How To Actually Resolve Reactive Hypoglycemia

Are you having hypoglycemia or reactive hypoglycemia? Maybe this is a new thing for you or you're just trying to figure it out. My name is Dr. Terranella. In this video, we're going to look at some approaches to take for resolving reactive hypoglycemia, including both obvious and not-so-obvious approaches.

So in this video, we're going to look at resolving reactive hypoglycemia. In previous videos, we laid out some of the symptoms and tests that might be helpful in identifying if you have reactive hypoglycemia or hypoglycemia in general. But I wanted to point out a few other things on this topic before going into the strategies on resolving it.
First, I think it's important to point out how the symptoms and the problems from hypoglycemia can linger in our bodies for far longer than the actual blood sugar episode itself. This is because there are compensatory responses that take place in our bodies in order to resolve the hypoglycemia or low blood sugar levels. And those swings from high to low can still be felt in our bodies hours later. You can think of it like dropping a stone into a still lake.
The larger the stone, the larger the waves, and it will start right when the stone drops in. And as the initial wave spreads out further and further, you notice it less and less, but the compensatory mechanisms may still be going on. So in this case, the stone is the meal or the glucose rise that initiates the waves.
The larger the glucose rise, the more imbalanced or symptomatic our bodies become. Of course, our blood sugar is going to rise after each meal, but the question is how much is it going to rise? How quickly and how long does it stay elevated? The goal, of course, is to have the least amount of rise for the least amount of time and the lowest slope of that rise.
So now the question is, what do you do? The most obvious thing to look at, from my perspective, is the amount of carbohydrates that you're eating in any given sitting. Some people are going to need more based on their activity level, and you can get a sense for how much more or less you need based on using a continuous glucose monitor. One thing is sure: if you are having reactive hypoglycemia or hypoglycemia, you're eating too many carbs.
Sometimes it's more so the type of carbohydrate that you're eating, more so than the total quantity as well. So there's something referred to as the glycemic index of foods, which measures and categorizes how quickly those carbohydrate sources raise your blood sugar. Taking a look at this and limiting the high glycemic index foods will go a long way to reducing these reactive hypoglycemic events or hypoglycemia in general.
These are important, but other less obvious things to consider include the depth and the amount of sleep that you're getting each night and your overall digestive health. As far as gut health is concerned, there is evidence that dysfunction in your microbiome makeup can alter metabolism, specifically glucose, through some complex mechanisms that we're still trying to understand.
This dysfunction, referred to as dysbiosis in the digestive tract, is what seems to be causing this. With dysbiosis, there are altered microbes in the digestive tract, meaning you have pathogenic ones or problematic microbes that shouldn't be there in the amount or abundance that they are.
And it can lead to alteration and metabolism of things like bile acids and other products that can affect the metabolism of glucose. One example of this is people with a post-infectious IBS can oftentimes get a decreased barrier function, which is the digestive tract barrier also known as leaky gut. People with that problem, leaky gut, are more likely to have metabolic dysfunction as well, higher triglycerides, higher glucose, insulin resistance, et cetera.
So this is a diagram from a study that is basically looking at this specifically and seeing this is the normal situation. And here we have increased penetration of problematic microbes or microbial proteins coming into the [digestive tract].
The immune system gets activated.
And we could see the interplay of GLP-1 here, which is basically a satiety hormone, and the immune cells getting activated by these problematic microbes can basically lead to problems in the overall regulation of these signals.
So that's just kind of a basic look at that, and you can certainly check out this study. I'll put a link in the description for this one as well. You can get a more detailed understanding of that, but this is something that is being researched right now. More details will be coming out soon on this topic.
So while it's not fully mapped out and understood, having higher levels of pathogenic microbes and not enough of the good ones definitely has some negative impacts on blood sugar and insulin levels.

Thank you for sharing!! I’ll be bringing this guy health idea to my Endo. For years I’ve struggled with increasingly more severe reactive hypoglycemia. It was truly suffering last year with several lows a day and Dr.s and Nutritionists never helped. Despite I searched RU-vid I learned about Keto. Although I was very weak and queasy at first, the lows resolved almost instantly. That was 7 months ago. Now perhaps stress related I’ve found myself out of no where getting more hypos again the past month. Would absolutely love to learn more about how to prevent this without just “eat constantly” ya know?

I’ve struggled with reactive hypoglycemia for as long as I can remember. I strength train 5 times per week, and I have for the past 18 years. I started eating less frequently, and that seemed to help, but sometimes it still happens and I don’t know why. 95% of the time I eat whole foods, carb intake is moderate, not high or low. Almost all low glycemic carbs. I have had a bowel resection when I was 23 because I had hamartomas in my small bowel that caused an intussusception and subsequent necrotic bowel. Last year I was diagnosed with SIBO, took the recommended herbal antimicrobials and things got better. Just got tested for SIBO again and am waiting for the results, having lots of bowel issues again.

SIBO test was negative, also had a colonoscopy, completely negative including negative for microscopic colitis. Really baffling…

Why do carbs cause the hypoglycemia? Is it because they trigger insulin production, and the carbs are triggering TOO much insulin production with those experiencing reactive hypo? Thank you for this video!

