I think this was very helpful video and i think the work that you are doing is fantastic! thank you for posting these videos. I have taken gaba supplements in the past and i definitely notice the calming and happy effect. There's definitely a decrease in worry and fear when i have taken them. thanks for taking the time to educate the public about this. Too many people go on Benzos and suffer bad results from them. It's nice to see a psychiatrist that is promoting other means of reducing anxiety! thank you.

Jose, I'm glad the video was helpful! Yes, benzodiazepines are overprescribed, associated with multiple side effects, and highly addictive. Why doctors are overprescribing this is beyond belief.

No you don't. You've simply accepted his incorrect reasoning as true without any independent thought. The amygdala is not the fear center of the brain. Your thoughts are the fear center of your brain. You get anxiety attacks because of your thoughts. Everyone knows deep down that this is true, but it's much more comfortable to believe in some magical "chemical imbalance", and that the treatment is to "block" anxiety with some supplements. The path toward a calm mind is that of reason and rationality, not "GABA". Did the Buddha tell people "you need to eat some supplements to get more GABA", or did he explain the nature of suffering and the path to "nirvana"? I'm not a Buddhist - just an example. I don't think there could be any greater example of irrationality than that of choosing to take supplements to boost GABA, over walking a path of contemplation and consciously resolving your fears, guilts, regrets etc.

Everyone should not start blaming the doctors. If you go to the doctor and you tell him or her that you are having a massive panic attack, well the doctors are quite limited in what they can prescribe to you, so that you can experience relief. Now a days most will start you off with an antidepressant and if they doesn’t work after the first month well the next step is Benzodiazepines. This is what they are taught in medical school and what would you like for them to do? The doctors themselves are limited. I did some research on different drug groupings. The amount of Benzodiazepines legally available for a doctor to prescribe on the U.S. market literally can fit on 2-3 pages (Hand Written) now take into consideration that amount of antibiotics an doctor can prescribed and you can fill 10-15 pages and even more. What if the doctor just tells you Sorry, I would recommend that you go out to the park and take an extended walk like for a hour or two and then tells you to do that everyday for a week and then come back to him. Is everyone willing to follow those directions and pay the doctor for his office fee? Let’s not kids ourselves, you walk into that doctor’s office expecting a prescription to be written for you or now a days sent directly to the pharmacy of your choice. The blame can go in either direction patient OR doctor.

Wow great video! I loved the visual aid. Generally speaking, all of the anti-anxiety properties of GABA are reliant on the idea that GABA is inhibitory. However, I have come across some evidence that show GABA to be excitatory in immature brains (before the maturation of the glutamergic synapses). What are your thoughts on this? Do you agree with that and if so, at what point would you say GABA becomes inhibitory in the brain?

Thank you for viewing! Yes, GABA is excitatory in immature brains, and the delayed expression of the chloride exporter from the neuron shifts the action of GABA to inhibitory. This process of shifting from excitatory to inhibitory is part of a cascade of brain maturation, which includes environmental influences. Great video. Have you ever heard of psychiatrists using valproic acid or Lamotrigine for anxiety disorders? I didn’t want to use benzodiazepines as I have been off of them for 8 years.

I am glad that you noted below that Benzos should NOT be taken for more than a week or two. As you noted, tolerance develops VERY rapidly. If one takes Benzos for an extended period of time, expect an extended period of nasty withdrawal if you ever decide to stoop taking them.. And I mean nasty.

Yeah ten years later I am still paying the price for my doctor's ignorance and arrogance. My whole like was fucked up. I would say no more than a week, even two weeks is too much. I was addicted after two weeks, in late 2007.

I am currently scheduled to get treatment for my persistent addiction in a few weeks, using a protocol of 8 hours a day of NAD+ infusion for 10 days, which has had remarkable success in treating alcohol, opiate and benzo addicts. I have researched this drug extensively and it appears to be some kind of wonder drug that has actually reversed *aging* in mice, if it is administered via the bloodstream. I also filed a formal complaint against my old doctor for what he did - or more accurately, failed to do - in being totally careless about my spiralling benzo use in 2008, even as *my use of a controlled substance more than doubled in six months.* Apparently, he's in trouble right now because of my complaint, and it must have been an unpleasant surprise, as it came right out of left field - and I am glad. I want him to know how badly he screwed up, and also ensure that he never does this to anyone else, and to spread general benzo awareness in the medical establishment. I see that now benzo prescribing is much stricter in Canada - if you go to the pharmacist even one day early, they won't give you the drug. Had such rules been in place in 2007-2008, I would have been spared a world of hurt.

