@@cookiesarelikecream If your feet ever slid in your shoes and hit the front of your shoe (this is why tying and tightening your shoes is important too) this can cause problems too. Also make sure the ankle fits too

You truly are a blessing :( .. I got mallet head to reconstruction surgery a few months ago and I wasn’t able to finish the full recovery process because of insurance issues. And I’ve been scared of my toes curling back. So this just made me so happy to see ❤️ thank youuuu

Wow, really good info, thank you! I'm from the Netherlands, i'm 57 yrs and type 1 diabetic since I was 20. Never had major physical problems, but it started last year. Achilles tendinites from January 21, followed by few accidents, and then complete rupture of my achilles tendon. This was surgically repaired in July 21, and takes long rehabilition time. So have not been able to walk properly for about 14 months now, and looks like my toes on right foot are developing into claw or hammer toes. GP recommends surgery, podiatrist is preparing silicone support to straighten the toes. Am searching for more info on how I can exercise my feet/toes. Thank you for this informative video!

Thanks for this video! My doc will no longer give me a cortisone shot for my 2nd toe metatarsal pain caused by hammertoe. Because my toe is still flexible he said I can improve the toe by wearing shoes that have more space in toe box, metatarsal pads, icing and anti-inflammatory pills/creams for pain. I've found that KT taping and Aleve work best for the pain. Now, I'm focused on working on the source of the problem, and I will start calf and ankle stretching immediately and daily, as well as wearing proper fitting shoes.

I've watched so many of these videos. I can't afford a physio or podologist so having to try and sort things out myself. My problems walking have gone from one thing to another. Started off with Achilles tendonitis which lasted for more than a decade..there were times when I literally hopped around the house. Anyways, I sorted that out by consciously not pronating...that took a while believe me! Using a.balance board was what finally helped strengthen up all my ankle and foot muscles to enable me to do that. So after years of a horribly inflamed Achilles tendon, I finally got rid of it to get plantar fascitis in the other foot!!! I over corrected the pronation problem I reckon so consciously walking on the outside of my feet LOL. Now, looking at my feet I realise I've developed a claw toe on that side too. Thing is, I've watched countless videos on each problem and in conclusion, have to state that your video is fantastic! It really brings together the nuggets of information I've gleamed in other videos. I have a very wide foot and realise that NO shoes except barefoot shoes are wide enough. This journey has got me looking at everyone's gait and feet and I've reckon that non of the standard shoes made today have a wide enough toe box for about 80% of the population! Barefoot shoes are ugly and not acceptable in many jobs. On top of that they are way to expensive for those on limited budgets. What I do now is buy a size larger than I need in the widest, least ugly shoes I can find within my budget and it seems to be helping. Anyways, thanks for the brilliant video. Keep up the great work! ☺️

I’m 15 and I developed claw toes because I’ve been wearing the same size 9 shoes I’ve had since i was 12. Today I bought a new pair of shoes and turns out I’ve actually been a size 11 for a really long time. I’ve been squashing my toes for years I really hope I can fix them cause I have terrible pain.

Don't wait to work on them if you're young. I had claw toes from about 9. I had so much pain as a teenager and it got much worse as an adult. Your feet are more pliable when young so now is the time to reverse the poor alignment. I wore spacers and went barefoot as often as possible. Also made my own splints. I was having secondary issues and a great deal of pain because I waited way too long.

I have been diagnosed with a rare condition: "Benign Fasciculation* Cramping Disorder," in which extreme cramping in one leg led to the ultimate unraveling of most or all of my left Achilles tendon. The inner calf muscle is still there, though now withered, and it still moves when asked, but it's just not attached to anything anymore at the bottom end--at the back of the heel. This has led, I believe, to my left foot's toes trying to "grip" the ground to propel me forward in lieu of that, no-longer-attached calf muscle. So I have "hammertoes" on the left foot because mine are trying to compensate for a missing "other." They are characterized by the front joint being flat on the ground, the second being steep, and the last being relatively flat again. All the work is being done by the tendons and muscles of section two. Suggestions, short of surgery? Which I should probably have done to replace the missing Achilles anyway, and not the derivative issue, the toes? [*My own fasciculations, BTW are NOT indicative of any form of ALS in my case (and thank God). All the nerves in my left leg work just as they should, so it's only the tendons, the now useless calf, and the overworked toes which are the problem...and the toes take on all of the burden for the rest, which have all gone missing. And which they just can't compensate for. Any ideas, other than pioneering Achilles reconstruction surgery? Thanks.]

I believe you are right. I have a flat transverse arch and otherwise flat feet. When I put inserts into my shoes and support the transverse arch my claw toe straightens out by itself. In fact I can see that it is especially the transverse arch that causes this problem. I also noticed that I have very tight calves. More so on the side which has the claw toe. So I shall follow all your advice on stretching. I think I might have caught the claw toe in time! Thank you for this informative video!