Yes that is it. In some cases there is a delay in the insulin response and it over corrects

Im so glad i watched dr eric, this doctor and other different sources. I had reactive hypo, because i used anti psychtoics and antidepressends. Some of these medicines raises the bloodsugar. Within 6 days of keto and IF i was on 1 keto meal a day. But i aid some pure chocolade an hour later, because i thought i needed some sugar. The other thing was i took my medications with the meal. This al together raised my bloodsugar to quick and to fast en for to long insuline was released, which gave me to long low bloodsugar, with all the symptoms of a hypo. Then i took some proteïn and some water. The last one did the trick. At least i think. The water thinnes the bloodsugar and after an hour i was almost back to normal. Maybe the protein helped too. So try take a few glasses of water if you think you have a reactive hypo and avoid sugars in the beginning. If you really have a hypo, then sugar will help. Of course, consult with your doctor first

Perhaps, you have not been diagnosed yet as prep-diabetic or diabetic. You might be insulan resistant. Go get blood test for A1C and Blood glucose.

Re-evaluate what you believe is a ‘healthy meal’. Try just eating meat and eggs. An animal based diet. Dr Chaffee on RU-vid is good to check out

If you want to know if you are having low blood sugar, then buy a finger prick blood sugar tester at any drug store and check when you are having symptoms.

@@jyusta1 I have. And worn a CBG monitor. My blood sugar is fine when having horrible symptoms. When blood sugar is actually low I have no symptoms. 😵‍💫

Whoa! This is the only video that mentioned your microbiome and underbaked reactive hypoglycemia after having exploratory surgery and I also had so many other fit issues after that as well

@@AR-k39 the microbiome issues can be tricky it is not clear that one bacteria is responsible or if it is bacterial overgrowth and if that same bacterial issue will cause the same thing in every person. The short answer is if you have digestive symptoms start by getting a detailed analysis and treatment for that problem.

I stopped having symptoms when I cut out the junk food. I still eat bread, pastas but I don't overdue it. It seems all the sugar in my coffee, the chocolate bars, milkshakes, etc was the root cause of my problem. My Macros are in check, I still eat carbs, just not to the degree I once did.

@@swintegrative Oh totally, a lot of people have it worse off than what I did. I suppose my poor eating habits finally caught up to me. I was initially going to try keto, but I knew cutting all carbs out right away was probably going to suck lol. So I cut all the junk food out and was slowly going to decrease my carb intake, but found out all that sugar was causing spikes and causing my sugar to spike and drop low quickly.

Ive had this issue for yrs, its the bane of my life..im forced to eaf hidious amounts of sugar..rotting m body and teeth.,So dysbiosis could be a cause..interesting. I have hypoglycaemia symptoms, but my blood sugar is within range when test it at home.

Cut out the sugar and eat protein and fat instead. Eating sugar just feeds the problem and makes it worse. It goes to high and then to low. Eat protein and healthy fat instead and you won’t have symptoms. Sugar is the worse thing for hypoglycemia

My diet is very clean and have currently taken it to a keto diet, however, I am still getting cramps just below the ribcage an 1hr or 2 after eating almost every "evening" which is strange. I have decided to take glutamine and collagen for the gut to see if it helps and that was before seeing your video. After seeing your video, it makes sense that it could be a leaky gut, but strange how the cramps only happen in the evenings?!.

ld love to hear your thoughts on dietary phenylalanine being converted to tyrosine to dopamine and how the pancreas uses it as it would relate to reactive hypoglycemia.

I have a severe case of this. Sadly, the advice in this vid (small frequent meals with low GI foods) does nothing to resolve hypoglycemia. I can only wish someone had a resolution.

Sounds like u have a serious case so I wish there was a simple solution I will post an update on this at some point there are some other things to consider but I didn’t recommend small frequent meals… low GI yes but not small frequent meals. In most cases there is strong endocrine component. Good luck

@@swintegrative Thank you for your reply, I appreciate it. Although I have not found any actual resolution, I have compiled a list of things that often (but not always) help. This includes a CGM, pickles before meal (vinegar), exercise after meals, interrupt the crash (after the peak) with complex carbs, etc. I'd be happy to share that if you are interested.

@@swintegrative I am not offering any medical advice - this is just what I have experienced My personal post prandial hypoglycemic observations and hypotheses: 1- NOTHING WORKS EVERYTIME! (Due to multiple variables) 2- Use a CGM to understand your patterns. 3- The glucose change slope is very important a. Mild slope means less deviation from 100 b. Steep slope means severe highs and lows are likely and predictable 4- Severe highs and/or lows lead to successive highs and lows 5- 100 is great, 150-160 still feels great but may be a warning for a pending crash 6- 70 is bad if it is preceded by a sharp drop 7- 70 is fine if it is not preceded by a sharp drop 8- Once hypoglycemic symptoms occur, they can persist even after levels normalize: a. Regardless of immediate glucose rise b. Need caution to avoid successive high/low swings 9- I Currently experience difficulty with night levels a. Tendency to bounce along above and below 70 b. Tendency to drop below 65 and pop back up 10- Tips and tricks that often work for me: 11- Stay hydrated 12- Include fiber 13- Include protein 14- Include complex Carbs 15- Interrupt sharp rises (exercise, walk, etc.) 16- Do not sit/relax after meals - Move! 17- Vinegar before meals (pickles!) 18- Cook then refrigerate carbs. Reheat to serve. This reduces rise/fall. 19- Avoid high carbs without fiber and protein 20- Eat some complex carbs mid evening to level out the night lows

Dr Berg suggested eating some cheese when this happens. He says the protein and fat may help. He did this when he went low carb to resolve the issues of hypoglycaemia.
