Its a common issue in Discus fish and its cause by a common problem in most aquariums. All fish have parasites as do all fish tanks. Most fish in good health can deal fine with a certain level of parasites but if you dont maintain your tank and water and your fish gets stressed the parasites can soon get the better of your fish. And then its a downward decline.
Theres a few causes but this might be the one. But without identifying the real issues its difficult to treat with random medications. There doesnt seem to be too many common and likely parasites and there is only a handful of medications you can use or get. But getting the right one for the issue you have is important as is the speed with which you react.
