There is site link extension and image extension should I used them in my Call Only Ads?

nope they wont display. Only website, callout, snippet and location extensions can display with call ads

Question. Im running a google search ads campaign and outperforming anyone else running ads by budget and CPC. My ads would always show up on top when i did a google search. but now they dropped to the middle of the page. im still bidding above my competitors why is this? would like to hear your thoughts thanks.

When you search for your ad from your computer google notes that and will stop showing the ad as it deems you not someone who is interested in purchasing the product or service. This could come in the form of lowering your ad down the page or simply removing it. Checking your ad regularly also lowers your click through rate which hurts where you rank and should only be done inside google ads and not through search. You can see how often you rank at the top by looking at impression share abs.

Can you ever get rid off competitors clicking your ads? So far i have followed your videos and generated a lot of customers,but i have noticed that only 10% of my shown clicks are actually calls.

Unfortunately, you can never get rid of your competitors completely. You can use software like clickcease but if your only using the normal google search network (not partner networks) I generally don't see a great return on it. 10% isn't a bad conversion rate either I would aim for 20-40% but 10% is a good starting point, especially with call ads.
