Dr. Bikman is an absolute Godsend! He has helped me to understand the biochemistry of metabolic dysfunction and how to steer my body into healthy territory. Fantastic lecture, I will be forwarding this to many people.

I’m T1D and my sole objective with this condition is to minimise my insulin while maintaining a ‘normal’ HbA1C…. I never eat carbs…. Eat modest amounts of dairy and eat loads of beef….

I'm 40 and went high beef too. With fasting and low carb my strength suffered and noticeably went down. I did not pay attention to micros nor eat a hugely varied diet. No organ meat and little veg. I think the MTOR path needs to be activated. I think the carnivore folks can eat enough protein in a sitting then exercise hard enough to activate it. To stimulate muscle growth. I eat carb to some extent with protein so i don't need as much protein to elevate insulin some and activate MTOR. Low insulin and never raising it can eat away at muscle. or rater hinder muscle growth. Something to consider and my personal issue.

Always a fantastic presenter. Having someone else controlling the slides was slightly distracting but Dr Bikman knew them cold, so just power through that challenge as it is worth it. I wish every professor was as knowledgeable and concise. Dr. Bikman is my go to for ketogenesis, blood sugar and insulin. Now we just need to educate the medical profession. My internist does not appear to know any of this. It is unconscionable.

2:17. What I find interesting right off the bat is this slide shows all of the markers for metabolic syndrome and LDL is not listed - this tells me right away that this fellow probably gets it.

WOW SO literary the last 3 minutes he explained WHY a diff doctor... Dr Shawn Omara is ALWAYS talking about how fermented veggies and or fruits are so GOOD for you .. short chained fatty acids including vinegar. Who knew. NOT i, that vinegar is a fat. I've learned so MUCH from you Bikman it's ridiculous. Oh I'm no clinician... but i work for them ..

Develop muscle mass and strength and you'll see an increase in insulin sensitivity...eat more protein and avoid ultra processed food and you'll see a huge difference

Dr lustig mentioned the glucose being normal but insulin being sky high. Or could be. I was pre-diabetic by my own analysis. One year apart from major diet changes. Fasting glucose was IDENTICAL. LCHF OMAD. Thank you all and digging into the science and what matters. I'll check again soon. I bet it's finally improved too. Maybe not. But will order the fasting insulin this next time too.

@@sorrykay3450. it’s not too expensive. in canada it’s $22 and comes with fasting glucose so that Homa IR can be calculated. i ask my dr to order it during my yearly blood labs and then i get it through a naturopath at least twice a year. i think it’s well worth the money.

Unfortunately Why We Get Sick argues that insulin resistance is the root cause of most chronic diseases. To solve this problem, the book recommends lifestyle changes that are supposed to keep insulin levels low, including fasting, exercising, and restricting carbohydrates and omega-6 fats. Key points from our review The book’s argument that carbohydrates and insulin are the main causes of insulin resistance is not well supported by evidence. 9 of the 10 references we reviewed were either irrelevant, not convincing, or weakly supported the claim. The book’s diet and exercise recommendations will likely help people manage insulin resistance, but we have concerns about heart health since there are almost no restrictions on eating fatty animal foods. Many people will find it hard to stick to the rigid low carbohydrate intake. @@trotskyite1 Your comment might be useful if unprocessed animal foods that contain natural saturated fats were actually associated with a risk of myocardial infarction or cardiovascular disease mortality, but the fact is that they are not. There is no link. In fact, these foods are significantly associated with lower total mortality as well as lower risk of stroke. So really there is no need to be concerned about heart health. Dr. Ben Bikman's recommendations remain entirely convincing and promote nothing but good and improved health. Useful study here: www.ncbi.nlm.nih.gov/pmc/articles/PMC9794145/ As we all know carbohydrates are not essential in the human diet - there is zero requirement to eat them in order to be healthy. In actual fact, the less of them we eat (particularly ultra processed carbs/foods), the less our brain and body desires them. So sticking to a low carbohydrate diet and focusing on one that is rich in quality proteins and healthy fats will leave people feeling fuller and more satiated for longer (no more hunger and cravings) so that they will no longer want to eat ultra processed carbs and snacks throughout the day. It really is quite simple and easy to do once you give your body the proper nutrition it needs to thrive.

At the end wow, I didn't know that acetic acid is a (very) short chain fatty acid. That explains people advocating for cider vinegar, which up to now I've dismissed as having zero use. Of course, that doesn't quite explain why acetic acid from apples would be better than acetic acid from other sources... a question of taste?

Well spotted, I also wanted to comment on that. It was a slip/mistake. You cannot lose fat if insulin is HIGH as insulin is an anabolic hormone and aims at targeting/opening the insulin/glucose receptors and channels on cell surface to cause the adipocytes/muscles/liver to either use or STORE carbohydrate/sugar as fat. The bit he says after that also confirms the slip namely "It is impossible to make a fat cell grow without elevated insulin". Hope that helps.

I see you didn't get an answer, so I will share my experience. I have no gallbladder and eat a very low carb diet with plenty of fat and protein. I don't have a problem. However, I do on occasion get a big dump of bike that results in an urgent run to the restroom! That isn't often and was not anything new as I sometimes had the same issue on the standard American diet. I will say I have absolutely no regrets on living this way! I've never felt better and my blood sugar is very much improved and I trust that my type 2 diabetes will soon be reversed. If you try it and have trouble with fat you can take digestive enzymes and or bile salts to help until your body adjusts. Hope this helps!

