Sir, a patient (23 year old male) came with sudden onset of excruciating pain over both foot. He can't walk. He had past history of fever for 10 days. He has similar pain over both wrist. Can this be a acute gout. He has no warmth swelling redness ovee the foot. Can acute gout present with all toes of foot.
Sir my ultrasound report shows that I am suffering from fatty liver grade1 and cystitis. For cystitis,my doctor has given Ciprofloxacin500 for 7 days ( 2 tablets per day). I got relief when I using the medicines along with Neeri Syrup. Sir, Ottawa emergency guidelines mention that the use of steriods in acute flare as first line drugs can cause reboud gout flare attacks. Will be grateful for your insights in it. Regards
Sir your videos are always worth watching the information is precise and practical. sir i wanted to ask whether the the duration of therapy for oral prednisone in acute gout and the duration of therapy with neproxen. the dosage is mentioned but the time frame is not.
In acute gout flare: NSAIDs - Naproxen 500mg BID, Ibrufen 800mg TID, Indomethacin 50mg TID colchicine 1.2 mg loading dose then 0.6mg one hour later then 0.6mg PO daily until two days after reduction of gout flare. Can use in combo with NSAIDs Steriods (2nd line ) : as mentioned above. Rebound gout flare is common with steriod treatment. Resource : Ottawa emergency medicine guidelines.
