Factoring Trinomials Program for TI-84 Plus CE 

For a tip, you can use grouping to easily simplify a 4 term polynomial into a trinomial

just graph y=(radical)/X(squared) then go to the table and find the lowest y value you can. Then your simplified radical would be x * squareroot(y)

Have you checked if you are using Alpha X and not x?

It should still be on the calculator after you clear it

Depending on how you clear your calc will decide whether the program will still be on it. If you clear it using the reset button on the back or it runs out of charge, it will completely clear the programs. You can download it to another device and reupload to your calc or re-input this program every time. However, if you hit the normal clear button, it should keep this program.

You programmed it wrong

I had the same Error and when I worked it out it was because in the second line of the "(-D" I used a negative Sign instead of a subtraction sign right before the square root symbol, that was my issue.

First pull out a 3x^2 This will result in (3x^2)(x^2+2x-15) You can now factor (3x^2)(x^2+2x-15) into (3x^2)(x-3)(x+5) This is your polynomial fully factored.
