First due cardiac arrest, 2nd due MVC roll-over

Ride along with Paramedic B-74 running back-to-back calls. The first call is a reported cardiac arrest in the western end of their first due. The call turns out not to be an arrest but it was a serious medical emergency, with county paramedics and a medical assist on the call.
After transporting that patient, Paramedic B-74 drops off the county medic at their station and in the process of that a vehicle accident call comes in just down the highway. Paramedic B-74 ultimately is added to that call as well.

@Jacob Webber Does that sound just when you get a call at the station, or do it also go off like ours does here EVERY DAY at noontime? If I was living in your neck of the woods, I would rather have it just go off ONCE at noontime everyday, wouldnt know how long it would take to get used to hearing this go off maybe Many times during the day, and most definiatly Hopefully not at nighttime.....LOL

Shane Shroyer the effectiveness of changing siren tones at an intersection has been proven “effective”. Personally I’ve had more success with air horns than any siren.

I'm a bit jealous of the short response times and the heavy resources. I am a Division Supervisor of an agency in Alaska. At my location, we have 1 Paramedic ambulance that covers 55,000 square miles first due, with 1 volly BLS ambulance that covers about 2000 square miles within that area. The rest is us. Next closest ALS is 150 miles north, or 150 miles west by highway. Plus side, we use a lot of skills, instead of short transport times. All told, my agency covers 1st due, an area the size of Arkansas with 2 ALS ambulances. I love it, and I love your videos.

@@majorpizza383 We have what is called "expanded scope" of practice. Basically, if your physician sponsor signs off on a skill, you can do anything the physician allows. I am an EMT-2, which would be fairly close to what was an NR EMT-Intermediate. As a EMT-2, my certification allows me to start IV's, IO's, and give a few basic medications. If I had current expanded scope like I did with a previous employer, I could also give CPAP, Diphenhydramine, Epi 1:1000 IM/SQ, Nitrous Oxide, Nitro (not script and w/o calling Med Command), D5, King Airway, IGel, Combi-Tube, 12 Lead procurement, and even stitching. I work with a Paramedic, and I help them with a large amount of things on scene, or if I have to stop the ambulance. There are many things that I can't do of course.

apple pen We are allowed to use GPS, however many of us have found it to be unreliable quite often, so we end up not using it the majority of the time.

It’s a volunteer fire department, the siren is usually mounted on the station itself, and sounds when a fire call comes in.

Paul Sale Thanks for watching Paul! I believe it was due to the information on dispatch. The incident was initially believed to be much more serious than it turned out to be. The chase unit provided ALS providers, the ambulances provided BLS transport, the Ladder truck provided rescue tools and manpower, the Engine provided manpower and water if needed for a cut job, the police made a large response also believing the accident was much more serious than it turned out to be. Everyone arrived within a few minutes of each other. Many units cleared once it was understood there was no entrapment, minor injuries and not multiple patients.

Most fire dept members cannot have any lights in their personal vehicles in my area. If we could, and I could choose any set up, I would prefer red over blue. Best option would be two colours and I would choose red and white. Red is much more visible than blue and white obviously is brighter than red. So I believe that would be the best combination for visibility/safety.

Delmar MD/DE is split between two states. The Paramedics in Delmar are certified in the State of Maryland and have Delaware Privileges. It's a teird system in Delaware so all FDs are BLS with the exception of Delmar.

Jusxt one question if I may, are your fire departments there able to control Signal lights so that when you come up to one, I guess the way it works with a push of a button either from a remote Control or maybe something on the trucks dash, you can minipulate the lights so where the direction that youy are going will stay green and the other directions red so you can make it through without actually having to pause....

Yes, it's called an Opticom. It is built into the lightbar and automatically activates when the emergency lights are on. It can be shut off separately as well, when needed. The newest one's are infrared, older ones are detected by the white strobe light.

Edwin Ortega Thanks for watching Edwin! We are in a unique situation, so the answer to your question is potentially complicated, however to keep it simple, I’ll answer this way. We are on the border of Maryland and Delaware. We run ambulances with an EMT (BLS) and a Paramedic (ALS) and all of our ambulances are ALS equipped. This set up is standard in Maryland, however in Delaware ambulances are BLS staffed and equipped (by Fire Depts) and Paramedics/ALS units are provided by County governments only. Delmar had paramedics long before the State of Delaware did and is an exception. In this video the calls were on the Delaware side of the border and they also respond. In that County the Paramedics respond in chase units/fly cars (whatever terminology you prefer) and meet ambulances on-scene.

Don Coleman For the most part everywhere in Delaware operates this way. I believe the City of Dover contracts with a private ambulance company for BLS/ALS service. I’ve been told that anyway. If that’s not entirely accurate then it’s likely Kent County ALS chase units would assist that company. In Wilmington there is a private ambulance company that handles BLS calls and New Castle County ALS chase units handle the ALS needs. Those are the primary urban cities in Delaware. Everywhere else, that I am aware of, Fire Dept’s handle EMS in a BLS capacity and Counties handle ALS by way of chase units.

Greg W. Thanks for watching Greg. No, 102 will remain available for calls. They would respond with one paramedic and a second medic unit, a supervisor unit staffed with one paramedic would also respond to back them up. In Sussex County, two paramedics respond to the call initially. One paramedic can transport in most cases. If it's a serious patient then both paramedics would transport. In this case, the patient did have two paramedics and an EMT in the back giving care, it was one DFD firefighter/paramedic and one County paramedic.

jslost It means response area. A fire station’s own response area is it’s first due. Responding on a box as the second closest station you are 2nd due, and so on.

jslost I don’t believe it’s short for any other word. I’ve always thought about it in the same way you would talk about bills being due. If your car payment is due on the 1st of the month, and it’s the 1st, then the bill is due to be paid today. If your fire engine is responding to a fire down the street from the firehouse, it’s a first due fire. Beyond that I don’t know the origins of its fire service usage.

Eli McKenzie In Delaware all emergency services may use red and blue lights, if they choose to do so. Medic 102 is a Sussex County, Delaware unit. In Maryland Fire and EMS cannot use blue. Delmar’s units are registered in both States, therefore cannot use any blue.

Motor Vehicle Collisions are on of the most common because they investigate the situation and in many areas also perform traffic control. Police also respond for overdoses, suicides, unattended deaths, assaults, shootings, stabbings, situations in which we have to force entry into a house, unattended minors, industrial accidents or any other generally unknown/suspicious circumstance situations.

ATTACKER45 THE BEAST: A majority of Fire/EMS is volunteer in the US. Some of these departments are combination and rely on paid, in-house staff and volunteers. Often times, a call will come in and the in-house crew will respond first. Additional personnel would be notified via pager, radio and if outdoors, the county/city siren.

Some places use them to warn people that emergency responders will be on the road, there's a small City a few hours away from me that has a volunteer fire department and they use the siren to get the volunteers to the station.

The rural town I grew up in NC has a siren at the station like that, but likely for a different reason. It's a volunteer fire department, and many of the firefighters are farmers, working in fields, chicken houses, hog houses, turkey houses, etc. The siren would be an immediate alert audible for miles to all the volunteers. They have pagers, radios, etc. But this was an extra way of alerting people since there's no crew at the station waiting to respond like full-time firefighters.
