From an optometrist... The basics that every single dry eye patient should have in their "dry eye regimen": 1) Lubricating artificial tears... mild = most drops should be fine, mod-severe = use PRESERVATIVE-FREE drops, and try gel drops or ointment for greater effect 2) Eyelid hygiene... cleansers safe for use around the eyes are necessary to help control the flora that live on and around the eyelids. Bacterial overgrowth causes blepharitis, which is an inflammatory reaction, and that can aggravate dry eyes further by affecting the meibomian glands. 3) Blinking exercises... take breaks more frequently from your computer and digital devices and make sure you spend those ~20 seconds looking at something far away and blinking slowly and completely to spread that tear film evenly over the surface of the eyes. 4) Omega-3... try one specifically formulated for dry eyes. You need at least 1500-2000mg daily of EPA/DHA ingredient --- which can actually be quite difficult with most non-specific supplements on the market. 5) Hot compresses... the best is the Bruder Mask. Pop it in the microwave for 20-25 seconds and leave it over your closed eyes for at least 5-10 minutes. Your eyes will be blurry momentarily after removing it but that's because it's working. Those oils will melt and disperse onto the ocular surface, which is critical for lubrication. Start there and if you need more, visit your optometrist for a full consultation. There are many that have special interest in dry eye and can offer a range of options. Hope that helps everyone. :)

The oil that is expressed naturally from the Meibomian glands situated within the upper and lower lids is necessary to stop the eye surface getting dry. When these glands malfunction (as we age, particularly in menopausal women) otherwise known as atrophy, then the oil within these glands thickens and doesn’t get pushed out of the glands so readily. So, by applying HEAT to the lids, this softens the oils. Then the lids are squeezed gently to stimulate oil production. Then you sometimes feel that the vision for a moment is “oily” because there is a sudden expression of oil on the surface. So, when the glands are blocked, the eye gets dry. This is why copious use of lubricants on the eye sometimes doesn’t work because the ROOT cause is MGD (Meibomian gland dysfunction), not lack of aqueous tears. It’s important to understand whether your Dry Eye Disease (DED) is caused by low aqueous production or lack of oils (which sit in the top of the aqueous layer and stops it evaporating)., 1) Heat 2) lid squeeze 3) lid cleanse 4) lubricant drops.

@@carolgardens7294 buy an “eye bag” which you can get in microwave. You need more heat applied consistently for much longer to really soften the oils. From an optometrist of 30+ years working in eyecare

Thank you! Oh my goodness, I have been going to the optometrist for two years for dry eye and she hasn’t explained this to me! Only giving me different prescriptions! I definitely will try the heat pad on my eyes! Lol and to blink more!

@@cluckygirl792 Hello, may you kindly elaborate what you said "You need more heat applied consistently for much longer to really soften the oils." What kind of heat? How much longer? Do the oils get soft by themselves over time if I, say, apply warm eye bags over my eyes every day for couple months? Or is it just to suppress the symptoms of dry eye? Or can it be actually reversed for good? Google is always saying Dry eye syndrome is not curable :(

@@alexisarnould2715 dry eye is a chronic condition, something we can manage but not "cure" most of the time. heat should be in the form of a Bruder Mask or equivalent, which is a mask you put over your closed eyes with heated beads, 5-10 mins at least once per day. It should improve the flow of the oils and with regular use it should improve your symptoms since there is less blockage of the oil glands.

Wow, that was so informative! I have dry eyes & was only told to let the warm water run over my eyes in the shower & use drops. This doctor goes much deeper than that. I hope you have him back on again in the future.

There is a book, written years ago, about the Bates method of eye exercises that help all kinds of things = vision, lubrication, etc. I bought the book and don't do all the exercises but even doing 1 or 2 of them helps a lot!

I have tried everything over 30 years of suffering. The one thing that helped is sleeping in a mask which keeps the eye closed at night. Drinking water helps. Avoid sugar. Anxiety is related to dry eyes. Someone really needs to look into the connection with anxiety. What I think happens, the nerves get desensitized, then that stops the blinking which causes inflamed eyes and clogged glands.It is a system. So like type 2 diabetes. That is an overflow problem caused by eating too many carbohydrates, eating too much and too often. Insulin gets overproduced. That packs the fat into cells. The solution is to stop eating so much. Blood sugar goes down. Insulin goes down. There has to be a way to resensitize the nerves and reverse this inflammatory cycle. I found the drugs to be useless.

