56-f793-45e4-b3ed-cefe5d871a94 When searching for a therapist, we recommend discussing your specific condition, treatment preferences, and goals with them during your initial consultation. This will help ensure that their approach aligns with your needs and expectations.

Hi Genna, We appreciate your perspective, and while we don't believe that everyone is incorrect, we do find that much of the information available may be outdated. Have you had the opportunity to try any of the exercises to gauge their effectiveness? We're proud to share that our methods have helped thousands of patients achieve recovery, and their success stories speak to the efficacy of our approach. We understand how frustrating it can be with all the information that is out there. It can be overwhelming and oftentimes contradictory. One thing to consider is that at El Paso Manual Physical Therapy we specialize in helping people avoid unnecessary injections & surgery while traditional physical therapy tends to focus on healing post-surgery. Because of this, the information we provide will be different from traditional physical therapy advice. Also, everyone's body composition is different so what works for one person may not work for another. What's important is finding options that work for you as you know your body better than anyone. To find a solution you may need to try different techniques and exercises that you find beneficial.
