Yes you can use Integra bilayer with the wound VAC. Since it already has the silicone layer as your secondary dressing the vac can be placed directly over the bilayer (note that if you use any other graft or skin substitute such as amnion you MUST use a secondary such as Mepilex/Adaptic touch) I will try and post a video about the integra bilayer soon!

Vac settings over grafts/skin substitutes- yes using lower settings eg 75 high or 100 low/intermittent - the rational is to optimize graft survival. Remember though that top 2 reasons for graft failure are 1) shearing forces and 2) fluid collection (hematoma/Seroma) under the graft . Clearly the vac nearly eliminates the first by virtue of its stabilization of the graft. If the vac settings are too low and fluid does accumulate then it may affect graft survival. So if you’re getting leak alarms or fluid under your grafts on the low settings then slightly increase the pressure or intensity. It’s a fine line and takes experience and practice.

BlackRedDragon777 all great questions 1) wound was from trauma 2) skin graft from back of head is ideal Bc nearly no pain from donor site, all the hair grows back, and the scar is hidden! 3) The foam conduit is fundamental to the integrated V.A.C. Therapy system. The black foam - V.A.C. GranuFoam Dressing -is a polyurethane (PU) , reticulated open pore foam dressing and is considered to be the most effective at stimulating granulation tissue while aiding wound contraction.

The sponge allows the wound to be under moderate vacuum and for excess fluid to be drained, but a little to stay in the would because it vastly improves healing. The whole setup sort of works like a blister protecting the site, but even better because of the vacuum.

midnight snow from what patients tell me, it’s actually much more comfortable than having a huge skin graft wound on the anterior thigh. The thigh wounds are very painful, bleed, and have a huge scar afterwards.

ScM40K good question. No, this wound VAC device does not take the place of a tourniquet. If someone is bleeding heavily the best idea is to call 911 and go to the nearest ER. If one is a medical professional and is trained in placing tourniquet, or if the EMT places a tourniquet, then that may stop heavy bleeding, but again, the patient needs to get to an ER quickly

Can or should? Arterial ulcers should primarily be addressed by Endovascular or open vascular treatment (if indicated) Arterial ulcers should only be treated with wound vac if vascular intervention is not indicated or available, and if the treating physician determines that this may be an off-label utilization primarily (Since wound vac isn’t usually placed over arteries directly due to risk of hemorrhage) It also should be mentioned that the physician should determine between arterial and venous stasis ulcers (of course)

@@arthurdesrosiersmd Angioplasty was done recently by vascular surgeons(not successful as patient is 92yrs old, had bradycardia) to the left leg and calcification still exists in tibial arteries. Don't know to go with VAC or PRP therapy..please suggest doc. Ulcer is just 1inch in size-superficial-since 5months.

Mozi Jakob great question. This wound vac is part of our limb salvage protocol and allows us to save limbs from amputation. This is one modality in the protocol. The wound vac itself acts to speed up wound healing, decrease infection, and improve patient comfort

OMG!!!! I am wearing a Vac now as well. My wound is healing really fast. But taking the foam out had me crying so hard, the first time. My second visit taking the foam out didn't hurt as bad. Tomorrow I gotta go back to Wound Care. I'm praying to God it doesn't hurt as bad as it did the first time! Hopefully a few more weeks and I should be finished with the Vac!

Desi N good questions! 1. The wound vac is useful for several reasons. In wounds that are not healing in chronic wounds when patients have multiple medical comorbidities, the wound VAC is useful because it aids in granulation tissue, decreases the risk of infection, improves patient compliance, and facilitates easier dressing changes for nursing staff. In trauma patients, we use it as a temporary dressing as a bridge to definitive surgery, for example, when the patient is in ICU and too sick for flap surgery. 2. how does it work? The wound vac uses a black Sponge which is reticulated and has air cells, that you see in the video. it then is covered with sterile plastic clear stickers which prevent infection and help to retain moisture to facilitate wound healing, and then this is connected via a sterile tubing to a small box with various suction settings. In general, the suction functions to promote wound healing, speeds up wound healing, and decreases the time to final healing endpoint.

We often do use the silver sponge when wounds are infected, the silver acts to help as an antimicrobial. In fact silver is one of the oldest antimicrobials and has been used for thousands of years.

Mohamed Osama Thank you Mohamed. I have, he said it's an unfortunate side effect for most patients to using the unit. I have found that changing the canister helps somewhat and now, when I have a special event, I just remove the unit. Thank you for your response.

I would go back to the doctor that placed the wound VAC unit. She/he should make sure there isn't an infection in the wound. Alternatively, it could be a normal smell if the doctor used a wound matrix such as a porcine SIS such as Oasis or ACell. Depends. But see your doctor.

Literature demonstrates that negative pressure wound therapy (wound vac) causes improved healing times because the vacuum pressure “shrinks” the wound and also demonstrates decreased complications for the patient because it seals the wound from external bacterial contamination etc

Kyle Gardner. Yes I was on life support & in icu/hospital for 63 days last year & the ECMO ( life support) was hooked to arteries in my groin. I have 4 inch incision on each side of my groin & I know the wound vac helped me heal faster but they changed it every 3 days & it was extremely painful every change. But all of the Drs/ surgeons said it would have taken months to heal without the wound vac. But my right incision became infected & plastic surgery had to put a skin graft over the artery cause it wasn't closing on its own.

it was originally a traumatic wound, but because of the underlying diabetes, it developed into a chronic nonhealing wound

Gavined It tracking the wound vac is a very useful technique, in some instances. However, it isn’t necessary in all cases (such as this one).

The wound vac can damage normal skin bc it is suction on normal skin causing skin breakdown and damage

Paul Hay yes in the United States we do have less complicated systems that are more plug and play (you can watch videos of these PICO here on our RU-vid channel)- but these are very very limited in their capacity to perform. They do not pull more than 70 mm hg of suction (unlike this wound vac that will pull 200 mm hg). They will fail miserably except in the driest of wounds- any fluid or exudate and they stop functioning. This wound vac is intended for doctors or advanced Practitioners who are trained in this special technique.

Lot 313 the sponge does not stick to the wound. There is a thin porous wound dressing called Adaptic that is placed over the wound, and under the black sponge. The vac doesn’t directly touch the wound at all. As for the cause of the wound, it was a traumatic injury.

@@josalynvilla8862 I only needed it for one month (thank God). It wasn’t super painful but it can hurt if the person putting it on don’t know what their doing. When I first started I had 2 open wombs which turned into 3 bc the first nurse that put it on messed up. But I’m healed now. I avoided a 2nd surgery bc of the womb vac.

Also, she does not touch the wound. And no, this patient did not get a wound infection with the vac. In fact, the wound vac helps to decrease the rate of infection.

ZyanHD actually this patient surprisingly did not have diabetes. However he did have many multiple comorbidities including peripheral vascular disease and multi drug resistant bacterial infection that was eradicated with dakins wound wash and hyperbaric oxygen therapy
