Contraindications to low back arching in discogenic or multi-componental low back pain

As seen in other videos, I recommend arching the low back in the treatment for discogenic low back pain. This will improve disc loading angles and enable the proper muscular support in the low back. However, this might worsen the symptoms for patients who have known or unknown foraminal stenosis due to facet joint arthritis, as the neuroforaminae (NF) narrow with extension. This is not a problem for healthy NF, but is a problem if the patient has preexisting narrowing.
Moreover, patients with low lumbar anterolisthesis (anterior slippage of the superior vertebra) may also see progression of listhesis if the sacral anteversion ("arch") is excessive.

What do you think about keeping the chest up? This seems to arch the upper back, which it feels like my body is not used to. But I feel it helps keep my clavicle raised and my neck long. Also seems to give the abdomen more room for breathing, which in turn has lead to less clenching since I can take deeper breaths.

Has some utility in symptom relief but it won't cause any long-lasting effects.

Hi, I have a bizzare question. I have spondylolisthesis grade 2 with desiccation of l5 s1 disc. I also have atlantoaxial instability. If I go into posterior pelvic tilt, I'll induce forward head posture, this worsening my craniocervical issue. If I go into a lordotic posture/anterior pelvic tilt, I'm increasing shear forces on my l5s1. Is surgery likely the only way? Thanks.

Question - if it increases symptoms to increase the arch in the back (causes increasing pain upon arching and significant increase when trying to relax back into normal posture), and decreases symptoms when going from the natural arch to a more flat lower back, would this likely be suggestive of one of these two conditions (or other where arching may not be optimal?

Look at my low back pain videos, especially the anterior pelvic tilt video. Only arthritis is a contraindication, not pain itself. It depends on how your images look, and also on whether or not radicular pains or myotomal weakness occurs with extension. I would have to review your images to answer this.

Is "mild facet arthropathy L3-L4" with no foraminal stenosis a serious contraindication? I'm hoping most of the pain is due to muscle weakness and extension on the injured discs as you've mentioned. Is "mild arthropathy" a contraindication like arthritis, or is this something I should push through and see if it gets better? It's tough to get a straight answer from doctor and PT. What do you suggest for patients with contraindications? Or is that something that is case-by-case?

If you arch your back and develop L3 radicular symptoms then you'll know. This problem is usually not reported properly by radiologists.