@@squamish4244 good luck...I hope this works for you. Doctors need to be more careful about prescribing benzodiazepines, as addiction/dependence is the iatrogenic effect of long term benzodiazepine treatment for many people exposed to such ill-advised practices.

I have been diagnosed with GAD around 3 to 4 years ago. Have had benzos, for i guess roughly one week to normalize my sleep. after that i only take them for severe panic attacks but I havent taken them for 2020 yet

I'm 54, I think I might be alcohol dependent. I have been hitting the whiskey hard in the past few months. I think I might be at risk of delirium tremons. I went to the hospital thinking I'm going to have a heart attack, or stroke. They performed an ecg,and said everything looks normal. But then the Doctor noticed I was shaking, like a tree in a hurricane. He asked me if I drank.,i said I did he turned to the nurse and said this man is at risk of a seizure. He gave me some tablets presumably benzos. He then said do I have any alcohol on me, I said I have some in my backpack and went to hand it to him, expecting him to take it away. He said no, you need to drink some of it now. I was shocked, I'm frightened. Do you think that herbal supplement that blocks reuptake might help me? I'm trying to taper on beer.

Thank you Dr. Carlo. I am currently experiencing benzodiazepine withdrawal after a long taper from being on klonopin and ativan for 2 years. The WD symptoms are horrible. Is their a way to naturally increase gaba production to ease the WD? Or is WD caused by a reuptake issue and not a lack of gaba? Meaning does a person in benzo withdrawal actually have less gaba or are they experiencing withdrawal because the up and down regulation mechanism is malfunctioning? Either way-is their a natural supplement and/or foods-drink that i can ingest to ease the withdrawal? Thank you so much in advance

Two benzodiazepines taken together for a long time makes your body tolerant to the effects of the benzodiazepine, thereby requiring increasing doses over time to get the same calming effect. Basically, your body has become dependent on the benzodiazepines, given you are in withdrawal. Benzodiazepines have nothing to do with reuptake. Benzodiazepines target GABA receptors, which suppress the amygdala, and hence decreases anxiety. But when you suddenly take away the benzodiazepine (in your case 2 of them) or decrease them too fast, then there is a rebound effect where the anxiety that was suppressed previously is now worse than where you started. I would suggest you follow-up with your psychiatrist to get expert clinical advise on how to address your withdrawal symptoms, which can become a more urgent medical problem if a psychiatrist is not managing your symptoms. Hello Dr, I have a few questions to ask you if you are able to answer please? I have recently started taking Escitalopram for generalised anxiety disorder. 5mg. I would like to know if this medication changes or damages brain chemistry as articles / videos I have researched have said they do hence the withdrawal/ discontinuation symptoms. Also how safe is it to taper of this medication in the long term as 5mg is the lowest Dose so how would you wean off from this? Thank you

Benzodiazepines are not meant to be taken beyond a few weeks, as they lose efficacy over time (tolerance) and you start to develop withdrawal symptoms when you miss doses (dependence). As such, it takes ever increasing amounts of benzodiazepines over time to get the same anxiolytic effect. While benzodiazepines are good drugs to take for anxiety over the short term, they have too many side effects and addiction can result over the long term. @@CarloCarandangI was prescribed Ativan for insomnia after two years of not sleeping due to menopause. I was warned that it was addictive and to use it sparingly. 60 mg over two years was enough to down regulate my gaba receptors ...i started having heart palpitations out of the blue. Doc said, I could take Ativan for that. Then I developed extreme anxiety...i could no longer do parts of my job, doc said take Ativan. Then I started having panic attacks and was prescribed more Ativan...then doc forced me on nine different psych meds to get at the root of my panic attacks! I had terrible reactions and side effects to each drug (probably because I didn't need them). I am almost five years off all pysch meds and benzos and still suffering the horrendous effects of lack of gaba. It affects every major system in the body. Fourteen years of my life ruined because I trusted my doctor. The most I took was 60 mg over a year. I think taking it once in a while or as needed is just as dangerous as taking it for two weeks straight. Make your warnings louder and share with other prescribers. My doctor doesn't believe the literature I've brought him over the years or me. He insists that i need an antidepressant for my anxiety and panic...he ignores my physical symptoms, saying they are caused by anxiety...but no, i get different scary physical symptoms and they trigger panic, making it worse. Right now suffering from heart palpitatios, difficulty breathing, like an elephant on my chest, cant fall asleep because i stop breathing, hard to swallow, five days ago woke to TMD or TMJ apparently caused by stress (and self diagnosed as my doctor isn't seeing patients due to the pandemic). For the first ten years, no ER DOCTOR, pharmacist, therapist, psychiatrist ever thought to mention that it might be the benzo causing my problems.