I have either mallet toes or claw toes (not really sure which) in all of my toes except my big toes. I have high arches and am not overweight. I’ve never had any foot injuries. I cannot straighten them out with my fingers even when I’m sitting down. I first noticed it in middle school when other kids teased me about it, and now I’m 30. I had no idea this was a medical issue, I thought I just had ugly toes.

Generally if there is popping and dislocating this might be a torn plantar plate, that may be a tougher problem, if worried always get it checked out, things unfortunately can always get worse.

So I'm coming to the conclusion that my spine injuries which led to my partial paralysis and stiff leg muscles are what led to my curled toes and plantar facsiatis. So I need to stretch my back legs and feet which I've been doing but I started walking again very slowly and only for about 5 minutes at once but if I strengthen and stretch better it might make things even better. I started noticing my curled toes straighten out after I started walking and stretching.

And then? what can I do? I think I have soft tissue adaptation, I can with effort put the finger with my hands as normal but when I take out my hand from my finger it goes the same position, and it hurts, I am 18, also I have plantar facitis from most time of my life because of weak hips (which I am improving)

I was recommended this Flexor Tenotomy. Podiatrist gave Zentoes but that doesn’t seem to help my pain when walking. Is it a fairly safe procedure which will eliminate my pain without long term complications? Do I get a second opinion?

When the Dr. says tight tendons, what he means is that calves muscles and tendons, and the plantar fascia, and and such, get overstretched...So the tendon part under the toe gets pulled and stays like so...for having to compensate for all the stretch on the previous section. That’s his #1 reason for hammer toes.

Absolutely, overpronatian leads to the long flexor tendons and the long extensor tendons trying to re-supinate your foot, this causes the metarsal joints to bend up and the proximal interphalangeal joints to contract downwards

Hey Doc nice content. I love how u clearly state what works and what doesnt. The Gaitguys stated its more a hyperton EDB causing the toes to curl up which make sense because of the lower attachment. And I thought the EDL everts the foot, how can it contribute to supination?

Hi, this was a very interesting video but I have a question. I'm quite sure my claw toe (next to my little toe) is caused by ill fitting shoes. I have a really wide short foot with a slim heel so finding a shoe that fits with a big enough toe box is hard. I have strong arches, I rollerskate and stretch daily. My toes in general have been starting to straighten up since switching to barefoot, flip flops or crocs wherever possible but this one toe seems like it needs a bit of help. I have found when hiking it has caused me pain and a bilster in the arch of the toe over 10-20 miles. in this case is the best option to use toe separators to try and train the toe to be straight again or is there something else I should do?

I studied each of my toes til I understood how it was moving wrong. Some were S-shaped when forced "straight" after being clawed for so long. I walked on the edges of my toenails which really hurt. My feet are similar to yours. Really poor fitting shoes all my life meant scrunching my toes up tight. My RT foot being worse, when I'd wave all my small toes downward, doing exercises, they'd move abruptly to the left, pushing tightly against my big toe. The muscles were conditioned to move sideways. Using wide spacers and exercising them up and down was key for that issue. It took a couple months to override the misaligned movement and now I can wave them up and down while spread apart. I still have to focus a bit to do this and many other vital exercises, but it was impossible before. I feel like I walk as close to normal as I did as a very young child. The corns and deformities are almost gone. It feels great. Good luck

We have a lot of success if the toes are not completely arthritic yet, even then a properly fitting shoe will stop the knuckles from rubbing

I've seen a podiatrist for my obnoxious feet, and he did quite literally nothing. I know that I've probably multiple issues causing the problems with my feet. But I was so totally disappointed with him I'd really not gone further. I have long COVID, and for the past two years I've had a series of issues. Much of my problems from it were from my musculoskeletal system. My left leg spent about a year known as my "Gumby Leg." It started with my knee, went to my ankle, into my foot (COVID toes), back to my ankle, and after PT wound up with almost everything working except the knee. In the meantime, it moved to my right side, attacked my hip and knee there, my lower back... So I know a myriad of issues have affected my feet. I do have two hammer toes, and I believe my 4th toe on the left foot is a claw toe. I have neuropathy also, primarily in my... left foot. I am going to try to do some of the exercises you recommend. Right now I'm in PT for my right hip, hoping to avoid replacement surgery. I see a rheumatologist in a couple weeks, and will know if this is going to give me a reprieve. So THAT is my primary task at the moment, since both my hip and the right knee have started popping and snapping like a bowl of Rice Krispies. (I'd heard about this, but had never imagined how awful it actually sounds!) I guess I may be thankful I'm also in Michigan, albeit on the other side of the state. I believe it's close enough to get a referral and make the drive over to see you. I now see a homeopath in Adrian, who is helping me with my Long COVID, so you're only about twice as far!

