The question to me then is this: Are there any active people who perhaps lift weights as part of that activity who are of a healthy weight and still develop diabetes? Or is exercise, particularly strength training, the path to avoid this problem?

It seems that something is being left out regarding muscle insulin resistance. Roy Tylor mentions that either a muscle cell is insulin resistant or it is not. He does not explain what causes that resistance. According to the NIH: "Lipids stored in skeletal muscle cells are known as intramyocellular lipid (IMCL). Disorders involving IMCL and its causative factor, circulatory free fatty acids (FFAs), induce a toxic state and ultimately result in insulin resistance (IR) in muscle tissue". The way I see it IMCL fat accumulation happens in muscles before the the liver is involved.