I believe there is a difference between dry eye disease and having dry eyes at least that’s what my ophthalmologist told me. I have a condition called chronic blepharitis, which does involve a lot of itching watery eyes especially in the summertime I’ve had an ophthalmologist or two and an optometrist recommend washing my eyes with baby shampoo and when you wash your eyelashes wash them vigorously but gently left and right left and right. This has helped me tremendously. Thanks for the great information!

Very good video, thanks. I would like to add that any problem affecting assimilation and absorption of oils and fat in the body will affect the output of the oil glands, including health life transition such as peri and menopause. Problems such as acute or chronic pancreatitis, aging which lessens overall digestive enzymes in the pancreas, fat malabsorption etc. Those root causes need to be addressed to help improve dry eyes. In addition, there are simple dry eye treatments that can be done at home and more electorate in the ophthalmologist office /clinic. Heating the eyelids, oils gland pressing or expressing, and such. Called probing at the eye doctor clinic. Very helpful

Weird how when I'm having issues, Bob-n-Brad come through with solutions. Today dry eyes! Yes. Dry indoor climate during the winter, even with a humidifier. Sometimes my eyes burn alot and yes, I do a little computering every day. I get those flakes in my eyelashes that are really hard to wash off. Tedious. Never heard of eyelid wash but am on my way to find some. Thanks!!!

Occusoft eye lid scrub is great they are individually wrapped it is a small towelett for your eyelids really helps with dry eye!!!

Yes, a priority “lid scrub”….. well, a lid cleaner is good. Highly recommend a lid cleanse daily- the order is: 1) heat applied to lids (use EYE BAG) 2) lid squeezing 3) lid CLEANSING (yes, you can use baby shampoo etc etc but a proprietary lid cleaning solution far better and easier and you don’t need a lot of it). 4) lubricant drops, ideally 3/4 times daily

Great Info. When I was in my late 20s (early 80s) the ophthalmologist told me I had dry eyes and I shouldn't wear contacts. That's it I did try them but it's true that my whites got red! So that didn't last long. Eventually, I only wore my Rx sunglasses and only Rx regular glasses when driving at night. My eyesight seemed to get better over the years, not worse. My astigmatism is what's bothered me the most all these years. I'm 66 now and I don't need reading glasses so much either. Hubs who is 58 can't read his phone nor computer without readers, .. strange things our eyes!

I find a morning cup of coffee really helps to lubricate my eyes, even if it's only temporary. Plus the Peep Club eye wand once a day, along with warm compresses twice a day, all really help (along with eye drops of course)

Netti pot 2 to 3 times a week cleared mine up. Eyes are related to your nasal passages and often when there is dysfunction in one it is actually the other that is the problem.

Wonder, if I didn't stare at screens til I was older, if it isn't as bad for us older adapters and maybe harmed younger users more as they stared at screens while eyes were developing?

Really interesting article and very informative, however you haven't mentioned menopause as a cause of dry eye in menopausal women. I am currently having severe issues since menopause (my eyelid actually stuck to my eyes at night and I had to massage to enable my eyes to open) It would seem that hormones are playing a huge role in this. Apparently estrogen protects the tear producing glands from the immune system attacking the glands causing Sjorgrens syndrome.

I cured my menopausal dry eye in about 1 1/2 weeks with krill oil. Cured it forever and never stopped taking my krill oil. I learned all this from Dr. Mercola on the internet.

A few seconds of warm compresses, drops of colloidal silver & then coconut oil slathered across eyelashes... I did that just a few times (I think it was actually 5 times) over 3 days & was surprised that I didn't have trouble with it anymore. ☮️💕🙏🏼

I got xanthelasma/ yellowish cholesterol lump, I swear it was the coconut oil, I had used for months, yes it was organic etc...now trying special castor oil 👁️ 🍀✨

I am 75 I have been listening to a doctor Bob on the radio for years and he always recommended can’t see eyedrops so I decided it was time that stuff is amazing and also there are vitamins that your eyes need that are also amazing no more dry eye

A very informative video indeed. I got dry eyes problem after LASIK and my surgeon has put me on some lubricant meds. But yet the problem does not seems to go away. What shall I do?

