Episode 173:Glomerular Filtration Rate: When your Kidney Number, eGFR, is the wrong thing to focus on. Estimated Glomerular Filtration Rate (eGFR) is the best test to estimate your level of kidney function and determine your stage of kidney disease. Your doctor can calculate it from the results of your blood creatinine test, your age, body size, and gender. Your eGFR tells your doctor your stage of kidney disease and helps the doctor plan your treatment - but it isn't always the right number to be focused on. Dr. Rosansky discusses WHEN your eGFR is the wrong number to focus on and explains what you should be focusing on (such as protein leakage). Dr. Rosansky is a retired Nephrologist and author of the book Learn The Facts About Kidney Disease.
📚 Purchase Learn The Facts About Kidney Disease by Steven Rosansky at go.dadvicetv.com/book
Learn The Facts About Kidney Disease by Steven Rosansky written for the average reader, offers useful information for patients with very mild Chronic Kidney Disease (CKD) to those patients who need to plan for dialysis or kidney transplant. It offers scientifically proven ways to slow progression of Chronic Kidney Disease, including a chapter on a Smart Diet for all Chronic Kidney Disease patients. This diet not only can slow Chronic Kidney Disease progression but can also help patients to live longer and better. It offers the best treatments for the medical problems that can come with a diagnosis of Chronic Kidney Disease. For most patients this book will alleviate concerns about having Chronic Kidney Disease and for some patients with advanced Chronic Kidney Disease it offers an approach that can delay the start of dialysis for many months or even years in some cases. It explains why low and very low protein diets, promoted by many kidney books for patients, is usually not beneficial and may even be harmful. It provides the latest recommendations for treatment of Chronic Kidney Disease that comes from international kidney doctor organizations. The book is organized in a way that provides information for all patients with Chronic Kidney Disease in the first part of the book and has information in later chapters for patients whose Chronic Kidney Disease has progressed.

I am glad I stumbled and subscribed to this channel. I'm 43yrs old African . No diabetes and no high blood pressure. Also, no protein in urine and urinalysis results is normal. Blood function with eGFR is 64 & creatinine is slightly above the border range. eGFR was 77 about 10 months ago. Local doctor said I'm okay and not to worry when I visited yesterday. I'd like some advise here to take up the low protein diet plan OR just live normal healthy lifestyle. Pls advise me I'm so worried. I also don't have any symptoms. Thank you.

Just happened upon the show at the perfect time it was great and informative as I only today got full bloods done. 52 years old do a lot of training still, slightly raised hematocrit and hempglobin been that way for years. High creatinine 117 but in normal range, high creatinine kinase through muscle loss with egfr of 61! Zero symptoms. Should I be concerned? Or are my variants off setting the equation? Iv not had urine test recently but last one few months ago was protein free
@DADVICETV
@DADVICETV Год назад
Welcome! You kidney function is slightly below normal for your age, and most likely they won’t be a problem by keeping your blood pressure in check, staying active, and living a healthy lifestyle. You will want to see if you are leaking protein in your urine - that is one of the best indicator of possible future trouble. All in all you and your doctor should be able to easily keep things in check with very little effort.
@scraggs11
@scraggs11 3 месяца назад
Muscle size does mess up egfr, creatinine k etc. try getting your bloods done after 2 weeks rest from heavy exercise. Dehydration is also a factor blah blah blah get the proper tests done and do not go on and E= estimated result 👍

There was a time when doctors put people on dialysis with such a high eGFR - but they have now learned that it is actually better for the patients to manage the disease until the eGFR is much lower. I encourage you to ask your doctor WHY he/she wants you on dialysis with such a high eGFR. There may be a good reason...or your doctor may be using old methodology.

If you search hard enough online you'll find lots of places claiming all sorts of amazing magic. Sadly, none of these are proven or based on real science. I wish there was a cure - but these are all scams looking to take advantage of people desperate for help and drain their wallets and bank accounts. Anyone who does come up with a cure will be a Trillionaire overnight - and word will spread so quick everyone would know about it within days. The scammers are very tricky. Every week my mailbox and comments are slammed with post from the scammers offering cures, restores, heals, boost, etc.

While everyone is different, it could take decades for your GFR to get down to where most people should be starting dialysis. The key point would seem to be that it is the presence of protein in your pee , at first the microscopic albumin ( microalbuminuria) , is the time for real concern BUT I have seen videos by the likes of Jason Fung who say that the very early appearance of albumin in your urine can be treated with fasting and other life style changes. Only when you get macroalbuminuria -large amounts-will fasting fail to treat it.

Hi. I have bllood and proteinuria leaking. What kind of fasting? Full day or 16 hours kinds?
@michaelhussey440
@michaelhussey440 11 месяцев назад
see video ''Can Fasting Help Reverse Kidney Disease (2022) | Jason Fung'' where he talks about the 4 day Fasting Mimicking Diet ( which was made up by Valter Longo.) I think he says that it only helps the earliest stage of protein in urine. Do your research on that diet and make your own decision , consult your doctor too.

My nephrologist told me to hydrate myself very good before making the next labs, because he said: if the creatinine gets back to normal (from 1.1 ... female over 55+, automatically calculated egfr 53, BUN 22, no albumin in urine and normal albumin in serum) there is no kidney disease at all. It is then just a sign of not enough hydration and the cause why the creatinine and the BUN was climbing up a bit. His further explanation: because you can't "drink away" a real kidney disease. Are you his opinion too? Thanks and nice regards!
@DADVICETV
@DADVICETV 2 года назад
It is possible to over hydrate and dilute the amount of creatinine in your blood, making it appear as if you are improving. But that said, dehydration is one of the most common causes of low calculated kidney function. Ask your doctor how much fluid you should drink and drink that amount the day before and the appropriate amount the day of your labs. If things look good, your doctor may be right and you were just dehydrated. It’s very easy to be dehydrated and not feel thirsty. good luck!

@DADVICETV
@DADVICETV Год назад
I would be concerned if you are leaking protein in your urine. If you are not, there is a good chance you just got it at a lower moment (it fluctuates throughout the day and is impacted by things like your hydration level). The accuracy of an eGFR above 60 is very poor, most labs are now only saying >60 and not even giving a number above 60.

@@jasim4955698 I've cut out all dairy. I try to keep sodium low and protein relatively low as well. And only good sources of protein. I eat vegetable wraps at 1pm with arugola, radish and paprika, or sometimes with eggs. Which is the best source of healthy protein. at 6pm I eat rice with vegetables and fish or turkey. I am not counting calories, or the exact amount of protein. Snacks should be apples and kiwis. But I also still eat chocolate and even drink wine still. I use two supplements: vitamin D3 and Vitamin k2 (mk7 from natto). I was surprised that my renal function improved with 11 points just by cleaning up my diet a bit. My doctor thinks I may have a kidney desease just because my creatinine is high and my GFR low even though I told her repeatedly that I do weightlifting and have a high protein diet along with creatine supplementation
@DADVICETV
@DADVICETV 2 года назад
Your GFR calculation is based on creatinine, so if you take weight-lifting supplements that contain creatine or have a lot of muscle mass, your creatinine will be higher. Creatinine isn't bad - it's created by your muscles. There are other tests your health care team can do to check your kidneys - I'd look into that. Keep an eye on your urine - protein leakage is a key sign of kidney issues. All in all, it's never too late to look at your diet and lifestyle to see if there are things you can do better, especially for your heart and kidney health.