I have claw toes, supinate, and if I stand for more than 30 minutes at a time get very bad pain on the bottom of my feet (heels mostly). It's cost me several jobs and prevented me from pursuing careers. Also, I put holes in all my shoes above my big toe.

My second toe goes down a bit but the third toe, it's curved in and curled, not sure if it's claw or hammer toe but I can't get any shoes on and it hurts from rubbing on my shoe. What can I do for my third toe??? Everyone seems to speak of the second toe but it's my third toe that is the worst and I have a bunion on that foot too, this happened about three years ago.

I enjoyed the video, my only issue with it was I was waiting for you to give the stretches that would help. I have a 12 yr old daughter who has claw toes/ mallet toe. I’m thinking it’s bc she wore shoes that were too tight and small as a younger child. I used to ask her how her shoes fit and she would say “good”. I made the mistake of going with what she said verses getting her feet measured. I regret that till this day. I learned with my other kids but she now has ties that are bent under and she doesn’t like how it looks. Plus she plays basketball and she finds it harder to run on her toes to be faster. What stretches would you recommend to help straighten out her toes?

Does having knock knees contribute to getting claw toes? It seems like toe deformities are genetic in my family but mine seems way more advanced at a younger age than my relatives.

I have one hammer toe that I think is correctable. It came from an over walking challenge. I’m thinking of getting barefoot shoes. Will this hinder or help the hammer toe issue?

What do you think about barefoot shoes? I transitioned to barefoot shoes about a year ago and the majority of my knee pain has gone away. I also do squats for long periods to strengthen my knees and legs. But no matter how strong my feet become or my legs have my two little toes on my left foot face in words towards my middle toe. The rest of my toes have spread out nicely and look normal. But I just can’t get these two toes to do anything. And I’ve noticed that my knee pain on my left leg is still there, but not as bad as it was before. I can’t help but feeling like my toes are the cause of my knee pain, especially because it’s on the same side as those toes are.

i just panicked a while ago when my second toe just curled in like a hammer toe. it was painful like a cramps that i was actually terrified that i have to pull it back gentlyyy. what do i do if it'll happen again? edit: i think i overused my whole legs and feet since i have to walk like a 3 or 4 kilometer everyday ever since i moved to another city. idk, maybe it has contribute.

When I run, even with proper shoes, sometimes my pinky toes (or outer two toes) feel numb) and I’m not sure why I also have a mallet toe on my second toes on both feet but they’re still flexible enough for me to bend them straight. Would you separators work? I have flat feet when I walk

Wish mine was for easily addressed. I broke all three toes on my left foot 25 years ago running up stairs in Birkenstocks. Now I have a super stiff big toe and the next two closest have that mallet look. They are also turning a bit inward towards the big toe, which is the way they were probably allowed to heal, because I was a in my twenties and indestructible. Lol

I have claw toe on my left foot the third toe when I was born my foot was bent inwards and and over time it’s started to curl down to the floor I saw a doctor yesterday and he’s referred me to a foot surgeon

They have minimally invasive surgery where they cut the tendon and let it then heal naturally. Still have full flexibility. Process takes 15 minutes and recovery time very fast. Edit: I’m not a doctor but I’ve watched procedures and explanations by other podiatrists.

Absolutely we do a lot of these in the office, but the tendon is being cut and not fixed. This can be good if there is a wound as you are 80 years old but might not be the ideal choice for a young healthy person who can correct the reason for this occuring in the first place.

@@dennisb7317 Most info as well as my own experience says yes! If you can manually straighten toes out with hands, you can use spacers, splints (made my own design), and most critical, exercises to realign joints into a more normal position. I've worked on mine about a year, off and on, and they look and feel great compared to where I began.

Thanks for your videos! Can you tell me why my 3rd toe is separating from my second toe? There is a gap between them now that I am trying to remove. I do have Morton's neuroma that showed up after I broke my ankle and got out of the cast. And now my toes are moving apart???? I have a bandaid type device that holds my 3rd toe to the 2nd toe trying to fix it, but I don't know if that is the right thing to do. I am not overweight, 115 lbs.

@carolweaver3269 You are wonderful at explaining this and all you have said is totally in line with my podiatrist, I felt I should have surgery as all of the toes are hammer toe and one next to large one claw toe and I am 72 yrs old. The Dr said I need to change the type of shoes and larger shoes and clogs he recommended. Thank you for helping me to have a little more faith in the Podiatrist, as it made me wonder and yet do need x-rays to be taken yet. But he said: 'If you do not have pain, it would be best to not operate."

Overpronation of your subtalar joint puts your toe tendons and ligaments in akward positions for function. Making your leg flexible can out them in proper position.