Hi. What you have failed to highlight is the link between MGD and DED!! The Eye Bag is essential to get the oils released, THEN cleanse then drops. Also that DED is very common in menopausal/post menopausal women (oestrogen). Please don’t forget us older ladies who naturally get gland atrophy…..

@@medhikasingh6327 Tesrs produced when crying are watery tears. They are not lubricating tests. In fact, when one has “dry eye syndrome” (we say DED = Dry Eye Disease)), the surface of the eye is more sensitive to the environment. For example, when it’s windy outside, cold outside, too dry outside, too dry indoors because of heating or air con…. And such environments cause “reflex tearing”, and this type of tearing is watery, NON- LUBRICATING TEARS.. Does this make sense to you?

Funny I've only had a smartphone for last three years while recovering from chemo in 2014/2015 so wondered if that was the main reason for it which I hadn't thought about. I think also my optician was probably too polite about the phone/computer use. It's probably a combination of the two then. So if have had chemo need to lay off the screens as well as learning to blink more. Funny I used to work at a computer for 10 hours a day before chemo. Many thanks.

Dry eyes can also be caused by PTSD if you dissociate. Find somebody qualified to help you release the stored trauma or traumas which cause this dissociating or numbing out. You’ll feel much better in many other ways too

I agree with other users here. Castor oil drops definitely helps. It's best to put on at night right before going to bed because it can cause blurriness of course. You use castor oil as eye drops so the process is the same as you would with any eye drops! I personally like to massage my eyes and around my eyes while I'm at it with the excess oil that seeps out. Castor oils also good for hair growth so dry eye relief and lushes lashes! WIN WIN! Lol

About eyelid scrubs, I was given some individually wrapped eyelid “scrubs “. Because they’re called scrubs, I really did a scrubbing motion and that led to several of my lashes falling out and growing back in crooked. Was I supposed to scrub or was I supposed to simply wash? Since then I’ve been washing gently and that works fine. The crooked lashes don’t seem to get better even after several years.

Dry eye can just be from aging, especially after menopause. It can cause tearing because there's not enough of the oils left, so your eyes water, especially when you're outside. I have heard there's a medical procedure to heat up those glands, anyone tried it?

Optometrist here. There are in-office procedures such as Lipiflow that use the process of thermal expression. They apply heat to the inside of the eyelids up to the melting point of the meibum (oil inside the glands), and then the meibum is gently massaged or "expressed" to help clear out the glands. It works far better than trying to heat up your glands yourself although it is still highly recommended for the majority of dry eye patients to use one of those heat masks! More recently, other procedures such as radiofrequency and IPL (the same laser that is used in treating rosacea) have been utilized in dry eye to try to decrease inflammation and improve the quality and quantity of meibum. My coping strategies are to gently scrub my eyelids with a washcloth, warm water and Johnson's Baby Shampoo a few times per week. Been doing this for years, and it does help. I also take Hydroeye supplements, but am also on a Rx of cyclosporine and alternate that with an herbal remedy drop. I also limit my use of eye makeup, and only use it when I feel self conscious of my tired eyes. I still struggle, but it's better than not doing these things.

I found drops would get on my eyelashes, dry, and then flake off into my eye scratching them. Taking the time to lay down with an eye bag can be a bit annoying. For 6 months I have been applying Non GMO Jojoba oil to my eyelids before bed and in the morning. And have never had dry eyes again! I can even miss a day and not have a problem. My unscientific thinking was that if the eyelid skin gets dry (like all of our skin does) it is going to pull moisture from where it can - and the closest source is the eyeball. Now that my eyelid is never dry, my eye balls have stopped being dry. To test, I stopped for one week. I couldn't open them in the morning as they felt glued closed with sand, and painful. But, even that first morning back with the oil, the pain was gone by the time I finished brushing my teeth, and I forgot they were even dry when I went out the door 30min later. The skin is the largest organ of our body and EVERYWHERE needs moisture, even eyelids.