Hello, thanks for watching. In regards to your question, discontinuing benzodiazepines will lead to upregulation of the receptor, and is associated with the withdrawal symptoms from discontinuing benzodiazepines.

Sir for bipolar depression I have taken sertraline, fluxetine, lithium carbonate. But no benefit. Valproic acid made me little better. But clonazepam make me cured and best. Can i take it for years?

SSRIs take up to 6 to 8 weeks to work, as the post-synaptics effects take a while to translate into reduced symptoms. The last part of your question, more GABA more dangerous than more serotonin, is inaccurate.

Thank you, Dr for this video. I have a question. So what if GABA binds to the G-protein channel (another GABA receptor), what happens? Would it also play the role of the chloride channel for anxiety by inhibiting neurotransmitters?

A slow response to GABA is mediated by the GABA-B receptor, which is a G protein-coupled receptor. Hence, GABA-B receptors are much slower in response to GABA than GABA-A receptors, which are ligand-gated ion channels.

Great explanation! I do have a question about this topic though: how does the effectiveness of gabapentin compare to pregabalin? From what reading I've done, it sounds like gabapentin, although shaped like GABA, does not bind to GABA receptors, but pregabalin does.

Both gabapentin and pregabalin do not bind to GABA receptors. However, pregabalin increases GABA concentrations by inducing the production of GABA by increasing L-Glutamic acid decarboxylase (GAD), the enzyme that produces GABA. Both gabapentin and pregabalin are not considered first line treatment for anxiety disorder, as both are not robust in effectiveness. What’s the difference between serotonin and gaba since they both effect the amygdala? For me when I took gaba supplement the effects weren’t impressive at all. However with the serotonin it’s more noticeable. Thanks for the video!!

GABA-A receptors are ligand-gated ion channels, while GABA-B receptors are G protein-linked receptors. Benzodiazepines target the GABA-A receptor. To answer your second question, agonists cause ligand-gated ion channels to open more frequently, while a positive allosteric modulator enhances the activity of the agonist even further on the receptor (ion channel opens even more frequently than with just the agonist alone).

couple questions...melotonin...which is made from serotonin...is GABA also related? i know that when dopamine is increased serotonin is decreased vice versa is there any affect with GABA? such as well would GABA increase naturally as melotonin does as the day turns into night? released to get body ready for sleep...second question... are you familiar with modafinil and its affect on GABA i heard it decreases it so if one were to take both some tyoe of GABA supplement or medication like a benzo in conjunction with modafinil would they cancel each other out?

The only way to increase CNS GABA is via benzodiazepines, supplements, and psychotherapy. Modafinil has no effect on GABA, and definitely has no place in the treatment of anxiety disorders. I'm currently experimenting a little with the Russian drug called "phenibut" (Aminophenylbutyric acid), which is sold as a supplement here in the west. I don't have much anxiety in general, but I do have some social anxiety at times. So I heard that this supplement works similarly to benzos (in effect), just without some of the negative drawbacks like blackouts. And so I find that it actually helps a lot. I feel totally calm, while my mind is still in the right place and functional. I can see how one becomes addicted, though. I sort of miss the calmness when it wears off. I've also read that these supplements can "support restful sleep", but I don't see any improvement when it comes to sleep, however. For me it kind of seems to work like alcohol, where it is easy to fall asleep, but then you wake up 10 hours later feeling like you've slept 6 hours. Cannabis, however, makes me sleep like a rock. When I smoke cannabis I get sleep with zero dreams, then I wake up feeling energized and ready for the day. And 6 hours of sleep can feel like 8 hours after smoking. Hope it is okay for me to mention stuff like this here. I find psychiatry, psychology and (neuro)pharmacology extremely interesting. Especially when it comes to illegal drugs, where we have little scientific research but a sea of user reports. I'm only 18, so I hope I'll be able to study something like this in the future.