Ever since I was younger my toes all have spread out I don't know how to fix it. I do remember when I was Younger I hated wearing shoes because it hurt my feet.... I did surgery 5 years ago and that pretty much did nothing to help me.

I've mostly away worn open shoes, flip flops - thongs and slides, but I have a very high arched foot naturally and I realise now that those are very flat shoes. So you're essentially saying that I should switch to wear shoes where I can put in an insert to support my high arch to straighten my NO2 little and NO3 little toes - I have two hammer on each foot with the first joint bending up 😬❤️ ? Xx

Enjoy your post and your information and have subscribed to your post. I'm dealing with 2nd toe inflammation for the past three weeks. Doctor gave me some antibiotics (pills) and then cream which haven't reduce the swelling. The 2nd toe taping with KT Tape is the only solution I found that have reduce the swelling and pain. When I take off the kt tape, my toes swells and also under the palm of my foot, develop a watery callus. Any idea what it might be. Any feedback might help me. I used to go for walks , jogs and going on the treadmill which I can't and feel useless and frustrated. Thanks.

Multi sport athlete growing up. I had hammer toe corrective surgery at 19 because I was having pain with every step I took. It eliminated the pain but Im 34 now and my toes are in pretty rough shape. Terrible problems with toes curled and now it’s a different pain. I would avoid it and try everything else.

I've been dealing with extremely tight calves. Been going for acupuncture treatments and wow what a difference to loosening my calves. Feet still hurt though. This week my Hoka runners are being delivered. I bought these based on my podiatrist's recommendation. I sure hope they work. Btw....I can't find any good inserts. Just bought a $40 pair and they're not helpful. I'll check out what you recommended. These are for indoor barefoot shoes. Thanks.

A lot of the time starting with a softer insole, even the cheaper Dr. Schools at the store is a start, then as you get used to it moving up to more supportive insoles becomes very beneficial

@@DrTomBiernacki my Nike, which I've recently stopped wearing due to wanting a wider foot bed, has orthotics in them. Right now I'm thinking of trying them in my barefoot shoes. I bought metatarsal pads. I should try them to see if they help relieve forefoot pain. I'm doing foot strengthening exercises, as well as loosening my calves, and quite importantly, losing weight. The latter is not going as fast as I'd like. I appreciate you and your videos. Thank you 🌻

Is the mallet toe supposed to occasionally hurt? I get a stabbing pain that's really bad in the underside of my mallet toe. I have a painful bone that sticks under my foot due to the mallet toe. How to correct this?

What is it when your large toe turn upwards at a high angle I’ve always had a shorter large toe that has a high angle so when the toenail grows it always rubs against the inside of the shoe over time cuts through shoes

Steel toed 🥾 boots messed up my feet. Doctor said I have hammer 🔨 toe he noticed right away. It hurts to walk barefoot or stand for more than 2 hours. I skateboard and do a bunch of other sports I can’t have this injury anymore and have it get worse.

Its not necessarily the lack of strength in the toe muscles, but the position of the foot and leg that is the problem.

This is a long term adjustment, the ligaments and joints get used to this. Sitting for 1 hour won't reverse decades of changes. Temporary deformity leads to contractures leads to eventual arthritis.

It's hard to say without in person evaluation, most people have some soft tissue adaption. This means that even though it can be moved back by fingers, it wanted to stay in the hammer position. This means the soft tissue is used to that position. On the other hand if the bone is rock hard that means arthritis.

From your explanation, I gathered that due to the way the tendons and muscles of the foot attach to the toes, an overpronated foot position shortens these structures. When there is chronic shortening in these structures, they begin to pull on the bones in the toe causing the claw toe, hammer toe, and mallet toe deformities. Was this correct? Or am I missing something? Thank you in advance for your response

@@DrTomBiernacki I also have two more questions if you have the time. First, I have hammer toe in the third toe of my right foot, but it only occurs when I'm performing plantar flexion under load(ex. Calf raise). Do you know what's causing this and if there's a non surgery treatment? Second, I have an adductovarus deformity in the 5th toe of both of my feet. I also have very poor motor control over my fifth toes compared to my control of the other four. Is there a way to gain better control of my pinky toes and to correct the adductovarus deformity? Hi doctor! I have self-diagnosed flat foot, and I do ballet twice weekly where I have to stand on my toes and walk and turn for pretty long periods of time. I have bunions at the big toes but I also have a lot of pain and soreness on the pinky joint area. It’s really weird because I can hear my pinky joint creak when I roll my foot over which I always do in ballet to maintain balance and I am very worried about it. Are there such thing as pinky bunions? Could you do a video about how pointe shoes affect the toes? Or maybe just walking on tiptoes may affect the feet? Thank you so much!
