To make a claim for PIP, British Sign Language users can use the video relay service. To find out more, visit www.gov.uk/pip/how-to-claim.
You or your appointee can make a claim for PIP by telephone. Our Freephone number is 0800 917 2222. or textphone us on 0800 917 7777.
You can also claim by post, but this may take longer.
Write to: Personal Independence Payment New Claims, Post Handling Site B, Wolverhampton, WV99 1AH.
When you make a claim for PIP, we will ask you for some personal information.
It would be helpful to have this information ready before you contact us. You will need to tell us your date of birth, your contact details, including your address and telephone number. Your National Insurance number, which you will find on any letters about tax, benefits or pensions.
If you have applied for a National Insurance number, but it has not been provided yet, you can still make a claim.
We also need your bank or building society details. The name, address and telephone number of your doctor or health worker. And the dates for any time you spent abroad for more than four weeks at a time and the countries you visited.
After you have claimed PIP, we will ask you to complete the form ‘How your disability affects you’.
You should complete it and return it to us within one month. If you need extra time to send it back to us, contact the PIP enquiry line to let them know. Their number is 0800 121 4433 or textphone 0800 121 4493.
There are some notes with the form to help you fill it in. There are also organisations that can help you fill in the form, for example, Citizens Advice or your local support organisations.
When you fill in the form, think about what activities are difficult for you to do during the day or night, for example: eating, drinking or preparing food, washing, bathing and using the toilet. Dressing and undressing. Reading and communicating. Managing your medicines or treatments. Making decisions about money. Socialising and being around other people. Working out a route and following it, and physically moving around and leaving your home.
Remember, your claim is not about your disability or health condition, it is about how your disability or health condition affects your everyday life.
We will ask you to provide additional evidence to support your claim. If you do not give us enough information, we may not be able to award you PIP or give you the correct amount.
Please only send copies of documents you already have available to you. Here is a list of additional evidence you may wish to include with your form:
• medical reports or treatment plans
• letters you may have from your consultant about your condition or diagnosis
• your current prescription list
• any care plans you have
• test results you have from scans, hearing, or vision tests.
Do not ask your doctor or consultant for a written health report if you do not have one already, as you may have to pay for it. If we need information from your doctor to make a decision on your claim, we will ask your doctor for it.
Here are some examples of documents that are not useful to us, so please do not send them:
• appointment cards or letters
• general information
• fact sheets about your condition or medication
• bus or train tickets.
When we receive your completed ‘How your disability affects you’ form, we will refer your case to a health professional with your supporting evidence.