I never had a problem with dry eyes until the pandemic when I stopped wearing makeup. When I used to wear makeup, I'd clean my eyelids with a cotton pad and baby oil. Well, I stopped wearing makeup, and then I "suddenly" developed not only dry eyes (especially my right eye. I'd wake up every morning and it would be horribly painful from dryness), but I got multiple chalazions. :( Now I'm strongly thinking of wiping my eyes with baby oil every night to see if it would help. Eye drops are not really helping. :(

I use a micellar makeup remover, will that clean the ducts enough? My dry eyes began after menopause when all my skin and hair got very dry. I will blink at red lights from now on 😉

No. It just cleans the surface of the lid margin. The “ducts” are internal. Go see a good optometrist that specialises in treating DED. It is so important to understand your eyes and why they malfunction and what you need to do to help them.

When I was a child, I was told not to read by my bedside light or I would end up needing glasses. But I had so much homework that I had to work anyway. And needed glasses. I read a historical book where someone recorded that reading at night, by candle was bad for eyes. I guess any reading or watching by artificial light is bad.

Too much screen time and failure to properly cleanse eyelids will certainly aggravate dry eye, but has anyone mentioned possible lack of sufficient dietary iodine? Iodine deficiency symptoms include dry mouth and dry eyes, among many others. Perhaps we need to eat more quality wild seafood .. seaweed, kelp, wild-caught cod, shrimp, scallops and other crustaceans. Some other sources of iodine include eggs, dairy, yogurt, grass-fed meats. If the thyroid does not get sufficient iodine, all other glands suffer, including meibomian and salivary.

I'm a 68-year old male. I've had dry eyes, at night only, for the past ten years or more. I use Bion Tears, which I find comfortable and effective, whenever I wake up at night with dry eyes, which is most nights. I've never worn contact lenses or eye makeup. Omega 3 OK; I've been taking a supplement for years. Whole food plant-based diet for 18 years. I think I keep my eyelids clean. So it's a great mystery to me.

You likely have nocturnal lagophthalmos. This is something your optometrist can test for - try seeing them first thing in the morning so they can use a diagnostic dye to take a closer look for dryness in your eyes. Some people, myself included, do not sleep with their eyes completely closed and that leads to exposure of the bottom parts of your eyes overnight = dryness and discomfort. Even if you don't get a chance to visit your optometrist soon, I highly recommend switching to using a lubricating ointment every night before bed (thick, like Vaseline, but that means it stays put all night). This helps a lot of my patients. Good luck!

Your body is ageing. Your glands ie your endocrine system will not be working as efficiently. And you have numerous glands on and around the eye under neurological control that need to work well to keep the eye moist. DED is not only present in females but it far more common online females.

I got rid of my dry eyes with liver capsules 5 per day. Mine’s was ver bad. My daughter suggested the capsules I reluctantly took them with in a couple of days it was gone. The thing is you have to continue taking them or it comes back.

Will medication cause dry eyes? I’m 30 and wear contacts and have never had an issue with dry eyes. Now I can’t even wear my contacts around the same time I took some meds. A few weeks later and the dry patch is still present (got it checked out and doc said it’s a dry patch - not dry eyes all over).

Can a dry eye be caused from using a CPAP? I have one eye that has a growth on it, and my optometrist said it was caused by a dry eye and recommended some kind of eye drops. I forget what he called the growth. I use a CPAP and I suspect that air is leaking and blowing near my eye. He said the growth could be removed but it would come back. There is no way to stop air from leaking from a CPAP mask. I use the nasal pillow type, perhaps full face mask would help, but I can't tolerate that. Any advice, other than stop using the CPAP?

I was surprised when I realized air conditioning caused extreme dry eyes for me. They get so bad that I can't read or sew. One day I vaguely remembered hearing from somewhere that air conditioning can dry out your eyes. I live in N.C. and it gets hot here, but one day I opened the window and turned on the ceiling fan and turned off the AC. After a while I realized my eyes weren't bothering me. As long as I'm not in AC I don't have a problem with dry eyes. I'm looking forward to the fall of 2023 so I can leave the windows open.

Clean my outer eyelids several times a day with very warm water... NOT THE EYES. Sometimes people actually think that I'm flushing my eyes. LOL. Sometimes watered down hand soap can be useful. Sometimes Baby shampoo. If you use one of those two, be sure to wash the soap out completely. Good Luck.