Great presentation. Concise and very informative. Answered most of my questions about insulin resistance. I have a question though-can one have primary pre hypertension and also be insulin sensitive? I am insulin sensitive judging by my HOMA-IR of 1.5 but I have pre hypertension.

I have PCOS and IR. Thank you for this great information. I don’t say that I have “Metabolic Syndrome”. I say that I have an endocrine disorder that could become Diabetes if I don’t keep it under control. I inherited PCOS from my maternal line. It’s not fun. :(

Loved this clear explanation of insulin desensitization over time in the human body. One question: Is it possible that Metformin is causing mitochondrial uncoupling? If this were the case, then it might be a disservice to characterize Metformin's effect on the electron transport chain as poisoning. Perhaps it is an example of hormesis?

For measuring insulin resistance on your own: 1. hire a CGM meter and check if your insulin drops half an hour after eating a lot of carbs. 2. Test yourself! Dump nearly the carbs for one month and see what happens, did you loose weight, belly fat, improve body composition, feel better after a meal etc?

You don't understand. 1. Insulin is the fat storage hormone. Without insulin you cannot gain weight, you loose weight. With a CGM meter you kan measure your insulin reaction to different foods (insulin spikes) and if your insulin stays high for a long time after eating carbs and sugary food or if the spike drops quickly and everything returns quickly to normal 2. test yourself,: avoid all food that spike insulin (carbs and sugars), therefore your insulin production should be minimal and you will loose the weight because the hormone glucagon will kick in and release the fat from the fatcells@@HAL-1984

Ben is great. Insulin resistance is just a diagnostic concept, not a disease. It is actually a healthy response to excess glucose. When it is chronic it becomes a serious problem and leads to diabetes2.

Amazing conference ❤ What is the dietary solution for people who got their gallbladder removed? I know a few who ended up having huge problems with fat digestion. Are they doomed to no longer be able to be on a high fat low carb diet? As surgeons tell these people just to reduce the fat intake. But isn’t this pushing them to eat lots of carbs?

My sister has had her gall bladder removed. She is a Carnivore, as am I. The answer, is most likely to eat smaller meals as a few times within your eating window. It gives your body time to deal with each intake of fat without overstressing your body.

So what does one do with drug induced type2? My wife has to take Prednisone for inflammation for a significant lung condition. At 5'5", she weighs 100#, and we eat exceptionally nutritiously in the manner described here. Yet blood sugar is very much an issue because of the steroid. Thoughts?

GPs only request a standard lipid profile and then prescribe a statin. I'm 60 and never had a doctor ask what I eat or if I exercise. Must get the profit motive out of medicine!

Wondering what your thoughts are about Bart Kay send that insulin resistance is a construct. (An idea or theory containing various conceptual elements, typically one considered to be subjective and not based on empirical evidence.)

Yes, insulin resistance is a construct -- just like intelligence. In no way, does it mean that insulin resistance doesn't really exist -- but only that it may not be directly observable and easily measurable (e.g., readily observable phenomena are the height of a person, or the weight of a bar of iron). So, when scientists propose a construct, they have gone one step further from just defining a concept (such as insulin resistance or intelligence) and have proposed ways of how to measure it. This, then, provides the basis for including it in conceptual/theoretical models that can then be empirically tested. In other words, creating constructs is a necessary step in the scientific process. So, in fact it is just the opposite of what you (or maybe Bart Kay) suggest, turning something into a construct, provides the basis for including a concept in scientific studies.

Hi this is Dr Hari from India, had noticed a strange phenomenon. During fasting state the glucose levels are around 70 mg/dl and soon after taking low carb diet the glucose levels are raising to 130 mg/dl and remaining high for the rest of the day. Any comments on this . How can I personally contact you.

I think he would say test insulin. That fasting glucose lags behind what the fasting insulin will tell you and that once fasting glucose is up, you are already dealing with insulin resistance.

Ben, having followed you very many ears, you are among the brightest minds to solve this puzzle of un-illness. Therefore I urge you to make the connection between water EMF radiation, melanin, POMC and the leptin melano cortin pathway. I am a solid state semiconductor research scientist. Is Jack Kruse after something entirely significant. To me it seems that is the case. Still working to get the insulin connection and it might just be leptin.

49:15. One of the static ways to determine insulin resistance is triglycerides:HDL. What is the ratio for this? I’m assuming you want low triglycerides and high HDL, but at what ratio is bad? Good? Better? Best?

Less than 1.5 is normal. Less than 1 is optimal. More than 1.5 insulin resistance, heart diseases , etc

Please could someone let me know if a total cholesterol result of 363 is a problem to my health? I am in the uk so I multiplied my uk score of 9.4mmol/L by 38.66 All my other blood results look good

A better measure is to divide your Triglycerides by your HDL. I'm in the US so we have a different number system. For the US, if the Trig/HDL is 2 or less (the closer to 1 the better) then you shouldn't have anything to worry about. There are a bunch of calculators online to convert your numbers to the US system. A great book to read is "The great cholesterol myth" written by a cardiologist.

@@countryliving9718 thank you, I have done that calculation and the result is 0.3 I guess that is OK My doctor wants to see me, and as my LDL is over twice the uk top range, I expect that is what is worrying to her. I am wondering what she will think when I don’t want to take a Statin