I have been on Temazepam for about five years and I am finding the medication is loosing it's effect. I tell my doctor to put on another medication but he refuses and just keeps on giving me Temazepam. Sometimes when I take a Temazepam if I have to go to the bathroom in the middle of the night it wears off quickly and cannot go back to sleep. Can somebody tells me if this is normal?

I just bought two boxes of Lorazepam. There good. You have 1 milligram and 2.5mg tablets in the UK oh and 0.5s,it was cheaper to buy the 2.5s..hospital pharmacy stuff. It's just I'm prescribed Diazepam by my GP. But I've been on them too long and am on them for dependancy. I've found that even though I've not done much Lorazepam in my life as much as this,but what I notice is that if you take X amount of Lorazepam you feel a lot happier than on Valium/Diazepam. I'm going to be honest with my Dr on Friday.

Maybe this is spoken of in the video.. I haven't watched it all yet.... But I've heard gaba can't get into the blood/ brain barrier... I've also taken the supplement for Gaba which didn't help much at all. I was prescribed gabapentin, and it does work. ( it's a nerve pain treatment, that also is prescribed off label for bipolar disorder, anxiety ect) .

That is a complicated question, with the answer an dynamic interplay of genetics, environment, thought appraisals, and coping skills. The downstream result is anxiety and depression. These treatments just serve to alleviate some of the symptoms.

That is correct. But the overall effect of alcohol as a CNS depressant explains some of its effects on intoxication, amnesia, and tendency to stumble. Basically, alcohol goes too much in as a CNS depressant, and adds to the addictive potential from alcohol use.

That's incorrect. I'll have to do another video to illustrate further. Briefly, in anxiety, the amygdala is hyperactive. The amygdala is modified by serotonin neurons and GABA neurons. Increasing the signal of the serotonin neurons suppresses the amygdala. Additionally, increasing the signal of the GABA neurons supressed the amygdala. So suppressing the amygdala leads to decreased anxiety, either by increasing serotonin or GABA signals. SSRIs increase the serotonin signal, while benzodiazepines and CBT (cognitive behavioral therapy) increase the GABA signal. That's why anxiety is best treated with a multimodal approach, using SSRIs, benzodiazepines, and CBT to eradicate severe anxiety, such as panic attacks. SSRIs do not affect GABA. Benzodiazepines and CBT do increase GABA. Thanks for your question! Stay tuned for a video on this topic...

The only way is slow taper of the benzodiazepine over a period of several months, under the guidance of a psychiatrist, who can help to address the withdrawal symptoms based on what type of benzodiazepine you are taking.

GABA taken orally as a supplement is problematic, as ingested GABA does not cross the blood-brain barrier. However, there is a study showing that oral GABA is helpful for anxiety. But why take something when it does not reach your brain? It is the brain that contains the hyperactive fear circuit (based on the amygdala) which needs to be suppressed. If you take benzodiazepines, over time, you will develop a physical dependence on them, where your body's equilibrium pivots to accommodate it. Hey doc, does green tea increase gaba levels? I smoked weed regularly for about 3 months 4-5 months ago and I regret it. I’m 15 and I have a strong suspicion that excessive marijuana use is a detriment to intelligence. I’m trying to fix it by drinking green tea, is this feasible? Thanks.

That was just a hypothetical case...if GABA were not present, then benzodiazepines would not work. Benzodiazepines bind to the GABA receptor with GABA to open up the ion channel, which then leads to a reduction of anxiety.

I have been suffering vartigo and anxiety disorder last 9 years but I can't go out of home and feeling fear and vartigo . My Dr said I take tables life times , if I take tables vartigo not came but I can't go out .mix depression also. Can u get rid of me it sir. I feel signal problem

Tel me another point please whether gaba supplements are helpful or not. As u said it can not cross blood brain barier, so it means this is useless product made by now foods or other companies?