Use the baby shampoo, humidify at home, omega three caps twice a day. At work, I sit over a giant mug of steaming green tea for midday relief. I have also resorted to a steamy shower or a boiling pot of pasta when I come home. Try to get off computer/cell phone. Ditched the makeup years ago. After some years I had to resort to restasis, ultra expensive, hurts like you know what. In the winter I have the Dr add dissolvable plugs. It forces the thin and thick material to mix and helps with moisture. Reduces scrubbing which makes things worse. When you go for your annual physical get an ANA titer to make sure you are free of Shogrens or other autoimmune conditions. I have been lucky. Good piece Dr. Allen, looking forward to more...

How about a dr who did cataract surgery when patient went in for floaters. Dr said they had cataracts. Dr. said next day after surgery use eye drops you need them from now on ..WHAT? Now has dry eye for over a year. Went to new dr and said now to use eye drops . Have refused to use chemicals in eyes. Going back soon to try degradable eye plugs to see if they'll work. HOW ABOUT DRS. START TREATING THE CAUSE INSTEAD OF THE SYMPTOM.

There are so many factors to consider when looking at an “uncomfortable eye” and yes, omega supplements (excellent advice given by another optometrist on this blog) and better hydration are always recommended. But depends how much time your optometrist has….and how much he’s being paid….

Go see your optometrist! If your over 50 and female it can just be due to a dry ocular surface and this is triggered by wind/cold/bright light/hot room….but you should make sure you see your optometrist as there can be other reasons…..

Question: I have been seeing the floaters you talked about( I’m soon 65)but a few days ago I felt a twitching in the outside crease of my Left eye , kind of felt like a fluttering of that area but earlier this evening I started noticing something in my line of vision that I’ve never seen before. I would describe it as a spider web and it stands out as if I had 3D glasses on and my eye is very achy . I called an after hour emergency number for a local optometrist but unfortunately the “ mailbox was full.” Left a message with my eye dr hopefully he’ll call me in the morning. Just wondering if it is an emergency situation?

An optometrist is NOT a doctor. They are technicians that measure sight acuity and prescribe lenses to correct vision. An ophthalmologist are best that can tell you how to treat eye issues such as dry eyes or other eye health issues.

If caffeine is your friend then you most likely have dry eyes, If you are in smoke the eyes are doomed. Look for the cause. How much water do you drink? I have dry eyes but let me tell ya that recusing the caffeine to around 200 from 700mg plus drinking 3-4 liter of water a day has been a minor miracle. Keeping smoke out of my eyes.Today I never reached for my eye drops and that is not the norm. I also had lasik. Man when I quit caffeine my eyes will be juicy lol getting there an already feel better

Here in the 21st century, I was told by an eye surgeon that cosmetics & moisturizers cannot & do not get into - onto the surface of - our eyes & result in temporarily-cloudy vision. That any cloudy vision after using moisturizer is coming from inside the eyeball. I had asked for a safe way to remove makeup from our eyes when it migrates there. Wiping it with my grubby fingers didn't seem to be the best approach. "Are eye drops the best way?" The doctor's answer was that makeup never moves onto the surface of the eyes and never makes vision blurry while it's there. So I asked why blurriness happens only after I use moisturizer on my face. On non-moisturizer days or weeks, no cloudy vision. Another medical person (doctor? nurse?) told me back when I complained of dry eyes, "Unfortunately there are no oil ducts in the eyes." Even at the time, I knew that wasn't a good answer. Somehow some eyes for some years don't feel dry. Now, to hear confirmation those "experts" were wrong is wonderful. I am so tired of traditional doctors & "medical science." They are so full of crap.

I use vanicream mosturizers and cleansers on my face, and it realy helped me. I was getting excema flares on my lids. Makeup can cause flares leaving my lids red and swollen, so I have to be careful what products I use.

She was very detail oriented, so it was odd she was so wrong about such a little thing. Then she and my daughter laughed at how dumb I was NOT to catch on that it was on the inside of my eye. No, the moisturizer wasn't on the inside of my eye. Sheesh.

Title is misleading, you say in some cases there's nothing can be done? I think we really need to do research more,faster maybe care more in the country when it comes to eye health.
