In this video, which is really designed for my cervical coaching clients, I will show you how to test 4 muscle groups in the upper extremities, in attempts to find muscle weakness. Why do I care about muscle weakness? Because if you have nerve root compression from cervical stenosis and/or cervical disc herniation, muscle weakness is a sign that the nerve(s) is(are) in distress and should probably be decompressed more quickly - all depending on your other neurological, orthopedic, and history of injury findings.

I have found a new addiction watching these very informative videos presented in a easy to understand format. Much to my surprise I have also found that much back pain relief can be gained by carefully experimenting with stress free body positioning and support which appears to aid recovery or at least help with comfort until professional care is available.

Thank you Dr Gillard, for all that you do! I have learned quite a bit about reading my MRIs, spinal formations, and more from your videos. Are there any recommendations you could give regarding upper cervical nerve/strength testing? Although I have several lumbar issues, the foraminal stenosis in C3-C4 is listed as severe bilaterally now (ACDF in 2018), and C4-C5 is severe L/moderate R. It would be great to watch for signs of deterioration at home, since Neuro visits are so very expensive for a fixed income. Thanks again sir!

RU-vid recommended this video due to my searching for information on cervical stenosis. What I found interesting and rather ironic was that you have Guitars in the background in your video so I assume that you play guitars. I also enjoy playing guitar, but as I practiced more and more, I started finding out that I was not able to hit the strings with even pressure and in the exact order when playing or arpeggiating a chord. Over the years, my neurologist has done the same tests that you described in this video and I’ve always passed him with flying colors, except I still have the problem with fine motor skills and moderate pain and achiness in my arms. So I’ve been diagnosed with moderate to almost (borderline) severe stenosis in C-4 C-5 C6. my main symptoms are a loss of fine motor skills and pain or aching in my lower neck radiating down into my right arm (radiculopathy). For example, trying to hit the strings on my guitar strumming problems with dropping things when I reach to pick them up don’t quite have them securely and I end up dropping them but I still have plenty of strength. I just wanted to bring this up because they are so many different degrees of symptoms with this disease and my symptoms seem to be stable for now. I’ve been told I’m a candidate for surgery by neurosurgeons and I’ve been told that I don’t need surgery by neurologists, but I don’t feel that any doctor can know exactly how I feel in terms of pain, discomfort, lack of fine motor skills, because there aren’t very many test to demonstrate this. They want me to get an EMG but I don’t think that’s going to show anything either the last one I had showed nothing -no deterioration in nerve function whatsoever but I still had pain and lack of fine motor skills. I enjoyed your video. Thank you for posting it and keeping everyone informed and teaching.

Regarding muscle testing, in the video, you tested T1, finger abduction (T2, finger adduction) rather than C8, finger flexion?

Hello from Puerto Rico. I’m heading in for an MRI soon. I had one a couple of years ago with no cord compression from some significant stenosis. Given my recent symptoms it sounds like I may have some Myelopathy. I am not asking for a diagnosis but about the time I felt some symptoms all of the joints in my hands where I have arthritis become inflamed and painful. It has been some weeks and the joints are still painful. Have you heard of patients experiencing the same experience? Thank you…

I've learned so much by watching your videos. Are you able to differentiate between the weakness in an arm related to cervical spine Vs the same weakness attributed towards carpal tunnel? i.e. picking up a saucepan / frying pan off the stove/cooker top. (whole arm not forearm)

Its pretty easy to differentiate the two. Patients with carpal tunnel syndrome typically have pain in the wrist and hand that wakes them up at night, while this is not typically true of patients with median nerve radiculopathy. Carpal tunnel patients will typically have a Tinel's sign, where radiculopathy patients will. And then the history of injury can usually differentiate things as well. People who work repetitively (especially forcefully repetitively) with their hands and wrists are at risk for carpal tunnel syndrome.

Hi thank you for all your videos they are very informative. I recently got a cervical MRI and it states I have a congenital fusion at c2 c3.. Do you have any videos about those?

Those are called congenitally block vertebra and may or may not be a cause of nerve root compression or facet irritation. The biomechanics of the spine is a little messed up because of the block which will put pressure on the L3 disk and facets. Again, this may or may not lead to problems now (cervical spine pain + headaches) or in the future. I have a video on congenital anomalies but that's of the lumbar spine. Google cervical block vertebrae or blocked vertebrae that should pull up more information :-)

Hello. Hello doctor. Can I ask I have numbness and loss of sensation in my both palms along with some relative mild muscle weakness in my fingers. It feels like I have pressure in my palms making it a bit difficult to move my fingers. I also experience tightness in the diagram, balance problems and hyper reflexes in my legs. When I move my neck it feels like a nail of sort push in the spinal canal then I get light uncontrolled movements in fingers, weakness in hands and feet It have a constant feeling of vibration in the lower part of the body. also sometimes when I lay down and try to sleep it feels like I’m drowning. I’m scared What do you think it is

Hello Dr.... I hurt my neck sometime ago, it felt really painful then, but now after like over a year, i still feel the pain, and it is now affecting my throat, My shoulders, chest and Back spine, even also my head.. I'm having weak muscles not only on my neck, but all over my body.. I did a standing Cervical Xray and it showed Normal findings, but the pain and symptoms is killing me everyday.... Please i need your advise on this DR Thanks alot....

Please take care of you, how about an MRI. I have a brother who got paralyzed after having these symptoms and didn't take care of it properly. So you need to get doctors care to find out what is the damage and treat for that.

Had wife help me and I failed every one with least resistance barely touching me. I do have cervical stenosis shown through MRI. I've been using sounders traction machine.

My fingers on both hands always fall asleep all 5 but either 3 on 1 side of hand or 2 on other side. Also can't open water bottles most of the time and have dropped a few jars out of nowhere with no warning. My Dr sent me to west Texas neck and back they did 5 treatments well 4 before I walked out told them cancel 10,000.00 treatments after waiting forever and not using pads during neck traction.

There's no such thing as a "c-8" disc 😂 It's goes from C-7 to T-1. There are ONLY 7 cervical bones!!!!

My cervical spine injury has resulted in stenosis. I experienced gradual weakness in my right arm, that has lead to paralysis of my right hand. My hand has atrophied and my right arm, from the elbow down, has lost all muscle tone. I am afraid that I've permanently lost the use of my right hand. I will send you an email, and I will include a photo.
